SummaryWork plays an important part in everyday life. For people experiencing mental health problems employment may both provide a source of income, improved self-esteem and stability, and influence the course and outcomes of the disorder. Yet in many countries the work-place consistently surfaces as the context where people with mental health problems feel stigmatised and discriminated the most. This paper will review the existing evidence of stigma and discrimination in the workplace, consider the consequences of workplace stigma on the lives of people experiencing mental health problems, and discuss implications for further action.

People with mental illness often encounter stigma and discrimination. Mental illness-related stigma and discrimination represent the enormous obstacles that stand in the way of delivering mental health care. Little is known about stigma and discrimination toward mental illness in rural Sarawak. Thus, this study aimed to identify the stigma and discrimination towards mental illness among Sarawak’s rural community and the factors affecting them. A cross-sectional survey was carried out on 840 respondent adults aged 21 and above. A validated Community Attitude towards Mental Illness (CAMI) and Discrimination towards Mental Illness questionnaire was used for data collection. A Two-way Multiple Analysis of Covariance (Two-way MANCOVA) test was carried out to determine how much gender and education level influence stigma and discrimination towards mentally ill patients. Analysis showed that there was a statistically significant difference of standardised stigma and discrimination scores with education level (p<.001), but not with gender (p>.05). Age had a positive linear effect on both stigma and discrimination, whereas experience with mentally ill patients negatively affected both stigma and discrimination. Individuals with a lower level of education, older in age, and lesser experience in dealing with the mentally ill would have a higher level of stigma and discrimination towards mental illness. Thus, targeted and practical strategies need to be organised and implemented to combat mental illness-related stigma and discrimination.

Besides health and social costs, mentally ill, often, are also victims of stigma and discrimination, among many communities in Africa. Cultural beliefs, stereotypes are some of the social constructions used to perpetuate mental illness stigma. The purpose of this study was to describe the nature of stigma communication associated with mental illness, beliefs and stereotypes underpinning mental illness stigma. Generally, the findings indicate nature of mental illness stigma communication is an intersection of stigma messages, cultural beliefs, and stereotypes associated with mental illness. Cultural beliefs associated with mental illness are inextricably intertwined with the perceived causes of mental illness (which include curses, witchcraft, cultural misdemeanor, and possession by spirits or demons). Symptoms of mental illness (mainly aggression and nudity) mark the mentally ill as different and expose them to labeling by the rest of the community. Generally, the mentally ill are stereotyped as aggressive, symbol of shame, and unpredictable.

This chapter reviews the experiences of women with serious mental illness and the various disparities encountered by them. These disparities include higher rates of victimization, unemployment, poverty, homelessness, and inequities in clinical care. The impact of these disparities and associated stigma on the acceptance process is presented. This chapter centers on a discussion of intersectional stigma, or overlapping, multiple levels of stigma and discrimination, faced by women with serious mental illness. Several participant case narratives are presented in order to demonstrate the impact of intersectional stigma on the process of acceptance for women with serious mental illness. Clinical recommendations are provided to facilitate acceptance among women who experience intersectional stigma. A clinical strategies list, discussion questions, activities, the “Intersectional Stigma Worksheet,” and an explanatory table are included at the close of the chapter.

Abstract Background Reducing stigma is a perennial target of mental health advocates, but effectively addressing stigma relies on the ability to correctly understand and accurately measure culture-specific and location-specific components of stigma and discrimination. Methods We developed two culture-sensitive measures that assess the core components of stigma. The 40-item Interpersonal Distance Scale (IDS) asks respondents about their willingness to establish four different types of relationships with individuals with 10 target conditions, including five mental health-related conditions and five comparison conditions. The 40-item Occupational Restrictiveness Scale (ORS) asks respondents how suitable it is for individuals with the 10 conditions to assume four different types of occupations. The scales – which take 15 min to complete – were administered as part of a 2013 survey in Ningxia Province, China to a representative sample of 2425 adult community members. Results IDS and ORS differentiated the level of stigma between the 10 conditions. Of the total, 81% of respondents were unwilling to have interpersonal relationships with individuals with mental health-related conditions and 91% considered them unsuitable for various occupations. Substantial differences in attitudes about the five mental health-related conditions suggest that there is no community consensus about what constitutes a ‘mental illness’. Conclusions Selection of comparison conditions, types of social relationships, and types of occupations considered by the IDS and ORS make it possible to develop culture-sensitive and cohort-specific measures of interpersonal distance and occupational restrictiveness that can be used to compare the level and type of stigma associated with different conditions and to monitor changes in stigma over time.

SummaryThis article discusses the evidence that experiences of stigmatisation and discrimination among people with mental illnesses are common and may be severe. Furthermore, there are growing concerns that people with mental illness receive second-class physical healthcare. Beyond this, some aspects of psychiatric practice are reported as being insensitive, disrespectful or even disabling. We consider whether such claims are justified and what psychiatrists can do, directly and indirectly, to reduce stigma and discrimination and improve our practice.
