Background: Physician burnout has garnered increased attention in recent studies. It is attributed to the intrinsic stresses of medical practice and affects the quality of patient care. Previous studies have reported roughly half of orthopedic surgery and anesthesiology faculty and residents suffer symptoms of burnout. Objective: The purpose of this study is to determine if there is a significant difference in burnout rates among orthopedic surgeons and anesthesiologists, between faculty and residents in both specialties, and possible associated factors that may predispose participants to experience burnout.     Methods: Data was gathered using the Maslach Burnout inventory survey (42 questions), which was distributed during the spring/summer of 2017 to orthopaedic surgery and anesthesiology residents and attending physicians, anesthesiology residents, orthopedic surgery faculty, and orthopedic surgery residents from various programs in the northeastern United States. Results: Survey Response rate was 238/666 = 38% response. As compared to attendings, residents scored: worse on Emotional Exhaustion; worse on Depersonalization and worse on Personal Accomplishment. When comparing specialties; for Emotional Exhaustion, Orthopaedic surgery scored better as compared with Anesthesiology. On Personal Accomplishment, Orthopaedic surgery scored better as compared with Anesthesiology. Residents had higher levels of burnout compared to attendings. Regarding specialty, Orthopaedic surgery scored significantly lower with regard to Emotional Exhaustion and Personal Accomplishment. It was also discovered that Orthopaedic surgeons overall have more social events, and more residents have mentors. Conclusion: Residents consistently demonstrated higher levels of burnout than attendings in both anesthesiology and orthopaedic surgery. Having a mentor and more interdepartmental social events may protect against burnout.

Introduction: Previous literature suggests that emergency medicine physicians experience high levels of work-related burnout. However, these results are drawn primarily from physicians working in large urban emergency departments. The aim of this study was to compare physician wellness between emergency medicine physicians working in urban versus rural settings. Methods: Emergency medicine physicians were recruited to complete a wellness survey from both urban and rural emergency medicine departments in Southwestern Ontario. The primary outcome measure of interest was physician burnout as measured by the Maslach Burnout Inventory-Human Services Survey (MBI-HSS). This survey tool measures physician burnout in the three domains of emotional exhaustion, depersonalization, and personal accomplishment. Descriptive statistics, paired t-tests and Mann-Whitney U tests were used to analyze parametric and non-parametric burnout domain data respectively. Results: Surveys were completed by 67/99 (68%) and 22/66 (33%) of urban and rural emergency medicine physicians, respectively. An emotional exhaustion score ≥27 OR a depersonalization sub-score ≥10 was considered the threshold for burnout and was found in 71.4% (40/56) of urban physicians surveyed and 85.7% (18/21) (P = 0.20) of rural physicians. No statistically significant difference in mean emotional exhaustion, depersonalization, or personal accomplishment was noted between groups. Conclusion: High levels of burnout were noted amongst both urban and rural emergency medicine physicians. No statistically significant differences were noted between groups when compared on the Maslach Burnout Inventory survey tool. Despite many factors differentiating urban from rural practice, rural emergency doctors suffer similar rates of burnout. Thematic qualitative interviews exploring specific burnout factors may offer further insight into the drivers of physician burnout.

The purpose of this study was to examine the job satisfaction and professional burnout levels of the national and international wrestling referees in terms of various factors in Turkey.The method of the study was the descriptive method. The study group consists of 126 international and national wrestling referees on active duty in 2010-2011 wrestling season. The questionnaire form was used as the data collection tool in the study. The questionnaire form consists of 3 sections, which are the Personal Information Form, the Maslach Burnout Inventory, and the Job Satisfaction Scale. It was observed abnormal distribution of dates of inventory and scale according to the Kolmogorov-Smirnov Test. Due to this reason, the Mann-Whitney U Test was used for the two independent samples test in analyzing the data; the Kruskal-Wallis H Test and the Mann-Whitney U Tests with Bonferroni correction were used for multiple comparisons; and the Spearman Correlation was used in analyzing the relations. The critical point for the significance value was accepted as α=0.05It was determined in the light of the findings of the study that the job satisfaction of the wrestling referees were at the medium level; the emotional exhaustion levels were high; the depersonalization levels were medium; and personal accomplishment  levels were low. Significant difference was found among the depersonalization levels of the referees according to the referee categories of the wrestling referees (p<0,05). A significant difference has been found among the monthly average income levels and the emotional exhaustion levels of the wrestling referees (p<0,05). A significant difference was also determined among the viewpoints of the referees on the financial income of the referees and the job satisfaction levels according to the social status (p<0,05). In addition, a reverse, significant and weak relation was found between the job satisfaction and emotional exhaustion levels of the referees (r= -0,255, p<0,01); a reverse, significant and weak relation between the depersonalization (r= -0,97, p<0,01); and a weak positive relation was determined between the personal accomplishment levels (r= 0,341, p<0,01).As a result, the national and international wrestling referees in Turkey do not have adequate satisfaction from being referees, and are experiencing professional burnout. As the job satisfaction levels of the wrestling referees increase, depersonalization levels and emotional exhaustion levels decrease, and personal accomplishment levels increase.

Objectives: To assess the prevalence of burnout and associated factors among family medicine residents in Thailand. Materials and Methods: This cross-sectional study was conducted by all Thai Family Medicine residents year 1 to 3 during February 2019. Self-reported questionnaires, including demographic data, and the Thai version of the Maslach Burnout Inventory were distributed to 703 residents via electronic transmissions, including e-mail, Facebook, and Line instant communication application. Burnout was diagnosed by the following criteria: high-level emotional exhaustion, high-level depersonalization, and low-level personal accomplishment. Factors associated with burnout were explored by the univariate logistic regression model. Multivariate logistic regression analysis was applied to examine the independent risk factors of burnout among Thai Family Medicine residents. Results: There were 149 residents who participated in this study, with a response rate of 21% (n = 703). As no residents diagnosed with burnout using the proposed criteria, burnout was, therefore, redefined as residents reporting high-level emotional exhaustion and high-level depersonalization. The prevalence of burnout in family medicine residents in this study was 10.74% (95% confidence interval [CI]: 6.26%-16.85%). Our study found that having relationship problems with patients, having relationship problems with colleagues, and having thought of resigning from the training program were independently associated with burnout with odds ratios of 6.93 (95% CI: 1.64-29.27), 6.31 (95% CI: 1.89-21.12), and 4.16 (95% CI: 1.09-15.81), respectively. Conclusions: Burnout at high level in emotional exhaustion and high level in depersonalization can occur among family medicine residents. Concerning factors were found to be patient and colleague relationship problems and having thought of resigning from the residency program. Other factors that may contribute to burnout were type of training programs, insufficient income, and family relationship. We recommend that the training institute should be able to monitor residents’ stress level and to help prevent those who have burnout and reduce its impact.

Abstract Objective This study aimed to determine the rate of physician burnout among maternal-fetal medicine subspecialists and identify associated factors. Study Design Noninternational members of the Society for Maternal-Fetal Medicine (SMFM) were surveyed regarding burnout using a validated tool specifically designed for health professionals (Maslach Burnout Inventory—Human Services Survey). Burnout was defined as exceeding established cutoffs for emotional exhaustion or depersonalization. Demographic information and potential contributors associated with the presence of burnout were also examined. Results The survey was sent to 1,220 members and 44.1% of members completed the entire survey. The physician burnout rate was 56.5%. Factors associated with burnout included female gender, being 5 to 20 years in practice, self-perceived burnout, being somewhat or very dissatisfied with career or supervisor and charting for more than 4 hours per day. Factors associated with less burnout were being male, having some protected time for education and regular exercise. Women scored higher on emotional exhaustion and depersonalization and lower personal accomplishment compared with men. Conclusion Physician burnout among the SMFM members is higher than has been reported in other specialties. While some of these factors are modifiable, further study into why women have higher rates of burnout needs to be performed.

ObjectiveTo determine the prevalence of physician burnout during the pandemic and differences by gender, ethnicity or sexual orientation.Design, setting and participantsWe conducted a cross-sectional survey (August–October in 2020) of internal medicine physicians at two academic hospitals in Vancouver, Canada.Primary and secondary outcomesPhysician burnout and its components, emotional exhaustion, depersonalisation and personal accomplishment were measured using the Maslach Burnout Inventory.ResultsThe response rate was 38% (n=302/803 respondents, 49% women,). The prevalence of burnout was 68% (emotional exhaustion 63%, depersonalisation 39%) and feeling low personal accomplishment 22%. In addition, 21% reported that they were considering quitting the profession or had quit a position. Women were more likely to report emotional exhaustion (OR 2.00, 95% CI: 1.07 to 3.73, p=0.03) and feeling low personal accomplishment (OR 2.26, 95% CI: 1.09 to 4.70, p=0.03) than men. Visible ethnic minority physicians were more likely to report feeling lower personal accomplishment than white physicians (OR 1.81, 95% CI: 1.28 to 2.55, p=0.001). There was no difference in emotional exhaustion or depersonalisation by ethnicity or sexual orientation. Physicians who reported that COVID-19 affected their burnout were more likely to report any burnout (OR: 3.74, 95% CI: 1.99 to 7.01, p<0.001) and consideration of quitting or quit (OR: 3.20, 95% CI: 1.34 to 7.66, p=0.009).ConclusionBurnout affects 2 out of 3 internal medicine physicians during the pandemic. Women, ethnic minority physicians and those who feel that COVID-19 affects burnout were more likely to report components of burnout. Further understanding of factors driving feelings of low personal accomplishment in women and ethnic minority physicians is needed.

Introducción: El objetivo del estudio fue identificar la prevalencia del Síndrome de Burnout y sus dimensiones en el personal de enfermería de unidades de cuidado crítico y de hospitalización.Metodología: Se llevó a cabo un estudio observacional descriptivo, transversal en 90 enfermeras y enfermeros. Se utilizó el Cuestionario de Maslach Burnout Inventory para el personal de salud. Se realizó análisis exploratorio y análisis descriptivo e inferencial; se emplearon los estadísticos U de Mann Whitney y Kruskal Wallis.Resultados: Se encontró nivel medio de Síndrome de Burnout 82.2%; Agotamiento Emocional bajo 62.2%; nivel bajo de Despersonalización 57.8% y nivel bajo de Falta de Realización Personal 40%. Se evidenció diferencia estadísticamente significativa entre Síndrome de Burnout y turno de trabajo, doble turno al mes, períodos vacacionales al año y carga de trabajo; entre Agotamiento Emocional y tipo de servicio, ingreso quincenal, doble turno al mes, períodos vacacionales, tipo de contratación y carga de trabajo; entre Despersonalización y carga de trabajo; y entre Falta de Realización Personal y tipo de servicio, gusto por el servicio, turno de trabajo, ingreso quincenal, períodos de descanso en el turno, períodos vacacionales al año y tipo de contratación.Conclusiones: Un porcentaje mayor del personal de enfermería obtuvo nivel medio de Síndrome de Burnout. No se encontró diferencia estadísticamente significativa entre el síndrome de Burnout y sus dimensiones con las características sociológicas del personal de enfermería. Se encontró evidencia de que las características laborales son las que presentan mayor influencia en el desarrollo del Síndrome de Burnout. Introduction: The objective of the study was to identify the prevalence of the Burnout Syndrome and its dimensions in the nursing personnel working in the critical care and hospitalization units.Methodology: A descriptive, observational, and cross-sectional study was performed in 90 nurses. The Maslach Burnout Inventory for health personnel was used. An exploratory, descriptive, and inferential analysis was carried out; the Mann Whitney U and Kruskal Wallis tests statistics were used.Results: A medium level regarding the burnout syndrome, 82.2%, was found; emotional exhaustion decreased by 62.2%; low level of depersonalization, 57.8%, and low level of lack of personal accomplishment, 40%. There was a statistically significant difference between burnout syndrome and work shift, double work shift per month, vacation periods per year, and workload; between emotional exhaustion and type of service, two-week income, double work shift, vacation periods, type of procurement, and workload; between depersonalization and workload; and between lack of personal accomplishment and type of service, work shift, two-week income, rest period during the shift, vacation periods per year, and type of procurement. Conclusions: A higher percentage of nursing personnel had a mild burnout syndrome. No statistically significant difference between burnout syndrome and its dimensions and the sociological characteristics of the nursing personnel was found. Evidence that job characteristics are those that show more influence in the development of the burnout syndrome was found.

Objective: To assess burnout in medical educators and to identify factors associated with it. Methods: A sequential mixed methods research study was conducted over eight months from July 2018 until February 2019. Participants included medical educators, who are studying for or graduated with a postgraduate qualification in medical education. An online questionnaire was developed using Maslach Burnout Inventory to collect quantitative data. The findings were explored in-depth qualitatively. Descriptive and inferential statistics were calculated for the quantitative data using SPSS 20. For qualitative data, we performed thematic analysis. Results: Of total 160 medical educationists, 101 responded giving 63.1% response rate. Mean age was 41.4 years and majority 53.5% were females. Overall aggregate mean burnout level was 12.34 ± 7.36 whereas sub-domains of Maslach burnout inventory( MBI) like i) emotional exhaustion, ii) depersonalization and iii) personal accomplishment were found out to be 19.59, 10.42 and 11.21 respectively. Most respondents had moderate 71 (70.3%) emotional exhaustion and 8 (8.9%) had severe emotional exhaustion. Average level of depersonalization was suffered by 73 (72.3%) respondents and severe level was observed in 20 (19.8%) respondents. Personal accomplishment was found low in all 101 (100.0%) respondents. Selective in-depth interviews revealed that coping mechanisms like social gatherings, indoor and outdoor game facilities and outings and leisure time should be strategized for faculties. Conclusion: In this study medical educators were found to have quite high level of burnout. The purpose of this study was to assess the reproducibility of scores on an instrument designed to measure physical therapy students’ burnout. Physical therapy students (28 juniors and 28 seniors) completed an adapted educators version of the Maslach Burnout Inventory on two occasions within a week interval. At each testing session, a separate score was obtained for each student for the three (emotional exhaustion, depersonalization, and personal accomplishment) subscales of the instrument. These scores, analyzed with a two-factor repeated-measures analysis of variance, indicated a significant difference in the personal accomplishment score between the junior and senior students during the two time frames. Scheffé post hoc tests showed that the junior students reported higher personal accomplishment affect at both testing sessions than the senior students. Both junior and senior students reported higher personal accomplishment at retest than at baseline testing. A test-retest reliability coefficient of .850 was obtained for the Depersonalization subscale; .907 and .715 were obtained for the Emotional Exhaustion and Personal Accomplishment subscales, respectively.

Introduction: Physician burnout in armed forces involves emotional exhaustion, depersonalization and a sense of declined personal accomplishment. This can have an adverse effect on quality patient care, the healthcare team and can cost physician health in both in-training physicians and practicing physicians. The causative factors include excessive long work shifts, inefficient work systems and clerical burdens, professional home conflicts, lack of departmental support, limited work force and poor leadership culture. Objectives: This study aims at measuring the prevalence of burnout in physicians working in King Salman armed forces hospital Saudi Arabia and studying possible related socio-demographic variables. Methods: A cross sectional study was conducted between April and May 2015 among physicians. A self‑administered questionnaire was used that includes questions on socio demographic characteristics, sources of stress and burnout of the Maslach Burnout Inventory-Human Services Survey (MBI-HSS) in this study. Student’s T-test and chi square tests were used for analysis. Results: Majority were males 74.8% aged more than 35 years with the prevalence rate of 14.2%. The analyzed variables associated with emotional exhaustion, the following factors significantly affected the EE with P value<0.05, exercise, alternate shift duty, work over load, quality of life, satisfaction with work and specialty. As for the significant factors associated with DP, shift duty, work overload, quality of life perception and specialty were found to have P value less than 0.05. Conclusion: Burnout is prevalent among physicians; we identified variables significantly associated with Emotional exhaustion (EE), Depersonalization (DP) and Personal accomplishment (PA). However, further research is recommended to study other predictors not mentioned in the current study and all health policy makers must work jointly in designing and implement effective remedial measures for physician burnout.

Owing to the nature of their jobs, nurses all over the world experience burnout. The aim of this descriptive and correlational study was to describe the job characteristics, job satisfaction and burnout levels of home care nurses, and to predict what factors contributed to their job satisfaction and burnout levels. The study population consisted of 80 nurses working in home care units. Of them, 71 participated in the study. A socio-demographic questionnaire, the Minnesota Satisfaction Scale and the Maslach Burnout Inventory were used. Of the participants, 85.9 per cent were female, 56.4 per cent had a bachelor’s degree, and 46.5 per cent were employed in the public sector, 36.6 per cent in municipalities and 16.9 per cent in the private sector. The results revealed that their burnout levels for emotional exhaustion and personal accomplishment were high, and moderate for depersonalisation. Perceived work-related stress was more associated with emotional exhaustion and depersonalisation than with work satisfaction. Home healthcare nurses were suffering from high levels of burnout. Interventions are needed to improve job satisfaction, to reduce the burden of burnout among nurses, and to prevent them from leaving their jobs and retiring earlier.
