Background Multimorbidity is associated with physical-mental health comorbidity (PMHC). However, the scope of overlap between physical and mental conditions, associated factors, as well as types of mental illness involved are not well described in Eastern Europe. This study aims to assess the PMHC burden in the Estonian population. Methods In this population-based cross-sectional study we obtained health claims data for 55 chronic conditions from the Estonian Health Insurance Fund (EHIF) database, which captures data for all publicly insured individuals (n = 1 240 927 or 94.1% of the total population as of 31 December 2017). We assessed the period-prevalence (3 years) of chronic physical and mental health disorders, as well as associations between them, by age and sex. Results Half of the individuals (49.1% (95% CI 49.0–49.3)) had one or more chronic conditions. Mental health disorders (MHD) were present in 8.1% (8.1–8.2) of individuals, being higher among older age groups, women, and individuals with a higher number of physical conditions. PMHC was present in 6.2% (6.1–6.2) of the study population, and 13.1% (13.0–13.2) of the subjects with any chronic physical disorder also presented with at least one MHD. Dominating MHDs among PMHC patients were anxiety and depression. The prevalence of MHD was positively correlated with the number of physical disorders. We observed variation in the type of MHD as the number of physical comorbidities increased. The prevalence of anxiety, depression, and mental and behavioral disorders due to the misuse of alcohol and other psychoactive substances increased as physical comorbidities increased, but the prevalence of schizophrenia and dementia decreased with each additional physical disease. After adjusting for age and sex, this negative association changed the sign to a positive association in the case of dementia and mental and behavioral disorders due to psychoactive substance misuse. Conclusions The burden of physical-mental comorbidity in the Estonian population is relatively high. Further research is required to identify clusters of overlapping physical and mental disorders as well as the interactions between these conditions. Public health interventions may include structural changes to health care delivery, such as an increased emphasis on integrated care models that reduce barriers to mental health care.

Introducción: El personal de enfermería es uno de los grupos más afectados por la actual pandemia por COVID-19. Se han reportado problemas de salud mental en esta población, sin embargo, también es importante estudiar la resiliencia, para identificar sus fortalezas. El objetivo de este estudio fue estudiar la resiliencia en enfermeros mexicanos y los factores sociodemográficos y de salud mental asociados a ella. Se llevó a cabo un estudio transversal.Método: Se envió vía online un cuestionario sociodemográfico y relacionado a COVID-19, así como el Inventario de Resiliencia y el Cuestionario General de Salud-28. Se incluyeron 556 enfermeros, la mayoría fueron mujeres (80%), solteras (60.8%) y con edades entre 26-35 años (38.3%).Resultados: Se encontraron niveles bajos de resiliencia en los enfermeros más jóvenes (p<0.001, ɳ2=0.05), solteros (p<0.001, ɳ2=0.02) y con menor nivel educativo (p=0.001, ɳ2=0.02). Los predictores de resiliencia fueron la búsqueda de información sobre salud mental (β =-0.152, p <0.001), nivel educativo más alto (β = 0.142, p<0.001), niveles bajos de depresión (β=-0.307, p<0.001) y bajos niveles de disfunción social (β =-0.261, p<0.001).Conclusión: Estos hallazgos permiten identificar los factores asociados a la resiliencia en los enfermeros y cómo estos juegan un rol muy importante en su salud mental. Asimismo, estos datos permiten la identificación de grupos con mayor riesgo psicosocial, con la finalidad de guiar estrategias en salud mental orientadas a aumentar la resiliencia. Introduction: Nursing personnel are one of the groups which have been most affected by the current COVID-19 pandemic. Although mental health problems have been reported in this population, it is important to study resilience, in order to identify its strengths. The purpose was to study resilience in Mexican nurses and the mental health and sociodemographic factors associated with it. A cross-sectional study was used. Method: A sociodemographic and COVID-19 related questionnaire, the Resilience Inventory and the General Health Questionnaire-28, was sent via online. 556 nurses were included, the majority were women (80%), single (60.8%), aged between 26-35 years (38.3%). Results: Lower resilience was found among nurses who were younger (p<0.001, ɳ2=0.05), single (p<0.001, ɳ2=0.02) and with lower levels of education (p=0.001, ɳ2=0.02). Predictors of resilience included the search for mental health information (β =-0.152, p <0.001), higher education (β = 0.142, p<0.001), low levels of depression (β=-0.307, p<0.001) and low levels of social dysfunction (β =-0.261, p<0.001). Conclusion: This findings allowed to identify the factors which are associated with resilience among nurses and how this plays an important role in their mental. Likewise, this data allows for the identification of high psychosocial risk groups, to better guide mental health strategies aimed at increasing resilience. Problemas de saúde mental têm sido relatados nessa população, porém também é importante estudar a resiliência, para identificar seus pontos fortes. O objetivo deste estudo foi estudar a resiliência em enfermeiras mexicanas e os fatores sociodemográficos e de saúde mental a ela associados. Foi realizado um estudo transversal.Método:Um questionário sociodemográfico relacionado ao COVID-19 foi enviado online, assim como o Resilience Inventory e o General Health Questionnaire-28. Os preditores de resiliência foram a busca por informações sobre saúde mental (β = -0,152, p <0,001), maior escolaridade (β = 0,142, p <0,001), baixos níveis de depressão (β = -0,307, p <0,001 ) e baixos níveis de disfunção social (β = -0,261, p <0,001).Conclusão:Esses achados permitem identificar os fatores associados à resiliência em enfermeiros e como eles desempenham um papel muito importante em sua saúde mental. Da mesma forma, esses dados permitem identificar grupos de maior risco psicossocial, a fim de nortear estratégias de saúde mental que visem aumentar a resiliência.

Summary Background The 2019 coronavirus diseases (COVID-19) led out the mental health crisis. Aim To determine the psychological status and post-traumatic stress symptoms (PTSD) among general population (except confirmed and suspected cases, and close contacts) and their association with the coping strategy types during the COVID-19 outbreak. Design A cross-sectional study. Methods Participants were recruited from the community through snowball sampling with anonymous online questionnaires, using 28-item General Health Questionnaire, 22-item Impact of Events Scale-Revised and 28-item Brief Coping Inventory to measure their psychiatric disorders, PTSD level and coping strategies. Results Of the total 1109 participants, 42.65% and 67.09% self-reported psychiatric disorders and high PTSD level, respectively. Age, occupation and education level were significantly association with psychological status. The status of psychiatric disorders was also significantly related to high PTSD level. Using both emotion and problem coping was better for psychiatric status [adjusted odds ratio (aOR) = 0.72, 95% confidence interval (CI): 0.54–0.98], and problem-focused coping was significantly associated with high PTSD level (aOR = 2.09, 95% CI: 1.25–3.51). Conclusion Negative psychological outcomes were common among the general people during the COVID-19 outbreak, and the findings may provide references for intervention guidelines of mental health for the community population.

ObjectivesTo determine the relative extent that autism and intellectual disabilities are independently associated with poor mental and general health, in children and adults.DesignCross-sectional study. For Scotland’s population, logistic regressions investigated odds of intellectual disabilities and autism predicting mental health conditions, and poor general health, adjusted for age and gender.Participants1 548 819 children/youth aged 0-24 years, and 3 746 584 adults aged more than 25 years, of whom 9396/1 548 819 children/youth had intellectual disabilities (0.6%), 25 063/1 548 819 children/youth had autism (1.6%); and 16 953/3 746 584 adults had intellectual disabilities (0.5%), 6649/3 746 584 adults had autism (0.2%). These figures are based on self-report.Main outcome measuresSelf-reported general health status and mental health.ResultsIn children/youth, intellectual disabilities (OR 7.04, 95% CI 6.30 to 7.87) and autism (OR 25.08, 95% CI 23.08 to 27.32) both independently predicted mental health conditions. In adults, intellectual disabilities (OR 3.50, 95% CI 3.20 to 3.84) and autism (OR 5.30, 95% CI 4.80 to 5.85) both independently predicted mental health conditions. In children/youth, intellectual disabilities (OR 18.34, 95% CI 17.17 to 19.58) and autism (OR 8.40, 95% CI 8.02 to 8.80) both independently predicted poor general health. In adults, intellectual disabilities (OR 7.54, 95% CI 7.02 to 8.10) and autism (OR 4.46, 95% CI 4.06 to 4.89) both independently predicted poor general health.ConclusionsBoth intellectual disabilities and autism independently predict poor health, intellectual disabilities more so for general health and autism more so for mental health. Intellectual disabilities and autism are not uncommon, and due to their associated poor health, sufficient services/supports are needed. This is not just due to coexistence of these conditions or just to having intellectual disabilities, as the population with autism is independently associated with substantial health inequalities compared with the general population, across the entire life course.

AbstractObjectiveGreen tea has been widely acknowledged in Japan to induce a pleasurable mental feeling. Recent laboratory studies have suggested positive psychological effects as a result of consuming green tea. The present study examined whether green tea consumption in everyday life in Japan is associated with positive mental health.DesignA cross-sectional study was performed in February–March 2002.Setting and subjectsThe subjects of the study consisted of a general population of 600 Japanese aged 20–69 years. Responses of 380 subjects, obtained by home-visit interview, were analysed. The questionnaire inquired about consumption of brewed green tea and other beverages, perceived mental health status, lifestyle and others. The 12-item General Health Questionnaire (GHQ 12) was used for the assessment of mental ill-health (GHQ score ≥4).ResultsAfter adjustments for age, area, perceived mental stress, lifestyle and daily caffeine intake, the consumption of brewed green tea was not statistically associated with any decrease in risk of mental ill-health among either males or females (odds ratio (OR) = 0.78, 95% confidence interval (CI) = 0.47–1.29 for males; OR = 0.77, 95% CI = 0.51–1.14 for females). Daily caffeine intake (100 mg) inclusive of green tea, black tea, coffee and other caffeine-containing beverages was associated with a higher risk of mental ill-health among females (OR = 1.26, 95% CI = 1.01–1.56).ConclusionsThe results provide population-based evidence on the consumption of brewed green tea in everyday life and mental health, together with information on consumption patterns of various beverages and lifestyles.
