THE “UNIT DISEASE”: AN ORGANIZATIONAL MODEL TO PROMOTE AND ENHANCE THE RESEARCH ACTIVITY AT THE PUBLIC HOSPITAL AND THE LOCAL HEALTH AUTHORITY IN ALESSANDRIA

Objective: to promote and enhance the research activity at the Public Hospital and the Local Health Authority in Alessandria, the translation of results into clinical practice, excellence and constant growth in the medical, health and administrative area, through the creation of research-oriented project groups.Methodology: the system development methodology refers to the Lean Thinking, and in particular to the X-Matrix, a framework in management studies to support the strategy definition and management process.Results: 14 project groups named “disease unit” were created, with own referents and components, planned and timed actions related to specific thematic areas. Members cooperated to increase scientific productivity, to promote the activation of clinical studies, the participation in funding national and European calls, the integration into international and European research networks and collaboration with universities, hospitals and IRCCS.Conclusions: the institution and coordination of the unit disease increased scientific production, supporting the collaboration and integration between the Public Hospital and the Local Health Authority with the University of Eastern Piedmont on the themes of research, innovation, experimentation and training, in relation to the way towards the IRCCS recognition.

Autoimmunità e Covid-19: uno studio prospettico monocentrico di follow-up

Obiettivi: In un nostro recente studio abbiamo riportato la presenza de novo di autoanticorpi in pazienti con diagnosi di COVID-19 ospedalizzati. Per verificare se questi autoanticorpi permangono nel tempo e/o portano allo sviluppo di malattie autoimmuni, abbiamo condotto uno studio di follow-up a 3 (t3) e 6 (t6) mesi dal ricovero. Metodologia: Tredici dei 40 pazienti arruolati nel precedente studio hanno dato il consenso a partecipare a questo nuovo protocollo. Ai pazienti sono stati eseguiti gli stessi tests di autoimmunità effettuati al momento della diagnosi. Risultati: In 9 pazienti continuano a persistere autoanticorpi circolanti a 6 mesi dalla diagnosi. Una paziente, negativa al momento del ricovero per tutti i tests di autoimmunità, ha sviluppato una forte positività per gli ANA e gli antigeni Mi2- β e Ku a t3 e a t6. Un altro paziente si è positivizzato a t3. Infine, due pazienti sono sempre rimasti negativi per tutto il periodo di monitoraggio. Conclusioni: i nostri dati dimostrano che, dopo sei mesi, la presenza di autoanticorpi permane ancora nella maggior parte dei pazienti. Ulteriori indagini saranno necessarie al fine di poter verificare se questi pazienti si negativizzeranno nel tempo o potranno sviluppare una sintomatologia clinica compatibile con l’insorgenza di una malattia autoimmune cronica. Inoltre, sarà importante capire, se l’alterazione del sistema autoimmune può essere considerata come uno degli effetti del coronavirus (long Covid-19).

Nutrizione enterale a bolo vs. nutrizione enterale continua nel paziente adulto: una revisione sistematica e metanalisi

Introduzione. L'alimentazione a bolo e continua sono strategie di somministrazione della nutrizione enterale comunemente utilizzate. Attualmente non vi sono dati sufficienti per definire quale sia il metodo più efficace per ridurre le complicanze gastrointestinali nei pazienti adulti. Obiettivo. Valutare l’impatto della nutrizione enterale a bolo rispetto a quella continua negli outcomes aspirazione, aumento del residuo gastrico, diarrea, distensione addominale, vomito, indice glicemico sul paziente adulto con indicazione alla nutrizione enterale prepilorica in ambito ospedaliero. Materiali e metodi. La ricerca bibliografica è stata effettuata sui database Pubmed, Embase, Cinhal e Cochrane. La meta-analisi è stata eseguita attraverso l’utilizzo del Review Manager V.5.3. Risultati. Cinque studi hanno mostrato che il tasso di diarrea era più alto nel gruppo di alimentazione a bolo (RR= 2.50, IC 95% = 1.17– 5.34, P = 0,02). Cinque studi hanno mostrato che il tasso di aspirazione era più alto nel gruppo di alimentazione continua (RR = 0.55 IC 95% 0.35,0.87 P= 0.01). Nessuna differenza significativa per gli altri outcome. Conclusioni. L'alimentazione intermittente potrebbe ridurre l'incidenza di aspirazione, tuttavia sembra associarsi a maggiore rischio di diarrea.

Il sistema di sorveglianza dell’antibiotico resistenza AR-ISS: uno strumento efficace per migliorare la gestione degli antibiotici

Obiettivi: analizzare i dati di resistenza agli antibiotici raccolti dal Laboratorio di Microbiologia e Virologia dell'Azienda Ospedaliera SS. Antonio e Biagio e Cesare Arrigo di Alessandria per gli anni 2019 e 2020. Metodi: negli ultimi decenni, l’antibiotico resistenza è aumentata a livello globale, diventando una delle maggiori sfide per la salute pubblica dei giorni nostri. Il fenomeno attualmente è inserito nella lista dell’Unione Europea dei problemi sanitari speciali ed è monitorato tramite il sistema di sorveglianza EARSNet. In Italia, il sistema di sorveglianza dell’antibiotico resistenza AR-ISS, coordinato dall'Istituto Superiore di Sanità, è uno strumento essenziale per studiare e descrivere la comparsa e la diffusione del fenomeno. Il sistema di si basa su una rete di laboratori ospedalieri presenti su tutto il territorio nazionale, che inviano dati di routine sulla suscettibilità agli antibiotici per patogeni selezionati da infezioni invasive. In questo lavoro, nell'ambito della rete AR-ISS, vengono presentati i report di sorveglianza annuali 2019 e 2020 dell'Azienda Ospedaliera di Alessandria tramite statistiche riassuntive ed istogrammi. Risultati: sono state analizzate le specie batteriche sotto sorveglianza e i dati del 2020 confrontati con quelli del 2019. Inoltre, viene valutata la tendenza europea ed italiana del quinquennio 2015-2019 per quattro combinazioni farmaco/batterio. Particolare attenzione è stata rivolta a possibili previsioni di resistenza agli antibiotici. Conclusioni: i programmi di sorveglianza forniscono informazioni attuali e storiche indispensabili per una corretta stewardship antibiotica; inoltre, possibili previsioni future della resistenza agli antibiotici potrebbero essere utili per guidare le decisioni sulle strategie di mitigazione.

La musica e il canto corale come terapia: l’esperienza del Fuckcancer Choir

La musicoterapia come parte integrante del percorso terapeutico-assistenziale dei pazienti oncologici e dei relativi caregivers può portare a una migliore gestione del dolore e dell’ansia, promuovendo il rilassamento, facilitando le capacità espressive e il corretto incanalamento delle emozioni e, in ultima analisi, offrendo supporto durante l’esperienza del lutto. Il FuckCancer Choir, nella realtà alessandrina, rappresenta un esempio di canto corale che apporta benefici nell’area psico-sociale riferiti dagli stessi partecipanti (pazienti, caregivers e operatori sanitari).

The experience of the clinical trial center at Alessandria Hospital regarding the activation of COVID-19 clinical trials during the pandemic

Objective: The aim is to evaluate the speed in the activation of Covid-19 clinical trials at SS. Antonio e Biagio e Cesare Arrigo Hospital of Alessandria during the pandemic. Methods: Data collection related to the activation and the conduction of clinical trials was managed using a database created through a web-based platform REDCap (Research Electronic Data Capture). Results: 32 studies were activated in the period between March 23 and July 31, 2020. An average time of 14 days elapsed between taking charge of the request and the issuance of the authorization act. Conclusions: During the emergency it was possible to activate the trials quickly thanks to fast-track procedures, optimizing COVID-19 clinical research.

PROGNOSTIC VALUE OF PRO-ADM BIOMARKER IN PATIENTS WITH COVID-19.

This study was performed at the analytical laboratory of the Hospital SS. Antonio e Biagio e Cesare Arrigo" Hospital in Alessandria (AL) in the period March-June 2020. This is an observational, prospective, single-center study carried out on a cohort of patients diagnosed with ARDS secondary to SARS-CoV2 infection admitted in the first wave of the pandemic at the Sub-Intensive Care Unit of the Hospital of Alessandria. OBJECTIVE The aim of this study was to assess plasma concentrations of the regional mean proadrenomedullin biomarker (MR-proADM) at the time of patient access to the Emergency Department and subsequently 1, 3 and 5 days after admission, to verify its prognostic value in terms of short-term mortality. The second objective of the study was to compare this biomarker with other commonly used biomarkers such as PCR and Procalcitonin, to evaluate the effectiveness of MR-proADM in risk stratification in these patients. METHODOLOGY Samples used in this study were collected in a tube containing the anticoagulant EDTA, and measurements were performed using a fully automated instrument: the Kryptor Compact Plus. RESULTS Among the various Biomarkers assayed only MR-proADM and LDH concentrations were significantly higher in deceased patients than in surviving patients while no statistically significant difference was observed for PCR PCT concentrations. Increasing levels of MRproADM between the second and fifth day of admission to the intensive care unit represent the key to determining mortality risk. CONCLUSIONS Mr-proADM was found to be a more effective biomarker than other commonly used biomarkers (PCR, WBC, Procalcitonin) in identifying Covid-19 patients with poor prognosis at an early stage. This study allowed clinicians to routinely include the assay of this biomarker, useful in the stratification of these patients and in predicting a higher or lower risk of mortality, in the second wave of the epidemic.

THE ALESSANDRIA PUBLIC HOSPITAL AND THE VALORIZATION OF ITS CULTURAL HERITAGE. WHAT BENEFITS?

Alessandria hospital is pursuing an extensive project to protect and to promote its cultural heritage. This historical heritage is consisting of architectures, paintings, statues, books, vases and much more, and it’s bearing witness to the long life of the hospital and the strong link with the city community.

FECAL MICROBIOTA TRANSPLANTATION: FAST PARASITES DIAGNOSIS BY MULTIPLEX PCR

Objectives: Evaluation of the analytical performance of the Novodiag Stool Parasites biomolecular assay for the detection of intestinal parasites in faeces of the faecal microbiota transplant donor. Methodology: Comparison between the gold standard for the diagnosis of intestinal parasites, light microscopy, and the Novodiag Stool Parasites biomolecular assay through the use of 22 fecal samples. Results: Twelve out of 22 samples were positive on microscopic examination. In order to assess the overlapping between the results obtained through parasitological examination and the molecular methodology, all samples were tested to the Novodiag Stool Parasites assay. Positive samples detected by biomolecular assay were 11 out of 22. Conclusion: The preliminary evaluation of the analytical performance of the Novodiag Stool Parasites kit can be considered promising. The Novodiag Stool Parasites kit can be effectively used as a tool in the fast diagnosis of intestinal parasitoses on the faecal microbiota transplant donor faeces.
