Epidermoid cysts of the sole of the foot are rare lesions that must be differentiated from other, more common subcutaneous pathologic abnormalities located on the sole. Cases of epidermoid cysts that extend to the interosseous musculature are rarer still. We report the case of a giant epidermal cyst in a 64-year-old individual that extended to the intrinsic musculature of the third space of the right foot and that was diagnosed after fine-needle aspiration biopsy and subsequent cytologic study. Differential diagnosis of these lesions should be made with the support of additional imaging tests, and diagnostic confirmation should always be obtained after surgical removal and subsequent histopathologic study.

A 10-year-old Great Pyrenees was presented for anorexia and weight loss. On physical examination, the dog was emaciated and showed a large ulcerated lesion on the right lower lip in addition to an enlarged right testicle. Fine-needle aspiration biopsy of the testicle and surgical biopsy of the lip lesion were performed; the histopathological report was consistent with metastatic seminoma. The diagnostic and therapeutic approach in this unusual metastatic seminoma is presented and compared to the previous literature. A multimodality therapy consisting of surgery and chemotherapy is proposed for the clinical management of metastatic seminoma in dogs.

Latar Belakang: Pleomorfik adenoma (mixed tumor) adalah tumor kelenjar ludah yang paling sering terjadi (65%) pada kelenjar ludah mayor dan minor. Lokasi intraoral yang paling sering ditemukan adalah palatum. Tujuan: membahas gambaran klinis dan penatalaksanaan pleomorfik adenoma pada platinum. Kasus: dilaporkan 2 buah kasus pleomorfik adenoma pada palatum. Kasus pertama seorang laki-laki usia 29 tahun dengan benjolan pada palatum kanan ukuran 3x2 cm, konsistensi kenyal, berbatas tegas, warna seperti jaringan sekitar dan tidak nyeri. Pasien pernah menjalani operasi pada palatum kanan pada tahun 2005, dan kira-kira 3 tahun setelah operasi benjolan tersebut kambuh di tempat yang sama. Hasil biopsi aspirasi jarum halus adalah mixed tumor. Kasus kedua seorang wanita 22 tahun dengan benjolan pada palatum kiri ukuran 2x1,5 cm, timbul sejak 3 tahu yang lalu, warna seperti jaringan sekitar, konsistensi kenyal, dan tidak nyeri. Riwayat pesien menggunakan kontrasepsi hormonal. Hasil biopsi condong pada adenoma pleomorfik dengan bagian onkositik adenoma dan clear sel adenoma. Penatalaksanaan: dilakukan eksisi luas di bawah anestesi umum pada kedua kasus tersebut, dengan batas 1 cm dari tepi lesi pada jaringan sehat. Kesimpulan : telah dilakukan eksisi luas untuk penanganan kedua kasus pleomorfik adenoma dan palatum. Tidak di temukan rekurensi 1 tahun setelah operasi (kasus 1) dan 2 tahun setelah operasi (kasus 2). Blackground: pleomorphic adenoma (mixed tumor) is the most common tumor of the salivary glands (65%) of the major dan minor salivary glands. Palatum is the most common site in intraoral. Purpose: to elaborate clinical feature and management of pleomophic adenoma of palate. Cases: we reported 2 cases pleomorphic adenoma of palate. The first case was a29 years old male patient with a swelling at the right side of the palate, 3x2 cm sized mass, rubbery in consistency, well demarcated, pinkish in color, and pain less. He had undergone an operation at the right palate in 2005, but 3 years after the operation he got reccurence. The result of fine needle aspiration biopsy was mixed tumor. The second case was fermale 22 years old patient with a swelling at the left side of palate, 2x1,5 cm sized mass, was present 3 years before coming to the clinic, pinkish color, rubbery consistency and painless. She has been using hormonal contraception. The result of incisional biopsy was pleomorphic adenoma with the oncocityc adenoma part and clear cell adenoma. Management: widw exicion was performed under general anesthesia in both cases, with a limit 1 cm clinical margin at its periphery. Conclusion: both patients with pleomorphic adenoma ao palate were treated by wide excision. No recurrence were observed in 1 year (firs case) and 2 years (second case) after the surgery.

A five-month-old female Boerboel puppy presented with a rapidly expanding fibrous growth around the canine tooth of the right maxilla was diagnosed as having acanthomatous ameloblastoma based on findings from skull radiography, cytology of fine needle aspiration, biopsy and histopathology of resected growth. Lateral radiograph of the skull revealed moderate osteolysis of the maxilla with presence ofun-erupted maxillary canine teeth. Cytology of fine needle aspirate from the growth showed clusters of epithelial cells that were  hyperchromatic and showing palisade arrangement. The mass was surgically resected using diathermy instrument following general anaesthesia using combination of intramuscular injection of xylazine (0.5mg/kg), and intravenous injection of propofol (4mg/kg). Post-operative management included Ibuprofen suspension (100mg) administered for 3-days, intravenous vincristine (0.5mg/m2 ) once weekly and oral cyclophosphamide (50mg) was administered twice weekly for six weeks. Histopathology of the growth showed odontogenic epithelium embedded in pulp ectomesenchyme. Features of odontogenic epithelium include palisading epithelium with anti-basilar nuclei and centrally located cells having features of stellate reticulum. This is probably the first reported case of acanthomatous ameloblastoma in a Boerboel puppy that was successfully managed. Introduction. Intracranial epidermoid cysts are the most frequent congenital intracranial lesion. They rare and benign tumors that can present in different clinical situations depending on location and extension of the disease. Diagnosis is obtained with radiological imaging with RM and non-enhanced TC as elective investigating methods. Elective treatment is surgery, based on total/subtotal excision sparring healthy neurovascular structures, considering the benign nature of this lesion. Case Report. In this study we present the case of a 79-year-old woman affected by recidivist epidermal cyst of the posterior fossa. Clinical presentation was characterized by positional subjective vertigo, intense headache localized in the right part of the head increased by Valsalva maneuver and retroarticular subcutaneous swelling. Radiological investigation found a giant epidemoid cyst of the posterior fossa (8,4 x 4,8 x 5,8 cm), treated with surgery. In the postoperative, the patient was fine and no neurological deficit has been encounterd. REVIEW. In this study, we present a review of the literature regarding giant epidermoid cysts of posterior fossa. Only 11 cases were reported before ours, which actually is one of the largest ever described.

Abstract Objective. Suture granuloma, the rare complication of thyroidectomy, results from the use of nonabsorbable suture materials. Despite its typical ultrasound images and benign course, it carries utmost importance in the diff erential diagnosis of lymph nodes, recurrent nodules, and recurrence in the case of thyroid cancers. Subject and Results. Fifty four years old female patient, who underwent bilateral thyroidectomy in July 2010, was diagnosed with multinodular goiter and incidentally discovered micropapillary carcinoma (2 mm). Four years later, she was readmitted to hospital due to painless swelling in the right and left anterior neck region. Ultrasonography revealed nodules with irregular boundaries, containing micro- and macro-calcifications and hyperechoic lines in both sides of the thyroid bed and isthmus. Fine needle aspiration biopsy was performed in the right and left sided mass and the cytological examination was compatible with the diagnosis of the suture granuloma. Conclusions. Suture granuloma should be considered in the differential diagnosis of the local recurrence

BACKGROUND: Lipomas are benign formations with the mesenchymal origin, which are found in the head and neck region in a very small percentage of cases. In these cases, they usually occur in the posterior neck and usually grow very slowly without causing complaints. However, when they cover the front of the neck, it is necessary to confirm the adipose origin of a tumour and to exclude the connection with the thyroid gland. Although in principle, lipomas are benign, there are three more unfavourable possibilities of 1) malignancy of lipomas, 2) the occurrence of de novo liposarcomas or 3) the association of lipomas with other tumours, such as retinoblastoma. CASE REPORT: We present a 74-year-old woman with a painless subcutaneous formation in the neck. A Fine Needle Aspiration Biopsy (FNA) was performed, with the conclusion of the cytological result for lipoma. Surgical removal was planned under general anaesthesia. CONCLUSION: Usually, lipomas do not require surgery, but when they engage muscles they become indicative of surgical treatment.

A two-year-old, male domestic shorthair with a solitary lesion of the right forelimb was presented for fine-needle aspiration biopsy of a suspected cutaneous, neoplastic process. Neutrophils, lymphocytes, and acid-fast bacilli packed in the cytoplasm of foamy macrophages and giant cells were seen on cytological examination. Bacteriological culture of the material from the skin lesion was negative for Mycobacterium spp. Intraperitoneal injection of homogenized material from the lesion resulted in generalized mycobacteriosis in one mouse after 10 months. Based on these results, a diagnosis of feline leprosy was made. No medical or surgical therapy was performed. Nonetheless, the lesion showed progressive and complete spontaneous remission within 3.5 months from the time of diagnosis; after 14 months, the cat still is free of disease.

Hepatocellular carcinoma (HCC), the most common primary hepatic tumor, metastasizes in more than 50% of cases. However, metastasis of HCC to the submandibular glands is very rare. HCC rarely metastasizes to the salivary glands; only six cases of metastasis to the parotid gland have been reported in the English-language literature. Moreover, only one case of metastasis of HCC to the submandibular glands has been reported to date, and the affected patient died of hepatic failure. This clinical report describes a 55-year-old man who had undergone two surgeries for liver cancer and subsequently presented with a 2-week history of a right submandibular mass. We performed dissection of the right submandibular triangle, and metastatic HCC of the submandibular gland was diagnosed. Although HCC metastases to the oral cavity have been reported, this is only the second case of HCC metastasis to the submandibular gland. Fine-needle aspiration biopsy is recognized as a simple, direct approach for morphologic diagnosis of salivary gland lesions; however, histological and immunohistochemical examination of the surgical specimen remains necessary.

Abstract: Lipoma is rarely found in the oral cavity. Actually, lipoma is a benign tumor but it will create an operational procedure problem if its size is big and it grows in a difficult location. This tumor is usually found at the age of 40 – 60 years, more frequently in men, but the same for all races. Clinically, lipoma occurs as a mass with a smooth surface, yellowish to orange in color, and painless. The diagnosis of a lipoma is confirmed by using a CT scan that shows an irregular globular radioluscent mass with a clear border that seperates it from the surrounding tissues. The definite diagnosis should be based on a fine needle aspiration biopsy (FNAB) and a pathological examination. The management of an oral cavity lipoma is through an operation. We reported a case of a lipoma on the right side of the tongue of a 56-year-old male. The tumor was extirpated with a mandibulotomy approach, in which the mandible is cut to widen the operation field without removing the mandible bone. Lipoma merupakan tumor jinak namun akan menimbulkan masalah bila berukuran besar dan tumbuh di lokasi yang sulit untuk dilakukan operasi. Tumor ini sering ditemukan pada usia 40-60 tahun, lebih sering pada laki-laki, dan sama untuk semua ras. Secara klinis lipoma tampak sebagai massa dengan permukaan licin, berwarna kekuningan sampai jingga, dan tidak nyeri. Diagnosis untuk lipoma dilakukan dengan CT scan yang memperlihatkan massa ireguler globuler, radiolusen, dan ireguler, serta berbatas jelas dari jaringan sekitarnya. Diagnosis pasti ditegakkan dengan fine needle aspiration biopsy (FNAB) dan pemeriksaan patologi. Penanganan lipoma dalam kavum oris yaitu dengan operasi. Kami melaporkan kasus seorang laki-laki berusia 56 tahun dengan lipoma lidah. Tumor tersebut diekstirpasi dengan pendekatan mandibulotomi, dimana mandibula dipotong untuk meluaskan lapangan pandang operasi tanpa mengangkat tulang mandibula. Kata kunci: lipoma, lidah, mandibulotomi, ekstirpasi.

The investigation of thyroidal nodules is intended to select patients who are candidates for surgery, based on a cytological study with a good positive predictive value. Here we report the case of a 30-year-old woman presenting with a thyroid mixed nodule on the whole right lobule, cold on scintigraphy, and with no criteria of malignancy on cervical ultrasound and which cytological study did not reveal any suspicious cells. The 99mTc - methoxyisobutylisonitrile (MIBI) thyroid imaging showed a clear retention of MIBI in the cold nodule as described with technetium scan, motivating the realization of a partial surgery for which extemporaneous study favored papillary thyroid carcinoma, completed afterwards by a surgical removal. Thyroid MIBI imaging is a sensitive test for the detection of thyroid cancers and thus may be useful for improving the diagnostic accuracy of the thyroid nodules approach in the future and optimize invasive gestures from fine-needle aspiration biopsy to surgery.
