Cesarean Delivery and Risk of Excess Weight Among Brazilian Preschool Children

Objective: We aimed to assess the weight status and to determine predicted factors of excess weight among Tunisian preschool children. Methods: We conducted a cross-sectional study among preschoolers attending kindergarten during 2016-2017 school-years in Sousse, Tunisia. The children’s parents completed the self-administered questionnaire. Anthropometric measures (Weight and height) were taken for the children. Weight status considered body mass index (BMI) and excess weight was defined by the International Obesity Task Force (IOTF) standards. Odds ratios for excess weight were calculated using binary logistic regression analyses. Results: A total of 364 preschoolers were included with a mean age 4.49±0.9 years. The ratio boys/girls was 1.05. The prevalence of excess weight was 31.3%; CI95%:[26.7%-36.5%]). Female gender (AOR=1.74; CI95%:[1.07-2.83]), working mother (AOR=1.75;CI95%:[1.04-2.92]), sedentary behavior (AOR=1.87;CI95%:[1.01-3.51]), eating sweets (AOR=1.95;CI95%:[1.19-3.19]), eating while watching television (AOR=2.53;CI95%:[1.23-5.18]) were positively associated with greater risk of excess weight while sleep duration<10h (AOR=0.41;CI95%:[0.24-0.71]) and less educated mother (AOR=0.35;CI95%:[0.12-0.96]) were negatively associated with excess weight. Conclusion: Our results show that the prevalence of overweight and obesity among preschoolers was alarming. Prevention of obesity should start early in life through a multisectoral approach.

Objective: to evaluate the relationship between body composition of preschool children suffering from excess weight and birth weight (BW). Methods: probabilistic sample, by conglomerates, with 17 daycare centers (of a total of 59) composing a final sample of 479 children. We used Z-score of Body Mass Index (zBMI) ≥ +1 and ≥ +2, respectively, to identify preschool children with risk of overweight and excess weight (overweight or obesity). The arm muscle area (AMA) and the arm fat area (AFA) were estimated from measurements of arm circumference, triceps skin fold thickness. Results: the prevalence of risk of overweight was 22.9% (n=110) and excess weight was 9.3% (n=44). The risk of overweight and excess weight in children did not show correlation between BW and AFA, but it did with adjusted arm muscle area (AMAa) (rp= 0.21; p= 0.0107). The analysis of the group with excess weight alone also showed a positive correlation between BW and AMAa (rp= 0.42; p= 0.0047). Conclusion: among overweight children, lower BW is associated with a lower arm muscle area in early preschool age, regardless of the fat arm area presented by them.

Objective. After declining for many years, cesarean delivery rates recently increased. To explore whether this increase is associated with excess weight gain during pregnancy, resulting in macrosomic infants who require cesarean delivery, we examined trends in excess weight gain, macrosomia, and cesarean delivery. Methods. Analysis of 1990–2000 US Natality Files of birth certificate data were restricted to first birth, singleton infants of 37 to 42 weeks’ gestation to avoid confounding by repeat cesarean delivery, complications of multigestational pregnancy, and preterm and postterm birth. Excess weight gain was defined according to current guidelines (41+ lb) and macrosomia as birth weight &gt;4000 g. Results. From 1990–2000, excess weight gain rose steadily from 18.6% to 24.2%. There was a 19.3% decline in macrosomic infants among women who gained excess weight compared with an 11.9% decline among women who gained 15–40 lb, although the absolute risk remained substantially greater among women who gained excess weight (eg, 14.2% vs 7.2%, in 2000). From 1990–1997, cesarean delivery declined by 20.2% among women who gained excess weight compared with 15.7% among women who gained 15 to 40 lb. After 1997, cesarean delivery increased in all weight gain categories, and absolute risks in 2000 were 25.8% for women who gained excess weight compared with 21.6% for women who gained 15–40 lb. Overall, women who gained excess weight accounted for 24.1% of cesarean deliveries in 1990 and 28.1% in 2000. Conclusions. Excess weight gain and macrosomia do not seem to be the primary factors that contribute to the recent increase in cesarean delivery because cesarean delivery rates have increased in all weight gain categories and macrosomia rates have decreased steadily from 1990–2000. Nonetheless, women who gain excess weight account for a growing proportion of cesarean deliveries because their relative numbers have grown.

Healthcare professionals require a thorough understanding of stuttering since they frequently play an important role in the identification and differential diagnosis of stuttering for preschool children. This paper introduces The Preschool Stuttering Screen for Healthcare Professionals (PSSHP) which highlights risk factors identified in the literature as being associated with persistent stuttering. By integrating the results of the checklist with a child’s developmental profile, healthcare professionals can make better-informed, evidence-based decisions for their patients.
