Keratitis is a very serious clinical condition caused by bacteria, fungi or parasites. It is associated with serious complications like blindness or endophthalmitis if remained undiagnosed. A 42-year-old male patient with the history of foreign body inoculation presented with mild pain and redness. After clinical examination, fungal keratitis was diagnosed. Corneal scrapping was sent for microbiological investigation. A very rare fungi Colletotrichum dematium was isolated and identified. Patient was managed with Natamycin tab fluconazole and atropin eye drops. To know the geographical distribution of the rare fungi, more and more cases should be studied and reported, which will help in management of rare fungi and standardisation of therapy.

Objective. To describe a rare case of an impacted large foreign body (chicken bone) in the laryngopharynx.Case Report. A 28-years-old man presented with pain in the neck of 5 days duration. The patient gave a history of severe choking sensation while eating chicken. Laryngoscopic examination revealed a linear whitish large chicken bone impacted in the left pyriform fossa. The bone was removed under local anesthesia with the guidance of telescopic laryngeal examination.Conclusion. This paper describes impaction of a large chicken bone in the hypopharynx in an adult male patient and its removal with guided telescopic laryngeal examination.

Abstract Background Leech infestation in the nose or nasopharyngeal region is a rare occurrence. The most common known cause is drinking water from natural water sources like ponds and rivers. Its hidden location of attachment in the nasopharynx and its uncommon occurrence make it easy to miss during diagnosis. Case presentation We present a case of a 61-year-old male patient with recurrent unilateral epistaxis without any apparent cause. He was diagnosed with leech infestation in the nasopharynx on endoscopic examination. This article reports the management of nasopharyngeal leech infestation and safety measures for this animate foreign body retrieval. Conclusion A vigilant approach, thorough history, and examination are a must. Though leeches are simple to remove most of the time, certain necessary precaution should be kept in mind for better management and prevention of further complications.

In the urinary tract, foreign body is most commonly found in the urinary bladder. It is commonly self-inflicted but can rarely be introduced by other person. Various types of foreign bodies have been reported, which includes infusion set, aluminum rod, gold chain, pearl, fish, pencil etc. Here we report a case of a 28-year young man who gave the history of forceful introduction of a long wire of mobile charger into the bladder by another person. It could not be removed by himself and by the local doctors. Then he was referred to Enam Medical College & Hospital and subsequently was removed by suprapubic cystostomy. Abstract Background: Corneal blindness is a major public health problem worldwide and infectious keratitis is one of its leading causes. The aim was is to analyze the microbial etiology of corneal ulcers and to determine the antibiotic susceptibility pattern of bacterial isolates in order to find out the existing drug resistance pattern. Methods: Corneal scrapings were collected as per the standard procedure. All the isolates were identifies by conventional microbiological techniques. Antibiotic susceptibility testing was performed using disc diffusion method. Clinical grading of the ulcerative lesion was carried out by the recommended protocol. Results: Among the 84 cases of clinically diagnosed infectious keratitis investigated, 33 (39.3%) were farmers by occupation. All cases were reviewed for predisposing conditions and clinical severity. Ulcers were categorized as mild in 49 cases moderate in 12 and severe in 23 cases. Predisposing conditions were found in 55(65.5%) of the 84 cases. Forty-two cases showed culture positivity. Among these 42; 25 (29.7%) yielded fungal growth, 13(30.1%) yielded bacteria and the rest 4 (4.7%) showed mixed growth. Aspergillus spp. (10/29; 34.5%) were the commonest fungi isolated followed by Fusarium spp. (6/29; 20%). Similarly, Staphylococcus aureus, (8/17; 47%) was the most common among bacteria. Majority i.e 66.7% (26 of 39) of those having trauma, as the predisposing factor developed moderate to severe degree of corneal ulceration as compared to only 20% (9 of 45) of those without any history of trauma. (p<0.001). Interestingly, 20(68.9%) of the 29 cases that had yielded fungal growth on culture had had previous history of ocular trauma in contrast to only 19 (34.5%) of the 55 who had non-fungal origin of keratitis. (p=0.003). Fungal keratitis cases presented with more severe form of ulcerative lesions exhibiting more often with infiltrates and hypopion as compared to bacterial keratitis. Topical administration of fourth generation fluoroquinolone eye drops remained the most effective drug of choice with clinical improvement in all cases without any complications. Conclusions: Trauma with vegetative matter predisposed to most cases of fungal keratitis. Severe form of ulceration was noticed in patients with fungal infection. Surveillance of the antibiotic sensitivity is needed for quality patient management.

Hypoglycaemia in the post-operative period is mainly iatrogenic (related to anti-hyperglycaemic drugs), but can be explained by an endogenous hyperinsulinemic state. In the context of previous gastrointestinal surgery, a form of dumping syndrome can mask hypoglycaemia from an underlying cause, such as an insulinoma. The authors present a clinical case of a male patient who underwent oesophageal surgery for an oesophago-gastric junction adenocarcinoma and developed hypoglycaemic symptoms in the post-operative period, caused by an undiagnosed insulinoma. This case report portraits the diagnostic investigation of a hypoglycaemia state in the post-operative period, narrowing to the workup of an endogenous hyperinsulinemic hypoglycaemia and provides a summary of insulinoma’s treatment. An insulinoma should always be considered in a patient with endogenous hyperinsulinemic hypoglycaemia, even with a history of oesophago-gastric surgery.

<p class="abstract">We had a sixty years old male patient, who had one year history of foreign body sensation in throat and the history of odynophagia for the last ten days. The NCCT neck showed 3.08×2.28 cm homogenous calcified mass in left tonsillar fossa. The stone was removed and tonsillectomy was performed. Giant tonsillolith is a rare clinical entity. As per available literature, 54 cases of giant tonsilloliths have been reported and to the best of our knowledge, this is one of the largest tonsillolith in the world till date. </p><strong>Keywords:</strong> Tonsillolith, Oral cavity<strong></strong>

Abstract Background Solar retinopathy is a disease that causes photochemical toxicity in the retinal fovea tissues, leading to an acute decrease of vision. Case presentation This case report is an interventional case of an asymptomatic 17-year-old Caucasian female with a history of suddenly decreased vision due to solar retinopathy. The patient was managed with a custom-made needle injection of triamcinolone acetonide in the suprachoroidal space. Four months post suprachoroidal injection showed an anatomical and functional improvement in the ellipsoid zone layer through optical coherence tomography signal reappearance. In addition, the best-corrected visual acuity had improved from 0.1 to 1.0 on the Snellen chart with the disappearance of the scotoma. However, there was a mild increase in intraocular pressure after this procedure, controlled with topical hypertensive eye drops. Conclusion Suprachoroidal triamcinolone acetonide injection using a custom-made needle showed both functional and anatomical improvement of macular changes post-solar retinopathy, with acceptable safety outcomes in a young female.
