Survival of Hybrid Laminate Veneers using two different tooth preparation techniques: Randomized Clinical Trial

Abstract The aim of this study was to evaluate the survival of laminate veneers constructed using a recent polymer-infiltrated ceramic network material following the aesthetic pre-evaluative temporary (APT) technique of tooth preparation in comparison to traditional technique. Six patients received 54 laminate veneers. They were divided into two equal groups (n=27) according to the technique of tooth preparation: group T: traditional technique and group A: aesthetic pre-evaluative temporary technique. VITA ENAMIC material was used for CAD/CAM construction of laminate veneers. Cementation was performed using a light cured resin cement. The laminate veneers were evaluated at baseline, after 3, 6 and 12 months according to the modified United States Public Health Service (USPHS) criteriea. The data was collected, tabulated and statistically analyzed. Secondary caries, endodontic complications, cracks and loss of retention were not noted in any laminate veneer. Extensive fractures were not detected in both study groups through the study period. There was a statistically significant decrease of color match criteria between the two groups at the 6 and 12 months recalls. Based on this study, both preparation techniques resulted in successful clinical performance. After 12 months, all the veneers in both groups showed no post-operative sensitivity and all patients were highly satisfied regarding their veneers. However, there was a deterioration in color match criteria through the study period in both study groups.

The objective of the present study was to evaluate the one-year clinical performance of lithium disilicate (LD) and zirconium dioxide (ZrO2) class II inlay restorations. Thirty healthy individuals who met the inclusion criteria were enrolled for the study. The patients were randomly divided into two study groups (n = 15): LD (IPS e.max press) and ZrO2 (Dentcare Zirconia). In the ZrO2 group, the internal surfaces of the inlays were sandblasted and silanized with Monobond N (Ivoclar, Leichsteistein, Germany). In the LD group, the internal surfaces of the inlays were etched with 5% hydrofluoric acid. The ceramic inlays were cemented with self-cure resin cement (Multilink N). Clinical examinations were performed using modified United State Public Health Codes and Criteria (USPHS) after 2 weeks, 4 weeks, 6 months and 1 year. The one-year survival rate was evaluated. In total, one failure was observed in the ZrO2 group. The survival probability after 1 year for the ZrO2 inlays was 93%, and for the LD inlays was 100%, which was statistically insignificant. The differences between both groups for most USPHS criteria (except for colour match) were statistically insignificant. Within the imitations of the present study, the lithium disilicate- and zirconia dioxide-based inlays exhibited comparable clinical performances. However, the colour and translucency match was superior for the lithium disilicate restorations.

Introduction. Discrepancy between the crown border and prepared tooth margin leads to a microleakage that eases the penetration of microorganisms and causes the dissolution of luting cement consequently. Several factors should be considered to achieve optimal fitness, including tooth preparation taper and type of cementing agent. The study aimed to determine the relation of tooth preparation taper and cement type on the microleakage of zirconia crowns. Materials and Methods. Fifty-six freshly extracted premolars without caries and restorations were selected as the study sample and divided into two groups of different tapering degrees (6 and 12 degrees). Zirconia copings were designed and fabricated by the CAD/CAM system. The samples were divided into four subgroups for cementation, and each subgroup was cemented with a different luting cement (n = 7). After 5000 thermocycles at 5°C–55°C and dye penetration, the specimens were sectioned in the mid-buccolingual direction, and a digital photograph of each section was taken under a stereomicroscope. Data were analyzed by the Kruskal–Wallis and Mann–Whitney tests (α = 0.05). Results. The results showed significant differences among the four types of luting cement in marginal permeability (PV < 0.001). Regardless of the type of cement, the 12-degree tapering resulted in a lower microleakage (46.4% without microleakage) with statistically significant differences from the 6-degree tapering (PV = 0.042). Conclusion. Within the limitations of this study, increasing the tapering degree of the prepared tooth for CAD/CAM zirconia copings improved the marginal fit and decreased the microleakage score. In addition, total-etch resin cement indicated the least microleakage.

The aim of the present study was to evaluate the clinical performance of two types of ceramics: a slurry-powder ceramic (Duceram Plus, Degussa) - D and a hot-pressed leucite-based glass-ceramic (IPS Empress, Ivoclar Vivadent) - IPS. Eighty-six restorations, 44 IPS and 42 D, were made by one operator. A total of 33 onlays and 53 inlays on twenty-seven premolars and 59 molars were cemented in 35 patients of both sexes, mean age 35 years. All restorations were cemented with the dual-resin cement (Variolink, Ivoclar-Vivadent) under rubber dam and were evaluated at the baseline and after six months, using the modified U.S.P.H.S. criteria for postoperative sensitivity, secondary caries, fracture, color match, marginal discoloration, marginal integrity and surface texture. Additionally radiographs and intraoral photographs were carried out. At baseline 86 restorations were analyzed and all of them received Alfa rating, except for the following that received Bravo rating for postoperative sensitivity - IPS (2.27%); D (7.14%); for color match - IPS (2.27%); D (2.38%) and for surface texture - IPS (2.27%); D (11.90%). After 6 months 100% of the restorations were analyzed and the following received Bravo rating: color match - IPS (4.55%) and D (9.52%); surface texture - IPS (2.27%) and D (11.9%); marginal discoloration - IPS (6.82%) and D (4.76%) and marginal integrity - IPS (4.55%) and D (7.14%). The results were submitted to the Fisher and McNemar Statistical Tests. No significant differences were noticed between the two ceramics. Both ceramics demonstrated satisfactory clinical performance after six months.

AbstractTo compare the 1-year clinical performance of lithium disilicate and resin composite CAD/CAM onlay restorations. Twenty patients that required two restorations in posterior teeth, with at least one cusp to be covered, received two onlays. One was made with IPS e.max CAD (Ivoclar-Vivadent) and the other with Lava Ultimate (3M Oral Care). Two blind observers evaluated the restorations at baseline and 1 year after the onlays were cemented, according to FDI criteria. At each recall, digital photographs, bite-wing radiographs and impressions of the restorations were taken for SEM evaluation of the interface. Results were analyzed by Mann–Whitney U and Wilcoxon tests (p < 0.05). At baseline and in the 1-year recall, both CAD/CAM materials exhibited excellent results in most criteria with similar esthetic, functional and biological properties (p > 0.05). However, deterioration in surface lustre (p = 0.020) and color match/translucency (p = 0.039) were detected for IPS e.max CAD onlays after 1-year. Under SEM evaluation, there were no statistically differences in micromorphological criteria at baseline nor after a year between IPS e.max CAD and Lava Ultimate onlays. Conclusion: After 1 year of clinical service IPS e.max CAD and Lava Ultimate onlays showed a similar clinical performance that needs to be confirmed in long-term evaluations.

Clinical Relevance Lithium disilicate veneers for esthetic restorations show great accuracy and similarity, regardless of the type of fabrication technique. SUMMARY Objectives: This study aimed to evaluate and compare the clinical performance of two different ceramic veneer methods: CAD/CAM (IPS e.max CAD) and heat-press (IPS e.max Press) at 6 and 12 months of follow-up, and the level of patient satisfaction after treatment. Methods and Materials: Patients were selected according to eligibility criteria, with a minimum of two and a maximum of six veneers per patient, for a total of 178 veneers randomized in two groups. A split-mouth, longitudinal, interventional, double-blind and single-center study was carried out according to the fabrication technique. Scores were attributed to the veneers according to the criteria of the United States Public Health Service (USPHS) regarding marginal adaptation, color change, marginal discoloration, restoration fracture, tooth fracture, restoration wear, antagonist tooth wear, presence of caries, and postoperative sensitivity. All patients answered a satisfaction questionnaire using the Visual Analogue Scale (VAS). Statistical significance was determined using two-way ANOVA and Tukey test, with a significance level of 5%. Results: The marginal adaptation criterion showed statistical difference between periods (p=0.017), regardless of the processing method (baseline means: CAD=1.056, PRESS=1.067, 6- to 12-month follow-up: CAD=1.089, PRESS=1.078). The other evaluated criteria showed no statistical differences between baseline and after 6 to 12 months. The level of satisfaction assessed by the VAS before and after treatment was 7.06 and 9.5, respectively. Conclusions: The two methods presented similar clinical performance after 12 months, and the patient’s level of satisfaction was considered high.

SUMMARY This study evaluated the five-year clinical performance of ceramic inlays and onlays made with two systems: sintered Duceram (Dentsply-Degussa) and pressable IPS Empress (Ivoclar Vivadent). Eighty-six restorations were placed by a single operator in 35 patients with a median age of 33 years. The restorations were cemented with dual-cured resin cement (Variolink II, Ivoclar Vivadent) and Syntac Classic adhesive under rubber dam. The evaluations were conducted by two independent investigators at baseline, and at one, two, three, and five years using the modified United States Public Health Service (USPHS) criteria. At the five-year recall, 26 patients were evaluated (74.28%), totalling 62 (72.09%) restorations. Four IPS restorations were fractured, two restorations presented secondary caries (one from IPS and one from Duceram), and two restorations showed unacceptable defects at the restoration margin and needed replacement (one restoration from each ceramic system). A general success rate of 87% was recorded. The Fisher exact test revealed no significant difference between Duceram and IPS Empress ceramic systems for all aspects evaluated at different recall appointments (p&gt;0.05). The McNemar chi-square test showed significant differences in relation to marginal discoloration, marginal integrity, and surface texture between the baseline and five-year recall for both systems (p&lt;0.001), with an increased percentage of Bravo scores. However, few Charlie or Delta scores were attributed to these restorations. In conclusion, these two types of ceramic materials demonstrated acceptable clinical performance after five years.
