Abstract Background: It is challenging for the clinical management of malunited ankle fracture. The aim of this study is to evaluate the clinical outcome of realignment surgery for anterior translation of talus after malunited ankle fracture and to analyze its. Methods: A total of 11 patients with anterior translation of talus after malunited ankle fractures underwent corrective osteotomy in our institute were retrospectively reviewed. All patients were evaluated with radiological parameters and clinical functional scores. There were 4 patients with Weber type A fracture, 4 patients with Pilon type B fracture and 3 patients with Weber type C fracture. 8/11 patients had impaction on the anterior distal tibial plafond, intra-articular distal tibial osteotomy was performed in these patients, additional bony correction and soft tissue surgery were also performed to achieve congruent ankle joint. Results: The mean age at surgery was 32.8 ± 10.8 (range,16-48) years. The mean follow-up time was 50.5 ± 23.6 (range, 16-80) months. The mean AOFAS-AH score increased from 28.2 ± 19.1preoperatively to 72.5 ± 8.1 points postoperatively(p<0.05), the mean lateral talar station(LTS) improved from 9.2 ± 3.7 preoperatively to 1.5 ± 1.4 mm postoperatively(p<0.05). 10/11 patients had improvement or no worsening ankle osteoarthritis on sagittal plane, while 1 patient had advanced ankle osteoarthritis. Conclusion: A congruent ankle joint in sagittal plane could be achieved by corrective osteotomies with additional soft tissue procedures. The realignment surgery was a valuable treatment option for the salvage of anterior translation of talus after malunited ankle fracture.

Abstract The degradability of two commercially available pesticides was studied using heterogeneous photocatalytic and activated sludge treatment methods. The first pesticide contained 5% quizalofop-P-ethyl as an active ingredient and petroleum naphtha as a solvent, the latter causing difficulties both in photocatalytic and biological treatment methods. The active ingredient of the second compound was acetamiprid. The photocatalysis proved to be effective both under laboratory conditions (using UV light) and when exposed to sunlight, but the pesticides remained stable during the employed biological treatment. Preliminary information on its behaviour in soil was obtained from transport modelling.

Salah satu ancaman cedera yang kerap kali membayangi seseorang dengan aktivitas fisik yang tinggi adalah cedera sendi ankle, cedera  sendi ankle umumya merupakan gangguan pada bagian ligamen sendi yang diakibatkan oleh tarikan berlebih. Penelitian ini bertujuan untuk mengetahui efektivitas terapi masase terhadap nyeri gerak dan fungsi gerak sendi ankle pasca cedera ankle. Terapi masase yang digunakan dalam penelitian adalah soft tissue release dan deep tissue massage yang dilengkapi dengan reposisi gerak. Penelitian ini merupakan penelitian pre-experimental design dengan metode one-group pre-test – post-test design. Populasi sample penelitian ini adalah pasien Lab/Klinik Terapi Latihan FIK UNY yang selama tiga bulan (Februari – April 2019) diperkirakan sejumlah 100 orang. Teknik sampling data menggunakan insidental sampling dengan rumus Slovin (nilai kritis 20%) sehingga diperoleh subjek sebanyak 20 orang. Instrumen yang digunakan berupa catatan medis hasil dari anamnesa dan pemeriksaan yang dibuat dengan memodifikasi Lower Extremity Functional Scale (Binkley et al, 1999) yang telah di uji validitas menggunakan Pearson correlation dan reliabilitas dengan Cronbach’s Alpha. Nyeri gerak dan fungsi gerak sebelum dan sesudah perlakukan dianalisis dengan menggunakan uji beda dua kelompok berpasangan non- parametrik Wilcoxon Signed Ranks Test. Data pre dan post ini digunakan dalam uji efektivitas. Hasil penelitian menunjukan bahwa terapi masase yang meliputi pelemasan otot dengan soft tissue release dan deep tissue massage dan ditambah dengan reposisi gerak dapat mengurangi nyeri gerak dan meningkatkan fungsi gerak dari sendi ankle pasca cedera ankle (p < 0,05), dengan efektivitas penurunan nyeri gerak sebesar 70,31% dan peningkatan fungsi gerak sebesar 20,62%. Dapat disimpulkan bahwa perlakuan massage tersebut efektif dalam memperbaiki nyeri gerak dan fungsi ankle paska cedera. EFFECTIVENESS OF MASSAGE THERAPY ON MOTION PAIN AND FUNCTION OF ANKLE JOINTS AFTER ANKLE INJURYAbstractOne of the threat of injury that often overshadows someone with high physical activity is ankle joint injury, ankle joint injury is generally a disruption in the ligament of the joint caused by excessive traction. This study aims to determine the effectiveness of massage therapy for motion pain and ankle joint function after ankle injury. Massage therapy used in this research is soft tissue release and deep tissue massage which is equipped with motion repositioning. This research is a pre-experimental design research with one-group pre-test - post-test design method. The population of this study sample is the patients of the Lab / Clinical Training Clinic of FIK UNY which for an estimated three months (February - April 2019) amounted to 100 people. Data sampling technique used incidental sampling with the Slovin formula (critical value of 20%) to obtain as many as 20 subjects. The instrument used in the form of medical records from the history and examination made by modifying the Lower Extremity Functional Scale (Binkley et al, 1999) that has been tested for validity using Pearson correlation and reliability with Cronbach's Alpha. Motion pain and motion function before and after treatment were analyzed using a two-group non-parametric paired Wilcoxon Signed Ranks Test. This pre and post data is used in the effectiveness test. The results showed that massage therapy which includes muscle relaxation with soft tissue release and deep tissue massage and coupled with motion repositioning can reduce motion pain and improve motion function of the ankle joint after ankle injury (p <0.05), with the effectiveness of decreasing motion pain by 70.31% and an increase in motion function by 20.62%. It can be concluded that the massage treatment is effective in improving postoperative pain and ankle function.

ABSTRACT Background Malleolar fractures of ankle are usually complex injuries, as they are associated with significant ligament and soft tissue injury—injury to syndesmosis and injury to medial and lateral collateral ligaments. The open reduction and internal fixation is not feasible until recovery of significant soft tissue injury and subsidence of edema. Malleolar fractures are articular fractures and have associated subluxation and dislocation of talus. The aims of treatment are to restore normal anatomy and provide sufficient stability for early movements. Malleolar fractures more often require open reduction. Our study aimed to know efficacy and outcome of operative management of them. Materials and methods From January 2013 to March 2015, 35 patients with syndesmotic ankle injury and trimalleolar ankle fractures admitted to the Government Medical College, Latur, India, were operated and followed up prospectively. Results Mean age of patients is 35 years (25–60 years). Fracture union was seen radiologically in 3 to 4 months depending on fracture geometry. We achieved good to excellent results of 90%. Conclusion We conclude that malleolar fractures encountered in clinical practice need thorough assessment and meticulous surgical intervention, as they are associated with injury to ligament complex, i.e., ligament is a key structure in the stability of ankle mortise. Abduction and external rotation types of injuries are the most common types to be seen. We achieved stable fixation and performed early mobilization of the ankle joint, which limits the complications of mainly ankle stiffness. Each malleolus has got its inherent associated complications and calls for special attention for identifying associated conditions, such as syndesmotic injury, talus dislocation in posterior malleolar fractures, irreducible ankle dislocation with trimalleolar fracture, and entrapped fibula behind tibia with irreducible dislocation. Gas-to-liquid (GTL) technology involves the conversion of natural gas into several liquid hydrocarbon products. The Fischer–Tropsch (F–T) process is the most widely applied approach for GTL, and it is the main source of wastewater in the GTL process. The wastewater is generally characterized by high chemical oxygen demand (COD) and total organic carbon (TOC) content due to the presence of alcohol, ketones and organic acids. The discharge of this highly contaminated wastewater without prior treatment can cause adverse effects on human life and aquatic systems. This review examines aerobic and anaerobic biological treatment methods that have been shown to reduce the concentration of COD and organic compounds in wastewater. Advanced biological treatment methods, such as cell immobilization and application of nanotechnology are also evaluated. The removal of alcohol and volatile fatty acids (VFA) from GTL wastewater can be achieved successfully under anaerobic conditions. However, the combination of anaerobic systems with aerobic biodegradation processes or chemical treatment processes can be a viable technology for the treatment of highly contaminated GTL wastewater with high COD concentration. The ultimate goal is to have treated wastewater that has good enough quality to be reused in the GTL process, which could lead to cost reduction and environmental benefits.

AbstractSeptic nonunion of the pilon region with ankle joint infection is challenging for orthopedic surgeons to treat and is associated with a high risk of limb loss. Therefore, the aim of this study was to evaluate the effectiveness of bone transport for ankle arthrodesis in salvaging the limp after septic ankle destruction of the pilon region. We conducted a single-center, retrospective study including 21 patients treated for septic pilon nonunion with accompanying septic ankle destruction via Ilizarov bone transport between 2004 and 2018. In all cases, the complete excision of the nonunion and the resection of the ankle joint were carried out, followed by treating the bone and joint defect with a bone transport into the ankle arthrodesis. In 12/21 patients an additional flap transfer was required due to an accompanying soft tissue lesion. The overall healing and failure rate, final alignment and complications were recorded by the patients’ medical files. The bone-related and functional results were evaluated according to the Association for the Study and Application of Methods of Ilizarov (ASAMI) scoring system and a modified American Orthopedic Foot and Ankle Society (AOFAS) scale. After a mean follow-up of 30.9 ± 15.7 months (range 12–63 months), complete bone and soft tissue healing occurred in 18/21 patients (85.7%). The patients had excellent (5), good (7), fair (4), and poor (3) results based on the ASAMI functional score. Regarding bone stock, 6 patients had excellent, 7 good, and 6 fair results. The modified AOFAS score reached 60.6 ± 18 points (range, 29–86). In total, 33 minor complications and 28 major complications occurred during the study period. In 2 cases, a proximal lower leg amputation was performed due to a persistent infection and free flap necrosis with a large soft tissue defect, whereas in one case, persistent nonunion on the docking side was treated with a carbon orthosis because the patient refused to undergo an additional surgery. Bone transport for ankle arthrodesis offers the possibility of limb salvage after septic ankle destruction of the pilon region, with acceptable bony and functional results. However, a high number of complications and surgical revisions are associated with the treatment of this severe complication after pilon fracture.
