The article is devoted to the description of the X-ray anatomy of the mediastinum, the evolution of the classification of this anatomical region. As well as systematization of radiological signs of the most common formations of the anterior (prevascular) mediastinum in children. Based on these data, a table of the most characteristic radiographic features of various neoplasms of different groups. Reflected basic criteria differential diagnosis of various tumors of the anterior mediastinum.

Background. Recent decades in Ukraine have been characterized by a significant increase in the number of tuberculosis patients, often with forming cavities of destruction. X-ray diagnosis of lung cavitary lesions is one of the current issues of modern pulmonology and thoracic surgery. Pulmonary abscesses resemble other diseases with destruction and cavities substantiating the need for differential diagnosis with tuberculosis. Purpose – specifying particular scenarios of X-ray presentation of lung abscess and determining the capability of differential diagnosis of pseudotuberculosis with cavities of tuberculosis etiology. Materials and methods. The paper deals with the analysis of X-ray examination of thoracic viscera provided for 252 patients with lung abscess, aged 18 and up to 78. X-ray radiography in two projections, linear and computed tomography (56 patients involved) were performed. All patients underwent a study over time. Results. Almost in most lung abscess cases, there is a need for differential diagnosis with a range of medical entities. The obtained data have made it possible to suggest a classification of X-ray scenarios of lung abscess. The scenarios of X-ray presentation of acute pulmonary abscess are typical and atypical, among those: cystoid, pseudotuberculous, affected 38 patients (15 %), and pulmonary-pleural. The peculiarities of X-ray presentation of pseudotuberculous scenario along with the differences and signs allowing to make an accurate diagnosis have been specified. Conclusions. X-ray study remains an essential in diagnosing purulent-destructive diseases. Being familiar with the scenarios mentioned above and pseudotuberculous one, in particular, will make it possible to significantly improve diagnosis as well as differential diagnosis of pulmonary abscess.

Cephalalgia must be the leading or sometimes the single symptom of 50 different diseases. This paper is devoted to analysis the causes, the clinical features and methods of cephalalgia diagnosis in children, teenagers, as well as the modifiable and trigger factors. There is cephalalgia in 40-60% children and the prevalence of the disease increases with age, especially with the beginning of school, and reaches a peak with adolescent period 75%, this explains the importance of this work. In the last 30 years, scientists have registered the growth of the epidemiology incidence of primary headache disorders, including the chronic form of headaches: tension headache has grown in ten times, migraines – in 5-6 times. Due to the uncertainty of subjective sensations in infancy the revelation of cephalalgia becomes sometimes quite a complicated diagnostic problem. Many researchers mention the difficulty of differentiation of various cephalalgia types in infancy due to insufficient sensibility of diagnostic criteria, which were offered by the International Classification of Headache Disorders 2003. 20-35% of cases cann’t classified. In this article, the importance of headache studying is accentuated, that is necessary for life optimization of children suffering from headaches.

The specific features of psoriatic arthritis include the combined lesion of the joints of limbs and the spine, diversity of the clinical symptoms, variability of the initial manifestations, serological and X-ray negativity at the early stages. This causes difficulties in the diagnosis of the disease. The article presents the diagnostic criteria of psoriatic arthritis and principles of the differential diagnosis of the disease.

Since its first publication in 1975, the squamous odontogenic tumor remains the rarest odontogenic lesion, with around 50 cases in the English-language literature in which the microscopic characteristics are frequently very well demonstrated. However, articles which discuss the radiographic aspects are scarce, especially with emphasis on the differential diagnosis. The present treatise proposes an assessment of jaw lesions with the same radiographic characteristics of the squamous odontogenic tumor to clarify the main findings for dental clinicians during routine diagnosis.

Thymic carcinoma (TC) belongs to a group of rare thymic epithelial tumours (TETs) arising from the anterior mediastinum. One of the most relevant established prognostic factors is complete surgical resection. Given the high invasive potential of TC a lot of patents at the diagnosis present with a locally advanced or metastatic disease, therefore complete resection could not be achieved, and a role of systemic therapy is increasing. Due to the rarity of this tumor type in children and adolescents, treatment approaches based on the guidelines for management of TETs in adults. In this article we report a case of TC in a 2-year-old boy. The literature review describes the current histological classification of the TET, approaches for systemic chemotherapy and local control in patients with TC.

Papillary thyroid carcinoma (PTC) is a common carcinoma in thyroid. As many benign thyroid nodules have the papillary structure which could easily be confused with PTC in morphology. Thus, pathologists have to take a lot of time on differential diagnosis of PTC besides personal diagnostic experience and there is no doubt that it is subjective and difficult to obtain consistency among observers. To address this issue, we applied deep learning to the differential diagnosis of PTC and proposed a histological image classification method for PTC based on the Inception Residual convolutional neural network (IRCNN) and support vector machine (SVM). First, in order to expand the dataset and solve the problem of histological image color inconsistency, a pre-processing module was constructed that included color transfer and mirror transform. Then, to alleviate overfitting of the deep learning model, we optimized the convolution neural network by combining Inception Network and Residual Network to extract image features. Finally, the SVM was trained via image features extracted by IRCNN to perform the classification task. Experimental results show effectiveness of the proposed method in the classification of PTC histological images.
