The article deals with the problems of strategic management of an economic entity, the formation of its information base on the basis of the system of General quality management (TQM). Special attention is paid to the balanced scorecard (BSC) as the most promising direction of accumulation and obtaining information necessary for making informed management decisions in the short and long term, which is especially important in an unstable economic situation.

Today, when the concepts of general quality management science are applied internationally, the certification of quality management systems has become not only the advantage but also the necessity in contemporary competition environment. The conclusions of researches carried out abroad implies the hypothesis that a part of the companies lacks a quality development experience and, as a consequence, are not able to employ all possibilities offered by ISO standards; such standards often become at least partially the attribute of formal administration. The comparison of 2001–2005 year indicators, describing the effectiveness of international standards in organisation of Lithuania with other countries provides the conclusions on the possibilities for a faster and more effective employment of international experience, while working according to the standards of ISO series.

Aims and methodWe explored the views of foundation doctors on psychiatry placements to inform further post development. Following criticism of some South Thames Foundation School (STFS) psychiatry placements, STFS staff reviewed existing information on post quality and 21 foundation doctors in psychiatry posts took part in three focus groups.ResultsTrainees are concerned about the general quality of posts (including supervision and induction); isolation of mental health trusts from ‘acute’ trusts; the professional position of junior psychiatrists; and responsibilities related to the Mental Health Act and risk assessment. Requirements for posts to address these issues have been developed and are now being implemented locally.Clinical implicationsThe conclusions are relevant to the quality management of foundation posts in mental health trusts, responses to the Collins Report (2010) and future recruitment into psychiatry.

Rinkos dalyviai, reaguodami á ðiuos pokyèius, ieðko bûdø ne tik uþtikrinti, bet ir virðyti ðiuos reikalavimus. Organizacijos yra nevarþomos pasirinkti, kaip siekti kokybës – diegti ISO 9000 serijos standartus ar taikyti visuotinës kokybës vadybos nuostatas. Visuotinë kokybës vadyba vertinama kaip viena efektyviausiø organizacijos valdymo formø, o ISO 9000 serijos standartø populiarumà lemia operacinio organizacijos lygmens optimizavimo galimybës ir tarptautiniu mastu pripaþástamas sertifikatas, liudijantis organizacijos pastangas siekti kokybës. Reacting to these changes, the participants of the market are looking for the ways not only to ensure but also to transcend these requirements. Organizations are free to choose how to reach the quality – to instill the ISO 9000 series standards or to apply Total Quality Management regulations. Total Quality Management is estimated as one of the most efficient forms of organization management, and the popularity of ISO 9000 series standards is conditioned by the optimization possibilities of organization operating level and the internationally recognized certificate which proclaims the organization attempts to reach the quality. However, ISO 9000 standards are often criticized – purportedly they determine the manifestation of bureaucracy but not the improvement. Criticism on this aspect is fostered by the fact that the theory of general quality management does not regulate the necessity of documentation. The aim of the article is to assess whether the documents, from the quality management viewpoint, generate this surplus value or are only selfobjective activity towards obtaining the certificate.

Background: We surveyed the quality of risk stratification politics and monitored the rate of entries to our company-wide protocol for venous thrombembolism (VTE) prophylaxis in order to identify safety concerns. Patients and methods: Audit in 464 medical and surgical patients to evaluate quality of VTE prophylaxis. Results: Patients were classified as low 146 (31 %), medium 101 (22 %), and high risk cases 217 (47 %). Of these 262 (56.5 %) were treated according to their risk status and in accordance with our protocol, while 9 more patients were treated according to their risk status but off-protocol. Overtreatment was identified in 73 (15.7 %), undertreatment in 120 (25,9 %) of all patients. The rate of incorrect prophylaxis was significantly different between the risk categories, with more patients of the high-risk group receiving inadequate medical prophylaxis (data not shown; p = 0.038). Renal function was analyzed in 392 (84.5 %) patients. In those patients with known renal function 26 (6.6 %) received improper medical prophylaxis. If cases were added in whom prophylaxis was started without previous creatinine control, renal function was not correctly taken into account in 49 (10.6 %) of all patients. Moreover, deterioration of renal function was not excluded within one week in 78 patients (16.8 %) and blood count was not re-checked in 45 (9.7 %) of all patients after one week. There were more overtreatments in surgical (n = 53/278) and more undertreatments in medical patients (n = 54/186) (p = 0.04). Surgeons neglected renal function and blood controls significantly more often than medical doctors (p-values for both < 0.05). Conclusions: We found a low adherence with our protocol and substantial over- and undertreatment in VTE prophylaxis. Besides, we identified disregarding of renal function and safety laboratory examinations as additional safety concerns. To identify safety problems associated with medical VTE prophylaxis and “hot spots” quality management-audits proved to be valuable instruments.

Abstract. Borderline Intellectual Functioning (BIF) refers to a global IQ ranging from 71 to 84, and it represents a condition of clinical attention for its association with other disorders and its influence on the outcomes of treatments and, in general, quality of life and adaptation. Furthermore, its definition has changed over time causing a relevant clinical impact. For this reason, a systematic review of the literature on this topic can promote an understanding of what has been studied, and can differentiate what is currently attributable to BIF from that which cannot be associated with this kind of intellectual functioning. Using Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) criteria, we have conducted a review of the literature about BIF. The results suggest that this condition is still associated with mental retardation, and only a few studies have focused specifically on this condition.
