Objective: The authors conducted an integrative literature review of recent studies that explored the impact of interventions implemented in the U.S. that focused on improving access to dental care for low-income and vulnerable populations. Methods: The authors conducted an integrative literature review of studies published between 2012-2018 that addressed six oral health policy spheres. 1) Community-based dental access programs; (2) Medicaid reimbursement and expansion; (3) Student loan support; (4) Oral health services in non-traditional settings and dental residency programs; (5) Programs to improve oral health literacy; and (6) Use of dental therapists. Results: The authors included 39 articles for qualitative synthesis. Numerous public health initiatives and programs exist in the US aimed at increasing access to quality oral health care. Medicaid expansion, increased Medicaid fee-for-service reimbursement rates, and state loan repayment programs have demonstrated some success in improving access among underserved populations. A diversified dental workforce, with community dental health workers and mid-level providers like dental therapists, as well as interprofessional training of nurses and primary-care physicians in oral health have also shown positive impacts in advancing health equity. Further studies are needed to understand how oral health literacy programs can affect access and utilization of dental services. Conclusions: Improvements to the oral health care safety net will require a holistic and multifaceted approach in order to reduce oral health disparities. Policy levers should work, not in isolation, but rather in complementary fashion to one another.

O objetivo do presente estudo, por meio de uma revisão de literatura, levantar dados pertinentes sobre a síndrome G6PD, as limitações nas prescrições medicamentosas e a importância para o cirurgião-dentista, principalmente o Odontopediatra, em ter o conhecimento desta síndrome e suas repercussões sistêmicas. A pesquisa bibliográfica foi realizada em bancos de dados, Pubmed, Google Scholar e Lilacs, nos quais foram utilizados os artigos científicos publicados nos últimos 10 anos para a revisão de literatura. A fim de contribuir para a qualidade de vida dos portadores de deficiência de G6PD, estudos apontam os fármacos ou alimentos que devem ser evitados. Diante de tal situação, salienta-se a função primordial da G6PD na sobrevida dos eritrócitos, e destaca-se que o estudo desta enzimopatia levará ao conhecimento não apenas da via das pentoses, mas também dos mecanismos intracelulares que nosso organismo a todo momento lança mão para garantir o bem-estar do corpo humano.

AbstractAntiresorptive drugs operate in the bone metabolism modulation and are widely used in the treatment of bone metastases and bone losses related to hormonal deficiency. Although this therapy shows satisfactory results, there are adverse effects associated with its use, such as osteonecrosis of the jaws. Medication-related osteonecrosis of the jaws (MRONJ) is, therefore, a serious and challenging condition with important implications in dentistry. The aim was to conduct a narrative literature review on anti-resorptive drugs and their latest repercussions on the maxillary bones. The review was carried out through a bibliographic search using Decs/Mesh descriptors of interest, in Portuguese and English, in the PubMed, Virtual Health Library (VHL) and Scielo databases. After applying the inclusion and exclusion criteria, a total of 33 studies were selected for analysis. It can be noticed that therapy with anti-resorptive agents is complex, especially in dental practice, since MRONJ is a complication that is difficult to manage. Regarding the therapeutic options, these are divided into conservative, surgical or adjuvant therapy, however, there are no protocols in the literature, and there is no consistency regarding the indication of the suspension of the drug administration - "Drug Holiday". Thus, it is important that the multidisciplinary team seeks strategies that minimize complications and promote control over the use of these drugs. In addition, there is a need for investigations that contribute with guidelines for the management and control of adverse effects resulting from therapy with antiresorptive drugs. ResumoAs drogas antirreabsortivas atuam na modulação do metabolismo ósseo e são indicadas para o tratamento de metástases ósseas e perdas ósseas relacionadas à deficiência hormonal. Ainda que esta terapia apresente resultados satisfatórios, observam-se efeitos adversos associados ao seu uso, como a osteonecrose dos maxilares. A osteonecrose dos maxilares associada ao uso de medicamentos (OMAM) é, portanto, uma condição séria e desafiadora com implicações importantes na Odontologia. O objetivo foi realizar uma revisão narrativa de literatura sobre as drogas antirreabsortivas e suas respectivas repercussões nos ossos maxilares. A revisão foi realizada através de busca bibliográfica utilizando descritores Decs/Mesh de interesse, em português e inglês, nas bases de dados PubMed, Biblioteca Virtual de Saúde (BVS) e Scielo. Após aplicação dos critérios de inclusão e exclusão, um total de 33 trabalhos foram selecionados para análise. Pode-se constatar que a terapia com agentes antirreabsortivos é complexa, sobretudo na prática odontológica, visto que a OMAM é uma complicação de difícil manejo. Em relação às condutas terapêuticas para esta condição, divide-se em terapia conservadora, cirúrgica ou adjuvante, todavia, não existem protocolos validados na literatura, bem como não há consistência quanto à indicação do intervalo de suspensão da administração da droga - “Drug Holiday”. Desse modo, é importante que a equipe multidisciplinar busque estratégias que minimizem as complicações e promovam o controle no uso dessas drogas. Além disso, nota-se a necessidade de realizar investigações que contribuam com diretrizes para o manejo e controle dos efeitos adversos decorrentes da terapia com medicamentos antirreabsortivos. Steroid has recently been reported as a treatment for new coronavirus disease (COVID-19). The incidence of oropharyngeal candidiasis due to the inhaled steroid ciclesonide is lower than that due to other inhaled steroids. We report the first case of oral candidiasis with COVID-19 pneumonia using ciclesonide. A 75-year-old man was hospitalized for COVID-19 pneumonia. After admission, an oral combination of lopinavir/ritonavir was administered, and ciclesonide was inhaled for 7 days. On the 14th day of hospitalization, white plaque was found in his oral mucosa. Candida albicans was identified by oral bacterial tests, and amphotericin B was initiated. On the 35th hospital day, negative result for C. albicans was confirmed. Intraoral monitoring and intervention by dental care workers are considered important for the prevention of infectious complications induced by corticosteroids.

Cuidados paliativos são abordagens terapêuticas com o objetivo de aliviar o sofrimento, diante de doenças que ameaçam a vida dos pacientes. Essa conduta terapêutica além de garantir uma melhor qualidade de vida ao paciente e aos familiares que enfrentam alguma patologia apoia- se em uma visão de que a morte deve seguir seu curso natural, proporcionando assim uma maior dignidade ao paciente afetado e promovendo uma sensação de bem-estar. O presente trabalho tem como objetivo apresentar uma revisão de literatura sobre cuidados odontológicos paliativos em pacientes terminais. Foi realizada uma revisão de literatura nas seguintes bases de dados: PubMed, SciELO e Google Scholar. As palavras-chave utilizadas para a procura estudos foram “Cuidados paliativos”, “Equipe Hospitalar de odontologia”, “Cuidados de conforto”.  A Odontologia, nos cuidados paliativos, visa diminuir o número de infecções e também evitar a porta de entrada de diversos microrganismos. Esse serviço proporciona um atendimento humanizado ao paciente impossibilitado de ir a um consultório odontológico, e assim fazendo com que o profissional se desloque até o paciente por meio de visitas domiciliares. Os cuidados odontológicos paliativos em pacientes terminais são de grande importância, fornecendo conforto e amparo para o paciente e para família. É importante que hospitais incluam em suas equipes multidisciplinares cirurgiões dentistas capacitados para odontologia hospitalar, assim, tornando a terapia paliativa humanizada para os enfermos.   THE PALLIATIVE DENTAL CARE IN TERMINAL PATIENTS   ABSTRACT Palliative care is a therapeutic approach with the intention of relieving suffering when facing life-threatening illnesses. This therapeutic approach, besides guaranteeing a better quality of life for the patient and family members who face some pathology, is based on a perspective that death may follow its natural process, thus providing more dignity to the affected patient and promoting a wellbeing feeling. The present paper aims to present a literature review on palliative dental care in terminally ill patients. A literature review was conducted in the following databases: PubMed, SciELO and Google Scholar. The keywords used to research the articles were "Palliative Care", "Hospital Dentistry Team", "Comfort Care".  Dentistry, in palliative care, aims to decrease the number of infections and to avoid the entry gate for a range of microorganisms. This is a service that provides humanized care to patients who are unable to go to a dental office, and thus by heaving the professional to visit the patient through home care routine visits. Palliative dental care in terminal patients is of paramount importance since it provides comfort and support to the patient and family. It is worthwhile that hospitals include in their multidisciplinary team dental surgeons trained in hospital dentistry, and thereby enabling the palliative therapy to be humanized for the infirm.   Keywords: Palliative Care; Hospital Dentistry Team, Hospice Care.
