Abstract Aim The aim of this report is to present a case of severe fracture of the maxillary anterior alveolar process with substantial bone dislodgement associated with extrusive tooth luxation and avulsion. Background Dentoalveolar trauma is a challenge to dentistry, especially in young patients, for it can lead to early tooth loss which compromises oral function, esthetics, self-esteem, and alter the long-term plan of care for the victim. Case Report A 12-year-old girl with severe dentoalveolar trauma to the maxillary anterior region presented for emergency care for her injury. Treatment consisted of fracture reduction of the alveolar process, repositioning of the teeth that had suffered extrusive luxation, placement of a semi-rigid splint, and suturing of soft tissue lacerations. The traumatized teeth presented with pulpal necrosis and were treated endodontically. After 24 months of follow up, the fracture of the alveolar process was completely healed and the displaced teeth presented no signs of ankylosis or root resorption. Summary First-aid care contributed remarkably to this case allowing the re-establishment of esthetics, function, and patient's self-esteem. In spite of trauma extension the treatment outcomes were favorable. Clinical Significance Cases of dentoalveolar trauma should be evaluated on an individual basis. However, early emergency management and adequate follow-up can prevent further complications and contribute to treatment success. Introduction: Dental avulsion is a severe injury in which the tooth is totally displaced out of its alveolar socket. Replantation of avulsed teeth is a standard procedure. The success of the treatment of traumatized teeth revolves around the status of periodontium since it is a vital structure. Furthermore, other factors, including extra-alveolar time period, choice of storage media, and contamination of the avulsed tooth, can determine the success of replantation. Delay in the replantation of avulsed incisors increases the risk of dentoalveolar ankylosis and replacement root resorption. Case Presentation: This case report presented a 9-year-old girl with tooth avulsion subjected to a 3-year follow-up after the replantation of avulsed maxillary central incisors kept in dry conditions for seven days. The avulsed mature permanent teeth were managed through the application of treatment guidelines with prolonged extra-oral time. The avulsed teeth were replanted and splinted for four weeks. The canals of both teeth were filled with calcium hydroxide and replaced by mineral trioxide aggregate after six months. Two months after replantation, there were signs of external root resorption in tooth 21. Conclusions: In the third year of follow-up, both teeth were infraoccluded, although they had an acceptable level of functionality and appearance. These ankylosed incisors can be managed successfully with the decoronation technique in the future.

Inversion of premolars is an extremely rare condition, which usually requires extraction. This case report describes the inversion of an impacted maxillary second premolar in an 11-year-old male, and the multidisciplinary treatment approach for bringing the tooth into a normal position within the arch. In order to provide sufficient space for surgical reimplantation of the tooth, the mesially-drifted neighbouring maxillary first molar was first endodontically treated, followed by orthodontic distalization of the tooth. The inverted tooth was removed surgically and reimplanted without the use of splints for stabilization. After a 12-month follow-up period, the tooth maintained its vitality without any root resorption. Reimplantation of impacted inverted premolars can be a viable treatment alternative to extraction

Dental trauma is very common, especially in children and adolescents. Tooth avulsion (exarticulation), which is a result of complete rupture of periodontal ligaments is one of the most severe dental injuries. The treatment of choice in avulsion of permanent teeth is reimplantation. Survival of reimplanted teeth depends greatly on the time and conditions in which teeth were stored outside the oral cavity. This report presents a case of 18-year old male with avulsion of central maxillary incisors. The teeth were reimplanted 14 hours after injury. During 8-year follow up no major complications occurred and the teeth still maintain their function, although signs of mild replacement resorption are visible on the follow-up radiographs.

ABSTRACT Dentoalveolar trauma is a severe traumatic injury involving alveolar bone and dental structures. Like any trauma, it can leave irreparable sequelae and even cause tooth loss. The aim of this study is to report on the case of a dentoalveolar trauma in a 10-year-old male patient and the treatment performed with 12-month follow-up. The child had lateral luxation and displacement of teeth 11 and 21 in labial direction after a fall from his own height. The patient was examined at a hospital, received medication and was referred to a dental clinic. At the dental office, the teeth were repositioned, as well as received endodontic treatment and dental reanatomization. Two months after the trauma, root resorption was observed externally, which remained stable for 12 months. It was concluded that post-trauma treatment should be immediate and that follow-up sessions should be done carefully to minimize sequelae and to receive better prognosis.

Accidental tooth avulsion is a grievous injury and common among the children. Management of avulsed tooth within alveolar socket by reimplantation becomes a challenge for the clinician due to extraoral time and media of transportation. Although the long-term prognosis of reimplantation is poor, the time during which the tooth remain within the arch will guide the development of alveolar bone completely. Moreover, reimplantation will maintain anatomical, functional and esthetic rehabilitation of the patient. In this case report, we present a case of accidental avulsion where teeth were gently rinsed of any debris and placed in normal saline during the examination and preparation of the reimplantation sockets. The teeth were then reimplanted, functionally splinted followed by endodontical treatment. After 12 months follow-up the periodontal space was healed perfectly without any resorption or ankylosis.J Enam Med Col 2017; 7(3): 165-169

External resorption is sequelae of necrotic periodontal membrane over a large area of root following an injury to the tooth. This usually occurs after severe dental injuries such as intrusion, severe luxations or exarticulation injuries complicated by a prolonged extra oral period. This case report presents a clinical and radiographic follow up (13 months) of treatment of inflammatory external root resorption on maxillary central incisor using Vitapex®. Gradual healing of resorption was observed radiographically with no tenderness or pathological mobility.

In dentistry,root resorption is the breakdown or destruction and subsequent loss of the root structure of a tooth caused by living body cells attacking part of the tooth. When the damage extends to the whole tooth is called tooth resorption. Severe root resorption is very difficult to treat and often requires the extraction of teeth. Root canal therapy has been shown to be a verysuccessful means of treating inflammatory resorption. It has been recommended to include a calcium hydroxide paste in the root canal therapy to enhance the success of the treatment out come. Calcitonin has also been suggested as an interim root canal medicament to assist in the inhibition of osteoclastic bone and dentin resorption.
