Abstract Background Frailty is an age-related condition resulting in a state of increased vulnerability regarding functioning across multiple systems. It is a multidimensional concept referring to physical, psychological and social domains. The purpose of this study is to identify factors (demographic characteristics, lifestyle factors and health indicators) associated with overall frailty and physical, psychological and social frailty in community-dwelling older people from five European countries. Methods This cross-sectional study used baseline data from 2289 participants of the Urban Health Center European project in five European countries. Multivariable logistic regression models were used to assess associations of the factors with overall frailty and the three frailty domains. Results The mean age was 79.7 (SD = 5.7). Participants who were older, were female, had secondary or equivalent education, lived alone, not at risk of alcohol use, were less physically active, had multi-morbidity, were malnourished or with a higher level of medication risk, had higher odds of overall frailty (all P < 0.05). Age was not associated with psychological and social frailty; sex was not associated with social frailty; smoking and migration background was not associated with overall frailty or any of its domains. There existed an interaction effect between sex and household composition regarding social frailty (P < 0.0003). Conclusions The present study contributed new insights into the risk factors for frailty and its three domains (physical, psychological and social frailty). Nurses, physicians, public health professionals and policymakers should be aware of the risk factors of each type of frailty. Furthermore, examine these risk factors more comprehensively and consider overall frailty as well as its three domains in order to further contribute to decision-making more precisely on the prevention and management of frailty. Trial registration The intervention of the UHCE project was registered in the ISRCTN registry as ISRCTN52788952. The date of registration is 13/03/2017.

bjective: To determine the prevalence of raised ALT, common causes, and associated fetomaternal morbidity  in pregnant mothers presenting, at cantonment general hospital Rawalpindi Materials and Methods: This was a cross-sectional study conducted at cantonment general hospital Rawalpindi from July 2016 till June 2017. Results: Out of 1924 women, 102 were identified with raised ALT making a prevalence of 5.3%. Sixty-one (59.8%) were booked. The hypertensive group which included severe preeclampsia, chronic hypertension with superimposed preeclampsia/eclampsia were 55(53.9%), intrahepatic cholestasis of pregnancy(ICP) 32(31.7%), acute viral hepatitis 9(8.8%), Acute fatty liver of pregnancy(AFLP) 2(1.96%), and unknown cause in 4(3.92%). Mean ALT levels were 54.1±6.94, 71.28±23.25, 84.22±27.82, 231.5±47.37 respectively. In four cases no definitive cause could be identified with the available tests were labeled as an unknown group, having a mean ALT level of 79.25±10.07. (p=0.01). Term delivery occurred in 71(69.6%), while 31(30.39%) were preterm. There was one termination of pregnancy. Vaginal birth occurred in 42(42.2%), and 53(51.9%) underwent emergency cesarean. There was one peripartum hysterectomy. Meconium stain of liquor was 19(18.6%). The birth weight of most babies 73(71.5%) was between 2-3 kilograms only three were ≤ 1 kilograms. Eight cases of postpartum hemorrhage, three maternal deaths, and six perinatal/early neonatal deaths were observed. Conclusion: Raised ALT in pregnancy leads to increased fetomaternal complications. Severe preeclampsia and obstetric cholestasis were the commonest causes. Women of younger age groups were having acute viral hepatitis. Timely recognition and diagnosis are essential to institute appropriate management strategies.

ObjectiveTo examine the rinsing and gargling mouthwash practices among frequent mouthwash users to determine if there are differences in use between gender, sexual orientation and sex work status.DesignCross-sectional study.SettingData obtained from patients attending a sexual health centre located in Melbourne, Australia.Participants200 frequent mouthwash users (four or more times per week), 50 for each of the following patient groups: men who have sex with men (MSM), female sex workers (FSW), females who are not sex workers and men who have sex with women only (MSW). Participants were observed and audio recorded using mouthwash.Primary and secondary outcome measuresDescriptive analyses were conducted to calculate the median age, time rinsing and gargling, amount of mouthwash used and proportion of participants who rinsed, gargled or both, as determined from the audio files. Kruskal-Wallis H test and χ2 test were used to examine differences between the patient groups.ResultsMedian age was 28 years (IQR: 24–33). During the study, most (n=127; 63.5%) rinsed and gargled, but 70 (35.0%) rinsed only and three (1.5%) gargled only. Median time rinsing was 13.5 s (IQR: 8.5–22.0 s), gargling was 4.0 s (IQR: 2.5–6.0 s) and the median total duration was 17.0 s (IQR: 11.5–25.8 s). Median duration of mouthwash did not differ significantly between the groups (females not sex workers: 18.8 s (IQR: 12.5–24.5 s); FSW: 14.0 s (9.0–22.0 s); MSM: 22.3 s (13.0–26.5 s); MSW: 15.8 s (12.0–25.0 s); p=0.070) but males used mouthwash longer than females (median 20.3 s compared with 15.5 s; p=0.034). The median volume of mouthwash used was 20 mL (IQR: 15–27 mL). And most (n=198; 99.0%) did not dilute mouthwash with water.ConclusionOver a quarter of frequent users do not gargle mouthwash at all (35%) and used it for a substantially shorter period of time than it was used in the randomised trial (1 min) where it was shown to be effective at inhibiting Neisseria gonorrhoeae growth. Our findings suggest that many frequent mouthwash users do not follow the manufacturer instructions for using mouthwash and may not use mouthwash in a way that was shown to reduce the growth of oropharyngeal gonorrhoea.
