The investigation demonstrates the influence ofperoral combined contraceptives and preparations containing only progestagens on carbohydrate and lipid exchange and haemostatic system. The factors, which exert influence upon metabolic indices in women with type I insular diabetes, are estimated. The correlation between the intake of combined and containing only progestagens contraceptive preparations and the risk of tromboembolism is revealed. The indications and contraindications are defined for the prescription of peroral contraceptives in women with type I insular diabetes.

Obesity and obesity-related low-grade inflammation are common findings in polycystic ovary syndrome (PCOS), the most common endocrine-metabolic disorder-affecting women in reproductive age. The terms metabolically healthy obese (MHO), and metabolically unhealthy obese (MUO) have been introduced to define individuals with obesity in whom cardio-metabolic risk factors are absent or present, respectively. To date, evidence investigating differences in body composition and adherence to the Mediterranean diet (MD) between MHO and MUO-PCOS women are lacking. Aim of this study was to better characterize the determinants of the metabolic health status in PCOS patients with obesity according to MHO and MUO phenotypes by evaluating endocrine-metabolic profile, inflammatory status, adherence to the MD, and body composition. The study population consisted of 94 treatment-naïve women with PCOS and obesity (BMI = 38.23 ± 6.62 kg/m2 and age = 24.12 ± 3.68 years). Compared PCOS MHO with PCOS MUO patients, the latter had higher levels of high-sensitivity C-reactive protein (hs-CRP) (p < 0.001), testosterone (p < 0.001), and insulin (p < 0.001), worse metabolic parameters, and higher Homeostatic Model Assessment of Insulin Resistance (HoMA-IR), Visceral Adiposity Index (VAI), and Fatty liver Index (FLI) (p < 0.001). Furthermore, PCOS MUO patients had lower adherence to the MD (p < 0.001) in spite of the same total energy intake (p = 0.102) as compared to PCOS MHO. The presence of MUO was associated with highest hs-CRP levels (OR = 1.49, p < 0.001), more severe hyperandrogenism and cardio-metabolic indices (p < 0.001). On the contrary, being PCOS MUO was associated with lower adherence to the MD (OR = 0.28, p < 0.001), and smaller PhAs (OR = 0.04, p < 0.001). Using a regression linear analysis model PREDIMED score entered at the first step (p < 0.001), followed by VAI (p < 0.001), and FLI (p = 0.032) in this analysis. At ROC analysis, a PREDIMED score of ≤4 (p < 0.001, AUC 0.926) could serve as a threshold for a significantly increased risk of presence the MUO-PCOS phenotype. To the best of our knowledge, this is the first study that characterized MHO and MUO-PCOS women on the basis of their adherence to the MD, body composition, and cardio-metabolic indices, providing evidence of the usefulness of adjunctive diagnostic parameters to better differentiate the MHO/MHO phenotypes in this cohort of PCOS patients with obesity.

Introduction. Working conditions, clinical and laboratory status of 236 workers of a chromium mine were investigated. Materials and methods. The observation group included 162 underground employees working in conditions of the combined impact of negative occupational factors (dust, noise, vibration, tension and severity there, cooling microclimate) - class 3.3-3.4. The comparison group included 74 mine employees working in acceptable working conditions - class 2. Results. In the observation group, the pathology of the cardiovascular and endocrine system (ICD-10: I00-I99, E00-E07) was detected 2.8-3.3 times more often (p = 0.001-0.02), the relative risk of disease formation was 2.7-3.2 times higher than in the comparison group (RR = 2.7-3.2; DI = 1.44-9.2; p = 0.001-0.02). Under the conditions of the combined effect of negative production factors, as features of metabolic disorders, it was noted that the most unfavourable - abdominal form of obesity was diagnosed 1.4 times more often (42.0%, p = 0.013), the atherogenic index was 1.2 times higher, and the level HDL is 1.2 times lower than in the comparison group (p = 0.017-0.047); “Metabolic indices” - lipid accumulation coefficient (LAP) and visceral obesity index (VAI) were 1.2-1.4 times higher than the values in the comparison group (p = 0.001-0.048). Conclusions. To implement a complex of medical and preventive measures aimed at increasing life expectancy and working longevity, early prevention of CVD, including among those working in the extraction of chrome ore, it is advisable to consider the introduction of the calculation of early markers of metabolic disorders - VAI and LAP indices in the medical examination programs.

Microglia and astrocytes are the glial cells of the central nervous system (CNS) to support neurodevelopment and neuronal function. Yet, their activation in association with CNS inflammation is involved in the initiation and progression of neurological disorders. Mild inflammation in the periphery and glial activation called as gliosis in the hypothalamic region, arcuate nucleus (ARC), are generally observed in obese individuals and animal models. Thus, reduction in peripheral and central inflammation is considered as a strategy to lessen the abnormality of obesity-associated metabolic indices. In this study, we reported that acute peripheral challenge by inflammagen lipopolysaccharide (LPS) upregulated the expression of hypothalamic dopamine type 2 receptor (D2R) mRNA, and chronic feeding by high-fat-diet (HFD) significantly caused increased levels of D2R in the ARC. The in vitro and in vivo studies indicated that an FDA-approved antipsychotic drug named trifluoperazine (TFP), a D2R inhibitor was able to suppress LPS-stimulated activation of microglia and effectively inhibited LPS-induced peripheral inflammation, as well as hypothalamic inflammation. Further findings showed daily peripheral administration intraperitoneally (i.p.) by TFP for 4 weeks was able to reduce the levels of plasma tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in accompany with lower levels of plasma glucose and insulin in obese mice receiving HFD for 16 weeks when compared those in obese mice without TFP treatment. In parallel, the activation of microglia and astrocytes in the ARC was also inhibited by peripheral administration by TFP. According to our results, TFP has the ability to suppress HFD-induced ARC gliosis and inflammation in the hypothalamus.

Objectives. The present study is aimed at investigating the frequency and associated factors of asymmetrical prominent veins (APV) in patients with acute ischemic stroke (AIS). Methods. Consecutive patients with AIS admitted to the Comprehensive Stroke Center of Shanghai Fourth People’s Hospital between January 2013 and December 2017 were enrolled. MRI including diffusion-weighted imaging (DWI), perfusion-weighted imaging (PWI), and susceptibility-weighted imaging (SWI) was performed within 12 hours of symptom onset. The volume of asymmetrical prominent veins (APV) was evaluated using the Signal Processing In nuclear magnetic resonance software (SPIN, Detroit, Michigan, USA). Multivariate analysis was used to assess relationships between APV findings and medical history, clinical variables as well as cardio-metabolic indices. Results. Seventy-six patients met the inclusion criteria. The frequency of APV ≥ 10   mL was 46.05% (35/76). Multivariate analyses showed that proximal artery stenosis or occlusion (≥50%) ( P < 0.001 , adjusted   odds   ratio   OR = 660.0 , 95 % CI = 57.28 -7604.88) and history of atrial fibrillation ( P < 0.001 , adjusted   OR = 10.48 , 95 % CI = 1.78 -61.68) were independent factors associated with high APV (≥10 mL). Conclusion. Our findings suggest that the frequency of APV ≥ 10   mL is high in patients with AIS within 12 hours of symptom onset. History of atrial fibrillation and severe proximal artery stenosis or occlusion are strong predictors of high APV as calculated by SPIN on the SWI map.
