Objectives: Glioblastoma (GBM) radiotherapy (RT) target delineation requires MRI, ideally concurrent with CT simulation (pre-RT MRI). Due to limited MRI availability, <72 h post-surgery MRI is commonly used instead. Whilst previous investigations assessed volumetric differences between post-surgical and pre-RT delineations, dosimetric impact remains unknown. We quantify volumetric and dosimetric impact of using post-surgical MRI for GBM target delineation. Methods: Gross tumour volumes (GTVs) for five GBM patients receiving chemo-RT with post-surgical and pre-RT MRIs were delineated by three independent observers. Planning target volumes (PTVs) and RT plans were generated for each GTV. Volumetric and dosimetric differences were assessed through: absolute volumes, volume-distance histograms, and dose-volume histogram statistics. Results: Post-surgical MRI delineations had significantly (p < 0.05) larger GTV and PTV volumes (median 16.7 and 64.4 cm3 respectively). Post-surgical RT plans, applied to pre-RT delineations, had significantly decreased (p < 0.01) median PTV doses (ΔD99% = −8.1 Gy and ΔD95% = −2.0 Gy). Median organ at risk (OAR) dose increases (brainstem ΔD5% =+0.8, normal brain mean dose =+2.9 and normal brain ΔD10% = 5.3 Gy) were observed. Conclusion: Post-surgical MRI delineation significantly impacted RT planning, with larger normal-appearing tissue volumes irradiated and increased OAR doses, despite a reduced coverage of the pre-RT defined target. Advances in knowledge: We believe this is the first investigation assessing the dosimetric impact of using post-surgical MRI for GBM target delineation. It highlights the potential of significantly degraded RT plans, showing the clinical-need for dedicated MRI for GBM RT.

Purpose: The purpose of this study was to evaluate the prevalence and the underreporting rate of sacroiliitis (SI) in a large cohort of patients with biopsy-proved Crohn’s disease (CD) who underwent magnetic resonance enterography (MRE) or computed tomography enterography (CTE). Materials and Methods: Patients with CD were recruited from eight Italian health centers in the period from January 2013 to December 2017. Disease activity was recorded according to the CD activity index (CDAI). The scans were read by two blinded readers who defined the presence of SI according to Assessment of SpondyloArthritis International Society (ASAS) classifications and European League Against Rheumatism (EULAR) recommendations. Moreover, SI was scored using a simplified Spondyloarthritis Research Consortium of Canada (SPARCC) scoring system. Results: Interobserver agreement in diagnosing SI on imaging was good (K = 0.72–0.83). SI was diagnosed in 129 (14.4%, 54 men, 75 women) out of 894 patients; however, sacroiliac joint (SIJ) abnormalities were not mentioned in the radiological reports of 112 patients (86%). Fifty (38.7%) out of 129 patients also underwent a subsequent SIJ evaluation through a dedicated MRI protocol to confirm SI. SI was found in a higher percentage of patients with “active” than “inactive” CD (18% vs. 4%). Conclusion: This study confirms the feasibility of CTE and MRE for the screening of SI in CD patients; however, it also underlines the remarkable problem concerning the underreporting of this entity in radiological practice.

Over the past years, magnetic resonance imaging (MRI) has become a cornerstone in evaluating anal canal and adjacent tissues due to its safeness, the three-dimensional and comprehensive approach, and the high soft-tissue resolution. Several diseases arising in the anal canal can be assessed through MRI performance, including congenital conditions, benign pathologies, and malignancies. Good knowledge of the normal anatomy and MRI technical protocols is, therefore, mandatory for appropriate anal pathology evaluation. Radiologists and clinicians should be familiar with the different clinical scenarios and the anatomy of the structures involved. This pictorial review presents an overview of the diseases affecting the anal canal and the surrounding structures evaluated with dedicated MRI protocol.

Abstract Pacinian corpuscles represent special nerve endings that serve as mechanoreceptors sensitive to vibration and pressure and are crucial for proprioception. This work demonstrates that the complex network of Pacinian corpuscles in hands and feet can be examined with three-dimensional Dual Echo Steady State (DESS) MR imaging at 7 T, while previous dedicated MRI reports were either limited to two-dimensional images or focused on the hands. The high-resolution MR images show the detailed architecture of the complex receptor network and reveal a “chain-like” arrangement of Pacinian corpuscles, a predilection for clustering around metacarpophalangeal/metatarsophalangeal joints, proximal phalanges and fingertips, and specific sensor locations both in the superficial subcutaneous tissue and adjacent to deep soft tissue structures such as tendons and joint capsules.

Background Orbital myositis is an idiopathic, non-infectious condition, typically seen in young females and usually affecting one extraocular muscle. Orbital myositis mimicking cluster headache is a rare clinical entity, and this is the first description of a case of a secondary trigeminal autonomic cephalalgia from orbital myositis responsive to high-flow oxygen. Case A young woman presented with new-onset, oxygen-responsive headache, periorbital pain and autonomic features. She had associated vertical diplopia on downgaze and subtle ocular misalignment. An initial diagnosis of cluster headache was made. Initial brain MRI was unrevealing, but dedicated MRI of the orbits showed enhancement of orbital muscles. The diplopia and the imaging findings were consistent with orbital myositis. Conclusion Orbital myositis mimicking cluster headache is rare, and not previously reported as an oxygen-responsive headache.
