Introduction: Rheumatoid Arthritis is a common form of inflammatory arthritis, occurring throughout the world and in all ethnic groups and affects approximately 1% of the population worldwide. In Ayurveda Amavata has a high resemblance to Rheumatoid Arthritis. Acharya Madhavakar was the first to give a detailed description of Amavata. Modern treatment of Rheumatoid Arthritis is not satisfactory therefore Ayurvedic medicines are the need of the hour. Aim & Objectives: To explore the mode of action of Simhanada Guggulu and to aware about medicinal properties and encourage the use of Simhanada Guggulu in the management of Amavata (Rheumatoid Arthritis). Materials and Methods: For this study Ayurveda Samhitas, authentic publications, internet and modern medical literature have been reviewed. Conclusion: The Simhanada Guggulu is Kaphavatahara, Pittavardhaka, Agnideepaka and Amapachaka. Thecontents of Simhanada Guggulu may act as Disease Modifying Anti Rheumatic Drugs (DMARDS). So it is concluded that Simhanada Guggulu can be used as an effective Ayurvedic medicine for Amavata (Rheumatoid Arthritis). Key words: Simhanada Guggulu, Rheumatoid Arthritis, Amavata.

Post-marketing surveillance activities are essential to detect the risk/benefit profile of biologic disease-modifying antirheumatic drugs (bDMARDs) in inflammatory arthritis. The aim of this study was to evaluate adverse events (AEs) in patients treated with bDMARDs in rheumatology during a prospective pharmacovigilance study from 2016 to 2018. Descriptive statistical analyses were performed to evaluate bDMARDs-related variables of patients without AEs/failures vs patients with AEs and failures. The risk profile among biologics was assessed by comparing patients treated with each bDMARD to patients treated with etanercept. A total of 1155 patients were enrolled, mostly affected by rheumatoid arthritis (46.0%). AEs and failures were experienced by 8.7% and 23.3%, respectively. The number of comorbidities significantly influenced the onset of AEs, while anxiety-depressive, gastrointestinal disease, and fibromyalgia influenced onset of failures. The probability of developing an AE was significantly lower in patients treated with secukinumab, while the probability of developing treatment failure was significantly lower in patients treated with golimumab, secukinumab and tocilizumab. A total of 216 AEs were reported (25.5% serious), mostly regarding infections (21.8%), musculoskeletal (17.6%) and skin (16.2%) disorders. Serious AEs included neutropenia (12.7%), lymphocytosis (9.1%) and uveitis (7.3%). The obtained results revealed known AEs but real-world data should be endorsed for undetected safety concerns.

Introduction: Rheumatoid arthritis (RA) is the most common variety of inflammatory arthritis. The annual inci- dence of RA worldwide is approximately estimated to be 3 cases per 10,000 populations. Based on the similar signs and symptoms RA can be compared with Amavata. It is not described in Brihatrayi as a separate disease the first detailed description was given by Acharya Madhavakar. The available contemporary treatment modalities are not satisfactory therefore Ayurvedic medicines are needed of the hour. Bhaishajya Rathnavali has been men- tioned Punnarnava Guggulu in the management of Amavata. Aim: To critically study the mode of action of Pun- narnava Guggulu in the management of Amavata w.s.r. Bhaishajya Rathnavali. Methods: For this study Ayurve- da Samhita-Madhava Nidana, Authentic publications and modern literature have been reviewed. Conclusion: The Punnarnava Guggulu is Shothahara, Vedanasthapaka, Vatahara, Agnivardhaka and Amapachaka. So, it is concluded that Punnarnava Guggulu can be used as an effective medicine for Amavata. Keywords: Punnarnava Guggulu, Amavata, Rheumatoid arthritis

Arthralgia is one of the most common symptoms that occur in patients with COVID-19. About 15% of patients present with arthralgia at some point. Although COVID-19 seems to attack the musculoskeletal system (muscles and joints) in its infective and postinfective stage causing inflammatory arthritis, not much is known about the rheumatic manifestations of this infection. In this case series of 5 patients, we discuss the occurrence of bilaterally symmetrical polyarthritis in patients, previously free from any rheumatic disease, after encountering COVID-19 infection. The musculoskeletal manifestations in these patients phenotypically resembled rheumatoid arthritis. These patients were treated successfully with low-dose glucocorticoids and disease-modifying antirheumatic drugs (DMARDs).

Objective. To evaluate the effect of access to and distance from rheumatology care on the use of disease-modifying antirheumatic drugs (DMARD) in US veterans with inflammatory arthritis (IA).Methods.Provider encounters and DMARD dispensations for IA (rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis) were evaluated in national Veterans Affairs (VA) datasets between January 1, 2015, and December 31, 2015.Results.Among 12,589 veterans with IA, 23.5% saw a rheumatology provider. In the general IA population, 25.3% and 13.6% of veterans were exposed to a synthetic DMARD (sDMARD) and biologic DMARD (bDMARD), respectively. DMARD exposure was 2.6- to 3.4-fold higher in the subpopulation using rheumatology providers, compared to the general IA population. The distance between veterans’ homes and the closest VA rheumatology site was < 40 miles (Near) for 55.9%, 40–99 miles (Intermediate) for 31.7%, and ≥ 100 miles (Far) for 12.4%. Veterans in the Intermediate and Far groups were less likely to see a rheumatology provider than veterans in the Near group (RR = 0.72 and RR = 0.49, respectively). Exposure to bDMARD was 34% less frequent in the Far group than the Near group. In the subpopulation who used rheumatology care, the bDMARD exposure discrepancy did not persist between distance groups.Conclusion.Use of rheumatology care and DMARD was low for veterans with IA. DMARD exposure was strongly associated with rheumatology care use. Veterans in the general IA population living far from rheumatology sites accessed rheumatology care and bDMARD less frequently than veterans living close to rheumatology sites.

The demonolexis in Yu. Andrukhovych’s long short story “Recreatsii” (“Recreations”) has been analyzed in the article. In the course of the research there have been outlined the following lexical-semantic groups of demonomens: toponymic and onomastic names, modified lexemes, names of the rituals, genuine Ukrainian demonomens, obscene words and expressions, demonomens of Biblical origin, names from the world mythology and general demonolexis. The special lexical-semantic group has been formed by non personificated demonomens, which serve to convey the peculiarities of the contemporary Ukrainian writers’ mentality, their habits through speech. Such nomens help to reveal the protagonist’s soul, show the positive and negative sides of his personal ego, demonstrate the duality of the human perception of the world, indicate the causes of phobias, emotions, sensations. Key words: demonolexis, demonomen, lexical-semantic group, non personificated demonomen.

"Utterance universalism" as a phrase is unclear, but it is enough to include the term "prophetism". As a metaphysical concept, it refers to a text written with inspiration which confirms visions of a "divine inspiration", "poetic" - "legal", that contains trace, revelation or interpretation of the origin of the creation of the world and life on earth but it warns and prospects their future in the form of a projection, literary paradigm, religious doctrine and law. Prophetic texts reformulate "toll-telling" with messages, ideas, which put forth (lat. "Utters Forth" gr. "Forthteller") hidden facts from fiction and imagination. Prometheus, gr. Prometheus (/ prəmiθprə-mee-mo means "forethought") is a Titan in Greek mythology, best known as the deity in Greek mythology who was the creator of humanity and charity of its largest, who stole fire from the mount Olympus and gave it to the mankind. Prophetic texts derive from a range of artifacts and prophetic elements, as the creative magic or the miracle of literary texts, symbolism, musicality, rhythm, images, poetic rhetoric, valence of meaning of the text, code of poetic diction that refers to either a singer in a trance or a person inspired in delirium, who believes he is sent by his God with a message to tell about events and figures that have existed, or the imaginary ancient and modern world. Text Prophetism is a combination of artifacts and platonic idealism. Key words: text Prophetism, holy text, poetic text, law text, vision, image, figure
