Introdução: O desconhecimento do papel do terapeuta ocupacional na Atenção Primária à Saúde (APS), especialmente na região norte onde a inserção deste profissional é incipiente, pode figurar como entrave à efetivação da interdisciplinaridade e oferta de cuidado aos usuários. Objetivo: Conhecer a percepção dos profissionais de saúde de um Núcleo de Apoio à Saúde da Família (NASF) da região norte do Brasil, acercada atuação do terapeuta ocupacional no âmbito da APS. Métodos: Foi realizado um estudo de abordagem qualitativa com 15 profissionais de saúde do NASF-Águas Lindas, localizado no município de Ananindeua, Pará. Para a coleta de dados utilizou-se como instrumentação um questionário e uma entrevista semiestruturada. Resultados: A caracterização dos participantes da pesquisa demonstrou a precariedade e/ou a inexistência de contrato formal no vínculo dos profissionais deste NASF. A análise de conteúdo gerou três categorias temáticas envolvendo a importância da atuação do terapeuta ocupacional na APS, o conhecimento acerca da inserção do profissional nos programas de saúde e a percepção sobre as ações interdisciplinares envolvendo a Terapia Ocupacional. Conclusão: Diante da análise dos discursos, embora tenha ficado clara a importância da Terapia Ocupacional na APS, notou-se que existem restrições quanto à compreensão, em especial, sobre os propósitos da atuação do terapeuta ocupacional em programas de saúde da APS e no exercício da interdisciplinaridade. AbstractIntroduction: The lack of knowledge about the role of the occupational therapist in Primary Health Care (PHC), especially in the northern region where the insertion of this professional is incipient, may be an obstacle to the effectiveness of interdisciplinarity and offer of care to users. Objective: To know the perception of the health professionals of a Family Health Support Center (FHSC) of the northern region of Brazil, about the work of the occupational therapist in the ambit of the APS. Methods: A qualitative study was carried out with 15 health professionals from FHSC-Águas Lindas, located in the city of Ananindeua, Pará. A questionnaire and a semi-structured interview were used to collect data. Results: The characterization of the participants of the research demonstrated the precariousness and/or absence of formal contract of employment relationship in the FHSC context of the study and the content analysis generated three thematic categories of analysis involving the importance of the work of the occupational therapist in PHC, the knowledge about the insertion of the professional in the health programs and the perception about the interdisciplinary actions involving the Occupational Therapy. Conclusion: In light of the analysis of the speeches, although the importance of Occupational Therapy in PHC was clear, it was observed that there are restrictions regarding the understanding, in particular, of the purposes of the occupational therapist's performance in PHC health programs and in the exercise of interdisciplinarity.Keywords: Primary health care; Patient care team; Professional practice; Occupational therapy.

In this commentary paper, we highlight the key role that community health workers and family health professionals can perform for the identification and care for women experiencing domestic violence in communities. These workers are part of the primary health-care strategy in the Brazilian public health system, who are available in every municipalities and neighborhoods of the country. Based on our ethnographic research, we argue that identification and care of abused women by these workers and professionals follow a pattern which we described and named “the Chinese whispers model.” We also point gaps in training these workers to deal with complex issues, such as domestic violence, arguing for the need of formal qualification for both community health workers and family health professionals by, for example, incorporating such themes into curricula, further education, and continuing professional development.

ABSTRACT: Introduction: The Family Health Strategy (FHS) should be first-contact care in the Brazilian Health System. However, Primary Health Care (PHC) still encompasses two models: the FHS and the traditional health care facilities. The expansion of the FHS has been slow and heterogeneous in many cities, rendering a comparative evaluation of key quality-related elements of PHC models crucial. Objective: To compare the performance of PHC models as perceived by health professionals. Methods: A cross-sectional study involving managers and health professionals from PHC of a medium-size city in South-eastern Brazil. Data were collected by applying the Primary Care Assessment Tool. The performance was estimated through primary health care indexes (general and partial PHCI by attributes). Univariate polytomous logistic regression was performed to compare care model performances according to their attributes. Strength of association was estimated by odds ratio with 95% confidence interval. Results: Three managers and 81 health professionals participated in the study. The FHS had a better index rating than the traditional care model for general PHCI and for the attributes longitudinality, comprehensiveness, family focus and professional level. Conclusion: Although the FHS attained higher scores compared to the traditional model, it has not yet achieved the performance expected. This scenario points to the need for increased FHS cover and quality improvements at the existing units.

Abstract This study regards the experience of implementing a professional internship intended to students of Occupational Therapy in Primary Health Care in a Federal University located in São Paulo State, Brazil. Occupational therapist profession in Brazil was inserted in this level of care to offer specialised support to teams, mainly concerning qualification of care to vulnerable and invisible populations (community, family and cultural aspects), health promotion, encouragement of team work, humanisation in health. Such curricular internship occurs since 2017 to students of the 4th and 5th years of undergraduate course, completing 240 hours. How is the practice programme in Primary Health Care organised in the pedagogic project? Which are the strengths and fragilities of this field, as far as education is concerned? Among the practice scenarios, there are Basic Health Unit, Family Health Strategy teams, Family Health Support Centre, Street Clinic and University Basic Health Unit, with tutors who establish the collaboration. The interns follow individual and group cases, take part in team meetings and actions for team qualification and are also supervised weekly by a professor. Among their strengths, we can list a diversified education, based on the most recent public policies for the level of care that represents the stepping stone to the health system. As well as the access to the occupational therapy professional in this first contact, if the team identifies such necessity. Regarding challenges, there is unawareness about professional practice, difficulty to access work instruments and resources, as well as physical space. We hope this experience can help other educational institutions to implement their practice programmes in and for the health system. Egresses report that enables learning to work in a team in different scenarios. Such strategy aims to form a generalist professional, with criticality regarding challenges and potentialities of Brazilian public health. Key messages This study reports the experience of a professional internship of Occupational Therapy in Primary Health Care, understanding the health system as a scenario for practice, teaching and learning. Educating occupational therapists to this level of care is necessary, in agreement with the attributes of Primary Care.

Contextualização: Trata-se de um relato de experiência de estudantes de Terapia Ocupacional na Atenção Primária à Saúde (APS), com acompanhamento docente e da equipe multiprofissional do Núcleo Ampliado de Saúde da Família e Atenção Básica (Nasf-AB). Processo de intervenção: As atividades foram direcionadas pelas ferramentas e atividades estruturantes do Nasf-AB. Análise crítica da prática: Experimentar a rotina do serviço e equipe permitiu que as estudantes aplicassem os conteúdos teóricos na prática e dimensionassem a multiplicidade de situações reais na APS. Síntese de considerações: Compreende-se que a Terapia Ocupacional nesse campo trabalha potencialidades do usuário para uma vida participativa, contribuindo também para que as estudantes vivenciem a especificidade profissional durante a formação.Palavras-chave: Atenção Primária à Saúde. O (TO), é um dos profissionais que podem compor os Núcleos de Apoio à Saúde da Família (NASF). O objetivo geral desse estudo foi, investigar na literatura científica nacional a contribuição do (TO) junto a equipe do NASF. Tratou-se de uma pesquisa bibliográfica, por meio de uma revisão integrativa de caráter descritivo. A estratégia de busca, foi realizada nas bases de dados Biblioteca Virtual em Saúde (BVS), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Coleção Nacional das Fontes de Informação do SUS (Coleciona SUS) e Revista Institucional Brasileira de Terapia Ocupacional REVISBRATO, no período de setembro a novembro de 2020 utilizando-se os seguintes descritores: “primary health care”, “occupational therapy”, “family health” com o operador booleano “AND” para a especificidade do levantamento bibliográfico. Os critérios de inclusão definidos para a seleção foram: artigos publicados na língua portuguesa que retratassem a temática, publicados e indexados nos referidos bancos de dados nos últimos cinco anos, sendo excluídos aqueles que não estavam disponíveis na íntegra. Foram selecionados 14 trabalhos, sobre a atuação do (TO) no NASF. Verificou-se que os grupos e oficinas terapêuticas, as visitas domiciliares, a organização de grupos comunitários e as reuniões com equipes da Estratégia Saúde da Família (ESF) foram as formas mais recorrentes de atuação desses profissionais no NASF, onde apoiaram-se no matriciamento e na reabilitação baseada na comunidade. Concluiu-se que a atuação do (TO) no NASF é um processo de edificação nos campos da promoção da saúde e da prevenção das doenças, desenvolvendo uma prática integral e interdisciplinar de saúde e contribuindo para a emancipação dos sujeitos em seus diversos papeis sociais, favorecendo o desenvolvimento humano e comunitário.

É relevante conhecer a diversidade de práticas de terapeutas ocupacionais na Atenção Primária à Saúde (APS) no contexto brasileiro, devido esse nível assistencial apoiar o rearranjo da atenção à saúde à população. O estudo identificou ações e atividades realizadas por terapeutas ocupacionais na APS. Trata-se de pesquisa do tipo Survey, efetuada entre novembro de 2017 e fevereiro de 2018, através de Questionário On line autoaplicável com terapeutas ocupacionais da APS. Participaram 105 profissionais, a maioria do gênero feminino, com até 39 anos de idade e tempo de atuação na APS variando entre um e cinco anos, inseridos, sobretudo, em Núcleos de Apoio à Saúde da Família das regiões sudeste e nordeste. Os profissionais informaram realizar atendimento individual (97,1%) e grupal (93,3%); atenção domiciliar (84,8%); apoio matricial (88,6%); educação em saúde (86,7%); promoção da saúde (95,2%); prevenção de doenças (89,5%); educação permanente e/ou continuada (84,8%); participação em reuniões de planejamento dos serviços (89,5%) e de articulação com redes saúde (81,9%). Também utilizam atividades, tecnologia assistiva e recursos terapêuticos em ações junto a pessoas com dificuldades na realização das atividades cotidianas. As práticas referidas indicam a integração dos profissionais às equipes e aos serviços de saúde e do território. A identificação das ações e atividades representa etapa inicial de construção de dados aprofundados sobre a atuação da terapia ocupacional na APS. Abstract It is relevant to know the diversity of practices of occupational therapists in Primary Health Care (PHC) in the Brazilian context, due to this level of support to the rearrangement of health care to the population. The study identified actions and activities performed by occupational therapists in PHC. This is a Survey-type, conducted between November 2017 and February 2018, through a self -administered On-line Questionnaire with PHC occupational therapists. A total of 105 professionals, most of them female, with up to 39 years of age and working time in PHC ranging from one to five years, were included, mainly in Expanded Core Family Health and Basic Care in the Southeast and Northeast Regions. The professionals reported individual care (97.1%) and group (93.3%); home care (84.8%); matrix support (88.6%); health education (86.7%); health promotion (95.2%); disease prevention (89.5%); continuing and/or continuing education (84.8%); participation in service planning meetings (89.5%) and articulation with health networks (81.9%). They also use activities, assistive technology and therapeutic resources in actions with people with limitations in daily activities. The practices referred to indicate the integration of the professionals to the teams and the health services and the territory. The identification of actions and activities represents an initial stage of construction of in-depth data on the performance of occupational therapy in PHC.Keywords: Primary Health Care; Occupational Therapy; Unified Health System.

ABSTRACT Objective: to identify the space in which the Family Health Support Center is structured in the services and how it works within the scope of the Unified Health System. Method: a conceptual study based on partial results of a multicenter, descriptive-exploratory study with a qualitative approach, carried out in 2017, in the State of Santa Catarina, with managers and professionals from the Family Health Support Center. Content thematic analysis was used with the data from semi-structured interviews with professionals and individual interviews with managers, totalling 18 participants. Results: the professionals of the Family Health Support Center are identified as specialists who work in or close interaction with Primary Health Care, in the context of matrix support, in order to assist in the planning of the Health Care Network, which strengthens the general practice in Family Health. In addition to primary health care, they can act in intercessory spaces and perform secondary health care; set forth possibilities for intervention and enhance inter-sectoriality and co-responsibility. Conclusion: the Family Health Support Center was recognized as an important articulator of interdisciplinarity in health work, cabable of promoting comprehensive and intersectoral care, in addition to its initial proposal of supporting the Family Health Strategy, which could help in the coordination and fulfillment of secondary health care in the Brazilian Unified Health Care System, thus being an intersecting space between primary and secondary health care.

ABSTRACT Objective To describe the profile and performance of nutritionists in Primary Health Care. Methods A cross-sectional study was carried out, and all nutritionists in two municipalities of Paraíba, Brazil, were interviewed. Information was collected through structured interviews on demographic characteristics, professional qualification, development of food and nutrition activities, knowledge and use of essential bibliography for the work in Primary Care. Results In one municipality there were 28 teams of the Family Health Strategy and in the other, nineteen teams. In all, nineteen nutritionists were interviewed, fourteen of whom were working in the health teams and five were working exclusively in the Family Health Support Centers. All but one were women and the majority were between 20 and 39 years; the majority (n=10) had no graduate training. Nutritionists from the basic health teams developed more public health nutrition actions, such as defining nutritional care protocols and vitamin A and iron supplementation than those from the Family Health Support Centers (11 versus 1; and 13 versus 1, respectively). About half were satisfied with work in general, and dissatisfaction was related to deficiencies in the availability and quality of anthropometric equipment, physical structure and material. Conclusion Nutritionists work in food and nutrition actions in collective health, emphasizing the importance of qualification and practices that better combine the programmatic agenda of this area with Primary Care.

Nos últimos anos, obteve-se maior inserção de terapeutas ocupacionais em serviços que compõe a Atenção Primária em Saúde (APS). O objetivo deste estudo é analisar a produção científica brasileira acerca da formação do terapeuta ocupacional para a atuação na APS. Realizou-se uma busca nas bibliotecas e bases de dados CAPES, SCIELO, LILACS e em periódicos da profissão no país, resultando em 16 artigos a serem revisados e discutidos através de análise temática. A partir da leitura do material foi identificado a repetição de núcleos de sentido sendo definido a organização do conteúdo nos seguintes temas: 1) Princípios, diretrizes e ferramentas da APS na formação do terapeuta ocupacional; 2) Diferentes cenários e estratégias de ensino na formação do terapeuta ocupacional e; 3) Principais desafios vivenciados na formação em terapia ocupacional para atuação na APS. Observou-se que na formação foram discutidas a importância da longitudinalidade do cuidado, a ordenação da rede, e a relevância do aprendizado das principais ferramentas, como o acolhimento, o vínculo, a clínica ampliada, o apoio matricial e o Projeto Terapêutico Singular. Esses conceitos da prática na APS são vivenciados desde a graduação até a formação continuada. Considera-se que o aprendizado vem se dando em consonância com as Diretrizes Curriculares Nacionais do curso, através de experiências teórico-práticas e sob a luz dos principais conceitos, princípios e diretrizes das principais políticas públicas voltadas ao estabelecimento e organização da APS, independente dos obstáculos encontrados. AbstractIn recent years, it was obtained greater insertion of occupational therapists in services that compose the Primary Health Care (PHC). The purpose of this study is to analyze the Brazilian scientific production about the formation of the occupational therapist to work in PHC. A search was performed in the libraries and databases CAPES, SCIELO, LILACS and in professional journals in the country, resulting in 16 articles to be reviewed and discussed through thematic analysis. From the reading of the material was identified the repetition of center of meaning being defined the organization of content in the following themes: 1) Principles, guidelines and tools of PHC in the training of occupational therapist; 2) Different scenarios and teaching strategies in occupational therapist training and; 3) Main challenges experienced in occupational therapy training for PHC. It was observed that the experiences are based on the main guidelines, recognizing the importance of longitudinality of care and ordering of the network, and the relevance of learning the main tools such as welcoming, bonding, expanded clinic, matrix support and the Singular Therapeutic Project. These concepts of practice in PHC are experienced since graduation until the spaces of continuing education as multidisciplinary residences and graduate programs. Learning is considered to be in line with the National Curriculum Guidelines of the course, through theoretical and practical experiences and in the light of the main concepts, principles and guidelines of the main public policies aimed at establishing and organizing PHC regardless of obstacles encountered.Key words: Occupational Therapy; Primary Health Care; Professional Qualification. ResumenEn los últimos años, se logró una mayor inserción de terapeutas ocupacionales en los servicios que componen la Atención Primaria de Salud (APS). El objetivo de este estudio es analizar la producción científica brasileña sobre la formación del terapeuta ocupacional para trabajar en APS. Se realizó una búsqueda en las bibliotecas y bases de datos periódicas de CAPES, SCIELO, LILACS y en revistas profesionales del país, lo que resultó en 16 artículos para ser revisados y discutidos mediante análisis temático. A partir de la lectura se identificó la repetición de núcleos de significado definiéndose la organización del contenido en los siguientes temas: 1) Principios, pautas y herramientas de APS en la formación del terapeuta ocupacional; 2) Diferentes escenarios y estrategias de enseñanza en la formación del professional y; 3) Principales desafíos experimentados en la capacitación en TO para la APS. Se observó que las experiencias se guían por las pautas principales, reconociendo la importancia de la longitudinalidad de la atención, el orden de la red, y la relevancia de aprender las herramientas principales como la bienvenida, el vínculo, la clínica ampliada, el apoyo matricial y el Proyecto Singular Terapéutico. Estos conceptos de práctica en APS se experimentan desde espacios de pregrado hasta educación continua. Se considera que el aprendizaje está en línea con las Directrices Curriculares Nacionales del curso, a través de experiencias teóricas y prácticas y a la luz de los principales conceptos, principios y directrices de las principales políticas públicas destinadas a establecer y organizar la APS independientemente de los obstáculos encontrados.Palabras clave: Terapia ocupacional; Atención primaria de salud; Formación Profesional.

Abstract Family Health Support Centres (NASFs, in Portuguese) aim to develop interprofessional practices anchored in the assumptions of Primary Health Care, guided by the criteria of shared care specific professional interventions, management processes, interdisciplinarity, intersectoriality, Continuing Education in Health and health promotion. This health management methodology (Matrix Support) still represents a challenge to workers and managers, as it switches the logic of clinical thought from individual-centred, ambulatory and disciplinary care to collective, territory and interdisciplinary care. This study might illustrate and allow sharing of experiences about a work management format for multidisciplinary teams in Primary Health Care. A partnership between the University and the state health department was established in order to elaborate a collaborative, educational and supportive action. We formed a group of workers in the target area (24 municipalities and 18 teams) who had a focus on Continuing Education in Health using Institutional Analysis as theoretical reference. The main goal of the action was to provide spaces to share experiences and learning in the perspective of Continuing Education in Health. How does Continuing Education in Health support interprofessional practices in the field of Collective Health? Among the main results, we list: reflection about organisation practices of multiprofessional work, reflection about care practices and clinic management in the technical-assistance and pedagogic perspectives of Matrix Support; encouragement to improve the services offered in this level of care and their relationship with other points of the network. This experience showed us the importance of three main aspects: Continuing Education in Health, as a strategy of critical analysis about work and workers; the partnership and integration between teaching and service; and interprofessional formation processes (necessary to NASF and Collective Health). Key messages Continuing Education in Health is a strategy to deal with challenges and possibilities of interprofessional practices in the field of Collective Health. Enabling experiences and providing spaces for health professionals to share experience and learning.
