In a previous paper in this journal (IJNS), it is mentioned about a possible approach to re-describe QED without renormalization route. As it is known that in literature, there are some attempts to reconcile vortex-based fluid dynamics and particle dynamics. Some attempts are not quite as fruitful as others. As a follow up to previous paper, the present paper will discuss two theorems for developing unification theories, and then point out some new proposals including by Simula (2020) on how to derive Maxwell equations in superfluid dynamics setting; this could be a new alternative approach towards “fluidicle” or “vorticle” model of QED. Further research is recommended in this new direction.

BackgroundWomen with gestational diabetes (GDM) have an elevated risk of developing type 2 diabetes (T2DM). NICE Guidance recommends women who develop GDM are screened 6 weeks post-partum and annually thereafter.AimTo evaluate conformity to guidance of screening in women with GDM by 6-week post-partum fasting plasma glucose (FPG) and annual FPG and determine time between delivery and development of T2DM.MethodRecords at a tertiary referral centre were used to identify women (n = 54) diagnosed with GDM by antenatal oral glucose tolerance test between July 1999 and January 2007. Data from laboratory records were used to collect investigations of glycaemic status during the follow-up period (median follow-up 12.4 years, range 9.5–17.1 years).ResultsOf 252 women, 102 (40.2%) did not have a FPG at 6 weeks (+/−2 weeks). Of these, median time to first test was 1.2 years (range 0.04–10.8 years), with only 43.1% followed-up within 1 year. In those who had a 6-week FPG, 17 (11.3%) women had no further tests. A total of 84 (33% of those with gestational diabetes in the index pregnancy) women were diagnosed with T2DM; median time from delivery to diagnosis was 5.2 years (range 0.35–15.95). We found the only significant factor for a follow-up test at 1-year post-partum was the use of insulin.ConclusionOur data suggest an alternative approach is needed for monitoring women with a history of GDM. This needs to be appropriate for a generally healthy group in which traditional screening mechanisms may not be adequate or sufficient.

Object Surgical treatment for periorbital inclusion cysts typically involves a brow, pterional, or partial bicoronal scalp incision for sufficient exposure. The authors have recently employed an endoscopy-assisted technique as an alternative approach intended to minimize the length of the skin incision and avoid scarring in the brow. Methods Children having typical clinical findings of a dermoid cyst located on the hairless forehead were selected to undergo endoscopy-assisted cyst removal. For suspected intradiploic lesions, MR imaging was used to assess osseous involvement. After induction of general anesthesia, a 1–2-cm curvilinear incision was made posterior to the hairline. A 30°-angled endoscope (4 mm) was then used for dissection in the subgaleal compartment. Subgaleal dissection was followed by a circumferential periosteal incision in which the authors used an angled needle-tip unipolar cautery. For lesions within the diploe, a high-speed air drill was used to expose the lesion. Complete removal was accomplished with curettage of either the skull or dural surface. Results Eight patients (5–33 months of age) underwent outpatient endoscopic resection. Seven cysts were extracranial, and 1 cyst extended through the inner table. In all patients complete excision of the cyst was achieved. There was negligible blood loss, no dural violation, and no postoperative infection. There have been no recurrences at a mean follow-up of 15 months. Conclusions Endoscopy-assisted resection of inclusion cysts of the scalp and calvaria is a safe and effective surgical approach. The technique results in negligible incisions with less apparent scarring compared with previously described incisions. This limited-access technique does not appear to be associated with a higher incidence of cyst recurrence.

The traditional approach to headache trigger management is to advise avoidance of all triggers, but we have advocated an alternative approach called ‘Learning to Cope with Triggers’ (LCT), in which the objective is to desensitise headache sufferers to some triggers or to build up tolerance for the triggers, using exposure techniques. A recent publication established the efficacy of this approach to trigger management. Reported here are three cases to illustrate how LCT is used in practice. Two cases were male and one was female, with ages ranging from 32 to 67 years. The headache diagnoses were frequent episodic tension-type headache, migraine without aura, and chronic tension-type headache; all had had headaches since childhood/adolescence. The headache triggers that were the focus of the intervention were heat, tiredness, and stress/anger. Post-treatment, changes in the capacity of the triggers to elicit headaches were reported in all three cases. Reductions in headaches from pre- to post-treatment, and from pre- to 4-month follow-up, were: case 1, 69% and 60% respectively; case 2, 76% and 80% respectively; and case 3, 73% and 61% respectively. Decreases in medication consumption, and enhanced self-efficacy were also recorded.

Two methods for solving coupled particle dynamics and flow field equations simultaneously by considering fluid-particle interactions to simulate two-phase flow are presented and compared. In many conditions, such as magnetic micro mixers and shooting high velocity particles in fluid, the fluid-particle interactions can not be neglected. In these cases it is necessary to consider fluid-particle interactions and solve the related coupled equations simultaneously. To solve these equations, suitable algorithms should be used to improve convergence speed and solution accuracy. In this paper two algorithms for solving coupled incompressible Navier-Stokes and particle dynamics equations are proposed and their efficiencies are compared by using them in a computer program. The main criterion that is used for comparison is the time they need to converge for a specific accuracy. In the first algorithm the particle dynamics and flow field equations are solved simultaneously but separately. In the second algorithm in each iteration for solving flow field equations, the particle dynamics equation is also solved. Results for some test cases are presented and compared. According to the results the second algorithm is faster than the first one especially when there is a strong coupling between phases.

Purpose: To report an alternative approach for rescue of an occluded aortofemoral bypass using the Gore Excluder Iliac Branch Endoprosthesis (IBE). Case Report: A 52-year-old man presented with acute right limb ischemia because of displaced and occluded iliac stents and was treated with aortofemoral bypass. On the third postoperative day, there was early bypass failure due to distal embolization from aortic thrombus. After fluoroscopy-guided balloon thrombectomy of the bypass, an endovascular bailout strategy was used. The Gore Excluder IBE was deployed below the renal arteries (with the external iliac limb opening in the surgical prosthesis and the gate opening within the aortic lumen). After antegrade catheterization of the gate, a Gore Viabahn endoprosthesis was inserted as the bridging endograft and deployed so that it landed just above the preimplanted aortoiliac kissing stents without overlapping them. Completion angiography showed technical success without complications; results were sustained at 1-year follow-up. Conclusion: The Gore Excluder IBE may represent a versatile solution for the rescue of complex cases when open surgery would be associated with a considerable risk. This off-label application of a well-recognized endovascular device is safe and feasible and may prove useful as a valuable alternative in properly selected patients.

We construct multisymplectic formulations of fluid dynamics using the inverse of the Lagrangian path map. This inverse map, the ‘back-to-labels’ map, gives the initial Lagrangian label of the fluid particle that currently occupies each Eulerian position. Explicitly enforcing the condition that the fluid particles carry their labels with the flow in Hamilton's principle leads to our multisymplectic formulation. We use the multisymplectic one-form to obtain conservation laws for energy, momentum and an infinite set of conservation laws arising from the particle relabelling symmetry and leading to Kelvin's circulation theorem. We discuss how multisymplectic numerical integrators naturally arise in this approach.

✓ Pediatric basilar invagination and cranial settling have traditionally been approached through a transoral–transpharyngeal route with or without extended maxillotomy or mandibulotomy for resection of the anterior portion of C-1 and the odontoid. The authors hypothesize that application of a recently described endoscopic transcervical odontoidectomy (ETO) technique would allow an alternative approach for the treatment of ventral pathological entities at the craniocervical junction in pediatric patients. The authors performed ETO in a consecutive series of pediatric patients presenting with myelopathy or bulbar dysfunction resulting from basilar invagination or cranial settling. All clinical, radiographic, surgical, and follow-up data were prospectively collected. The initial experience with ETO in the pediatric population is analyzed and outcomes are reported. Three patients required ETO for basilar invagination and 1 required ETO with anterior C-1 arch and distal clivus resection for cranial settling. All patients presented with myelopathy. One patient was wheelchair bound with severe quadriparesis. The mean age was 14 ± 3 years (mean ± standard deviation [SD]) in the 2 male and 2 female patients. The ETO and posterior fusion were performed as a 2-stage procedure in 2 (50%) and as a single-stage procedure in 2 (50%) cases. Prolonged intubation or postoperative placement of a gastrostomy tube was not needed in any case. The postoperative hospitalization lasted 9 ± 4 days (mean ± SD). At last follow-up (mean 5 months), head and neck pain had resolved and motor strength had improved or stabilized in all cases. All 4 children were independently functioning and ambulatory at the last follow-up. In the authors' initial experience, ETO has allowed ventral brainstem decompression without the need for prolonged intubation, worsening dysphagia requiring enteral tube feeding, or prolonged hospitalization, and has resulted in cosmetically appealing results. The ETO technique allows an alternative approach for the treatment of ventral pathological entities at the craniocervical junction in pediatric patients.

The mixing quality of a solid-liquid stirred tank operating in the turbulent regime was investigated, numerically and to an extent experimentally. Simulations were performed by coupling Computational Fluid Dynamics (CFD) and the Discrete Element Method (DEM). The results were evaluated against experimental data obtained using Electrical Resistance Tomography (ERT). This facilitated a novel and more rigorous assessment of CFD-DEM coupling – i.e. based on the spatial distribution of particle concentrations. Furthermore, a new mixing index definition was developed to quantify suspension quality to work in tandem with existing dispersion mixing indexes. This provides a more complete interpretation of mixing quality. In this work, it was found that the model underestimated suspension and dispersion due to model limitations associated with mesh size and fluid-particle interaction models. Furthermore, the predicted mixing quality was sensitive to changes in the drag model, including other fluid-particle interaction forces in simulations, and variations in certain particle properties

Introduction: The gingival melanin hyperpigmentation (GMH) is resulted from an abnormal deposition of melanin, but it is not a pathology. However, GMH is an esthetic problem for some people. Some alternatives of treatment for this situation exist. The epithelial abrasion has been an interesting alternative because it has a satisfactory esthetic outcome, is a fast procedure, of easy execution, and low cost. Recently, tips adapted in ultrasound (CVDentus) can bean alternative approach. Objective: To report a clinical case of GMH treated by the technique of the epithelial abrasion with association of instrument adapted in ultrasound. Case report: Patient aged 28 years, melanoderm, sought treatment due to esthetic dissatisfaction because of intense dark color in the maxillary gingiva. The GMH removal was proposed through the technique of the epithelialabrasion CVD bur. Conclusion: The technique of epithelial abrasion using CVD bur was effective in removing GMH at one-year follow-up showing to be easy and safe technique.
