Improving the dental care organization of industrial enterprises workers. Literature review

This paper reports the results of a literature review of health care organizations that have transitioned from one electronic health record (EHR) to another. Ten different EHR to EHR transitions are documented in the academic literature. In eight of the 10 transitions, the health care organization transitioned to Epic, a commercial EHR which is dominating the market for large and medium hospitals and health care systems. The focus of the articles reviewed falls into two main categories: (1) data migration from the old to new EHR and (2) implementation of the new EHR as it relates to patient safety, provider satisfaction, and other measures pre-and post-transition. Several conclusions and recommendations are derived from this review of the literature, which may be informative for healthcare organizations preparing to replace an existing EHR. These recommendations are likely broadly relevant to EHR to EHR transitions, regardless of the new EHR vendor.

Considerou-se a produção existente na BVS, no período de 2000 a 2010, totalizando 19 artigos. Enfermeiros têm refletido sobre a integralidade a partir das esferas de atenção, da formação profissional e da organização dos serviços, especialmente na saúde coletiva. Objective: The authors conducted an integrative literature review of recent studies that explored the impact of interventions implemented in the U.S. that focused on improving access to dental care for low-income and vulnerable populations. Methods: The authors conducted an integrative literature review of studies published between 2012-2018 that addressed six oral health policy spheres. 1) Community-based dental access programs; (2) Medicaid reimbursement and expansion; (3) Student loan support; (4) Oral health services in non-traditional settings and dental residency programs; (5) Programs to improve oral health literacy; and (6) Use of dental therapists. Results: The authors included 39 articles for qualitative synthesis. Numerous public health initiatives and programs exist in the US aimed at increasing access to quality oral health care. Medicaid expansion, increased Medicaid fee-for-service reimbursement rates, and state loan repayment programs have demonstrated some success in improving access among underserved populations. A diversified dental workforce, with community dental health workers and mid-level providers like dental therapists, as well as interprofessional training of nurses and primary-care physicians in oral health have also shown positive impacts in advancing health equity. Further studies are needed to understand how oral health literacy programs can affect access and utilization of dental services. Conclusions: Improvements to the oral health care safety net will require a holistic and multifaceted approach in order to reduce oral health disparities. Policy levers should work, not in isolation, but rather in complementary fashion to one another.

Abstract Background Care coordination is a key strategy used to improve health outcomes and efficiency, yet there are limited examples in dentistry. A large dental accountable care organization piloted care coordination by retraining existing administrative staff to coordinate the care of high-risk patients. Following the pilot’s success, a formal “dental care advocate” (DCA) role was integrated system-wide. The goal of this new role is to improve care, patient engagement, and health outcomes while integrating staff into the clinical care team. We aim to describe the process of DCA role implementation and assess staff and clinician perceptions about the role pre- and post-implementation. Methods Guided by the Consolidated Framework for Implementation Research, semi-structured interviews with clinical and operational administrative staff and observation at the company-wide training session were combined with pre- and post-implementation electronic surveys. Descriptive statistics and mean scores were tested for significance between each survey sample (t-tests), and qualitative data were thematically analyzed. Results With preliminary evidence from the pilot and strong executive support, a dedicated leadership team executed a stepwise rollout of the DCA role over 6 months. Success was facilitated by an organizational culture of frequent interventions deployed rapidly through a centralized system, along with supportive buy-in from managerial teams and high staff acceptance and enthusiasm for the DCA role before implementation. Following implementation, significant changes in attitudes and beliefs about the role were measured, though managers held stronger positive impressions than DCAs. DCAs reported high confidence in new skills and dental knowledge post-implementation, including motivational interviewing and the ability to confidently answer patients’ questions about their oral health. Overall, the fast-paced implementation of this new role was well received, although consistent and significant differences in mean attitudes between managers and DCAs indicate more work to fine-tune the role is needed. Conclusions Successful implementation of the new DCA role was facilitated by a strong organizational commitment to team-based dentistry and positive impressions of care coordination among staff and managers. Upskilling existing administrative staff with the necessary training to manage some high-risk patient needs is one method that can be used to implement care coordination efforts in dentistry.

O objetivo do presente estudo, por meio de uma revisão de literatura, levantar dados pertinentes sobre a síndrome G6PD, as limitações nas prescrições medicamentosas e a importância para o cirurgião-dentista, principalmente o Odontopediatra, em ter o conhecimento desta síndrome e suas repercussões sistêmicas. A pesquisa bibliográfica foi realizada em bancos de dados, Pubmed, Google Scholar e Lilacs, nos quais foram utilizados os artigos científicos publicados nos últimos 10 anos para a revisão de literatura. A fim de contribuir para a qualidade de vida dos portadores de deficiência de G6PD, estudos apontam os fármacos ou alimentos que devem ser evitados. Diante de tal situação, salienta-se a função primordial da G6PD na sobrevida dos eritrócitos, e destaca-se que o estudo desta enzimopatia levará ao conhecimento não apenas da via das pentoses, mas também dos mecanismos intracelulares que nosso organismo a todo momento lança mão para garantir o bem-estar do corpo humano.

Aim. Assessment of organs and tissues of oral cavity of professional athletes who use protective mouth guards. Methods. Dental examination of 340 professional athletes of Olympic reserve at the age of 18 to 32 years with a duration of sport experience of 9-13 years was performed. We investigated the effect of mouth guards on the condition of the marginal periodontium and microbial contamination. Changes in periodontal tissues were diagnosed by means of traditional methods of inspection, probing of the periodontal grooves, recesses, as well as determining tooth mobility. The control group included 20 athletes not using athletic mouth guards during training. Results. High prevalence of caries (in average, in all examined athletes the rate of carious teeth reaches 32.90%) and fairly high prevalence of severe forms of periodontal diseases in those patients were revealed. Evaluation of the structure of the index of teeth with decay and fillings and removed teeth showed major defects in timely dental care organization for professional boxers: their ratio of filled teeth does not exceed 40.36% in average. A significant number of extracted teeth (more than 23%) indicates that the examined patients require dental orthopaedic care. 30 examined athletes wearing protective mouth guards had the strains of P. gingivalis, A. Actinomycetemcomitans and P. oralis revealed. Conclusion. The study of dental status of professional athletes using protective mouth guards, showed high prevalence of inflammatory periodontal diseases and low level of hygienic state of oral cavity.
