Dupuytren’s disease is an acute and multifaceted problem that affects not only medicine but also psychology, social life, since a person who has lost the ability to perform his/her socially significant functions experiences moral and physical suffering. From the first description of Dupuytren’s disease to the present, questions of etiology, pathogenesis, classification, treatment methods, and features of the preventive measures of this disease remain debatable. Therefore, the main goal of the paper was to analyze the technology for predicting the functional state of the hand after surgical treatment of Dupuytren’s disease (contracture). The study was conducted at the Department of Hand Microsurgery of the Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan. The total number of patients under observation was 258, age 21-80 years. The research results were grouped into 6 combinations. It has been determined that three groups of factors affect the recovery degree: A (gender, age, examination period, severity); B (place of residence, physical and mental work, bad habits); C (surgeon's experience, types of operations, anaesthesia, section, skin plastic, reconstructive treatment, healing, etc.). It has been found that it is possible to achieve full restoration of the function of the wrist on the right hand in 67.5% of patients and on the left hand in 59.9% of patients in periods of more than one year.

This study presents the clinical outcomes of 35 hands with Dupuytren's Disease treated with the McCash technique between 1990 and 2009. Of the 31 patients (28 males and three females, mean age 53 yrs), four patients had bilateral involvement (12.9%). Thirty hands had no previous medical or surgical treatment for the disease, while the remaining five hands had been operated on at least once. The mean contracture of metacarpophalangeal (MCP) joint improved from 42.14° to 1.83°, while that of the proximal interphalangeal (PIP) joint improved from 62.60° to 7.09°. All wounds healed within a mean 9.8 weeks. Sensory evaluation revealed no permanent numbness. With realistic expectations, clear documentation, meticulous surgical technique and implementation of a demanding post-operative rehabilitation program, an acceptable outcome may be achieved with the McCash technique for Dupuytren's disease.

Analysis of Dupuytren’s contracture (DC) surgical treatment efficacy according to the Tatarstan Republic DC Register (258 patients, 343 hands) was performed. Partial wedge-shaped excision of palmar aponeurosis was the most common intervention on both the right and left hand. Late (1 year and over) postoperative complications (POC) — relapse, dissemination, progression were detected in 41.8% of patients at terms up to 15 years. Presence of POC on the operated hand caused disturbance of its function in 11.9% of cases on the right and 16.3% of cases on the left. Curability from DC made up 34.9 — 73.8% depending on the operated hand. Complete restoration of hand function was achieved in 22.2—100% of observations. Surgical treatment of DC enabled to ensure favorable patient’s condition at term 1 year and over in 67.5 and 59.9% of cases on the right and left hand respectively.

This study aims to know the direction of democracy and know the framing of news in Republika.com and Kompas.com, as well as the implications of the dissolution of FPI to development communication. The uniqueness of this research, FPI is a mass organization with a large number of masses but does not have an organizational license. This study uses the paradigm of constructionism, which considers the reality of social life is not a natural reality. Using the Robert N. Entman method to frame news stories coming from Republika.com and Kompas.com. The difference in framing in Kompas.com media and Republika.com is the news source. Republika.com choose a news source from an Islamic background, while Kompas.com choose a news source who is competent in his field is not based on his religion. In the first framing analysis, the level of problem identification and causal interpretation, the two online media have similarities. However, in terms of moral evaluation and treatment recommendation both media have differences. In Republika.com researchers analyzed treatment recommendations is the rejection of FPI activities. FPI has no legal standing. Kompas.com, his treatment recommendation consideration of the dissolution of FPI is to maintain the existence of ideology and basic consensus of the state of Pancasila, the 1945 Constitution, the integrity of the Republic of Indonesia, and Bhinneka Tunggal Ika. Basically, the right to organize is a constitutional right. If there is no fundamental reason, the dissolution of a community organization should not exist. However, FPI is an organization that offers intolerance in the religious field.

Relevance: Styloid process syndrome – a disease which is caused by irritation of the styloid process of the temporal bone surrounding the nerve, vascular and muscle structures. The syndrome is manifested by chronic painin deep division of the facial area referred to the root of the tongue, throat and ear, dysphagia, symptoms of circulatory disorders of the brain. Because of the low awareness of the disease practical specialists usually miss the diagnoses. Patients with different diagnoses are treated by different specialists; most often the prescribed symptomatic treatment is ineffective or does not give any result at all. The syndrome causes severe suffering to patients, reduces the quality of life. Purpose of the study: Assess the most significant clinical signs of the disease, mutual work between hypertrophied styloid process with the neurovascular neck formations, the effectiveness of conservative and surgical treatment. Materials and methods: 144 people were examined and treated by us. Of these, 103 women (71,5%), 41 men (28,5%), aged 25 to 70 years. The disease lasts from one year until 10-15 years. Tomography of the styloid processes with 3D reconstruction and contrasting of the main vessels of the neck was used. Results of the study and the discussion: we proposed a grading scale for the quality of the treatment, the five-point scale. The quality of the treatment was assessed by the number of reduction in scores and, accordingly, patient complaints. Of the total number of patients – 144 people, the success of the treatment by 0 points was assessed by 60 people (that is, absence of any symptomatology) (41,7%), 1 point – 44 (30,5%), 2-3 points – 27 (18,7%), 4-5 points – 13 (9%), that is significant effect of the treatment was not achieved. To all those people with the result of the treatment was estimated at 4-5 points, respectively, was proposed an operative method of the treatment – the resection of the styloid process from the corresponding side. Conclusions: Because of the lack of knowledge of such important topic it is necessary to improve diagnostic methods. Hypertrophy of the styloid process is not the main reason of the development of styloid syndrome; the degree of deviation of the process is important. Conservative treatment in most cases has a lasting effect, but has repeated symptoms after a certain period of time; surgical treatment should be directed to the maximal resection of styloid process for decreasing the axis pressure on the neurovascular bundle of the neck.

Relevance: Esophageal cancer ranks eighth in the structure of malignant tumors in the Republic of Kazakhstan. According to the Kazakh Institute of Oncology and Radiology, in 2018, 1225 were primarily registered with esophageal cancer, and 825 patients died from this disease. The purpose of this study was to analyze the esophageal cancer incidence and mortality among the population of the Republic of Kazakhstan in 2009-2018 and determine the cancer service efficacy. Results: The analysis of intensive indicators of esophageal cancer incidence showed a decrease in the number of primary cases from 8.2‰ in 2009 to 6.7‰ in 2018, and the mortality – from 6.9‰ to 3.8‰. The share of early detection (Stage I-II) increased by 38.2% since 2009 to reach 52.1% in 2018. The share of stage IV esophageal cancer decreased from 7.2% in 2009 to 5.8% in 2018. One-year mortality decreased by 14.7% since 2009 to reach 41.2% in 2018, which speaks of the improvements in the timely diagnosis of esophageal cancer. The number of patients who received radical comprehensive treatment increased by 8.6% to reach 32.2% in 2018. The number of patients who received surgical treatment in 2009-2018 remained almost at the same level and amounted to 18.9 and 18.0%, respectively. However, during the years of screening for esophageal cancer, the proportion of surgical treatment increased from 26.6% to 30.4%. The ratio of incidence to mortality decreased from 91.4% to 68.8% in the study period indicating an improvement in the level of cancer service in the Republic of Kazakhstan. Conclusion: The analysis of intensive indicators of esophageal cancer incidence showed a decrease in primary incidence. The screening conducted in 2013-2016 has shown an improvement in early detection of esophageal cancer (stages I-II) and a decrease in the share of the advanced stage. The findings of this study shall serve as the basis for planning anti-cancer measures

AbstractThe main problem in this study is how the implications of UU No.6 Year 2014 on Village against village authority. The authority of the village in question is the authority of the village derived from the right of origin and the local authority of the village scale, since these two forms of village authority are the spirit of village autonomy. The results show that the village can’t be treated the same as treating the district, because the essence of village autonomy is different from regional autonomy. The district was formed as an implementation of centralization, which carried out some of the powers granted by the Center. Different villages, because they have authority derived from the right of origin, not a gift from the center. The autonomy of the village existed long before the republic was established, and although redesigned many times through the central policy of the village, the autonomy of the village still exist, one of which is the existence of the authority of the origin right attached to the social status of the village head and the village official the name and the mention of it, and reflected by the behavior of the village community who uphold the social life of the culture. In the end the design of village authority is proposed as part of the solution, which tries to accommodate two constructs on the authority of the village, where the existing village authority enters the "container" No. 6 Year 2015 on the Village.AbstrakMasalah utama dalam studi ini adalahbagaimana implikasi berlakunya UUNo. 6 Tahun 2014 tentang Desa terhadap kewenangan desa. Kewenangandesa yang dimaksud adalah kewenangan desa yang berasal dari hak asal usul dan kewenangan lokal berskala desa, karena kedua bentukkewenangan desa tersebutlah yang merupakan ruh otonomi desa. Hasil riset menunjukkan bahwa desa tidak bisa diperlakukan sama sebagaimana memperlakukan daerah kabupaten, karena hakekat otonomi desa berbeda dengan otonomi daerah. Kabupaten dibentuk sebagai pelaksanadesentralisasi, yang melaksanakan sebagian kewenangan yang diberikan oleh Pusat. Desa berbeda, karena memiliki kewenangan yang berasal dari hak asal usul, bukan pemberian dari pusat. Otonomi desa sudah ada jauh sebelum republik ini berdiri, dan meski didesain ulang berkali-kali melalui kebijakan pusat tentang desa , namun otonomi desa tetep eksis, salah satunya adalah dengan keberadaan kewenangan hak asal usul yang melekat pada status sosial kepala desa dan pamong desa , apapun nama dan penyebutannya, serta tercermin dari perilaku masyarakat desa yang menjunjung tinggi kehidupan sosial budayanya.Pada akhirnya desain tentang kewenangan desa diajukan sebagai bagian dari solusi, yang mencoba mewadahi dua konstruksi tentang kewenangan desa, dimana kewenangan desa eksisting masuk dalam “wadah” yang dikonstruksi UU No. 6 Tahun 2015 tentang Desa.Kata kunci : kewenangan desa, hukum negara, hak asal usul desa, kewenangan lokal berskala desa

Digital nerve injury is a well-recognised complication of surgical treatment for Dupuytren's disease. We describe a simple test, the Tag test, that can be used intra-operatively to help identification of the digital nerves.
