Frequency of Gram-negative bacterial pathogens in bloodstream infections and their resistance to antibiotics in the Czech Republic

Staphylococcus aureus is one of the major causes of bloodstream infections. The aim of our study was to characterize methicillin-resistant Staphylococcus aureus (MRSA) isolates from blood of patients hospitalized in the Czech Republic between 2016 and 2018. All MRSA strains were tested for antibiotic susceptibility, analyzed by spa typing and clustered using a Based Upon Repeat Pattern (BURP) algorithm. The representative isolates of the four most common spa types and representative isolates of all spa clonal complexes were further typed by multilocus sequence typing (MLST) and staphylococcal cassette chromosome mec (SCCmec) typing. The majority of MRSA strains were resistant to ciprofloxacin (94%), erythromycin (95.5%) and clindamycin (95.6%). Among the 618 strains analyzed, 52 different spa types were detected. BURP analysis divided them into six different clusters. The most common spa types were t003, t586, t014 and t002, all belonging to the CC5 (clonal complex). CC5 was the most abundant MLST CC of our study, comprising of 91.7% (n = 565) of spa-typeable isolates. Other CCs present in our study were CC398, CC22, CC8, CC45 and CC97. To our knowledge, this is the biggest nationwide study aimed at typing MRSA blood isolates from the Czech Republic.

Population and evolutionary analyses of pathogenic bacteria are frequently hindered by sampling strategies that concentrate on isolates from patients with invasive disease. This is especially so for the gram-negative diplococcus Neisseria meningitidis, a cause of septicemia and meningitis worldwide. Meningococcal isolate collections almost exclusively comprise organisms originating from patients with invasive meningococcal disease, although this bacterium is a commensal inhabitant of the human nasopharynx and very rarely causes pathological effects. In the present study, molecular biology-based techniques were used to establish the genetic relationships of 156 meningococci isolated from healthy young adults in the Czech Republic during 1993. None of the individuals sampled had known links to patients with invasive disease. Multilocus sequence typing (MLST) showed that the bacterial population was highly diverse, comprising 71 different sequence types (STs) which were assigned to 34 distinct complexes or lineages. Three previously identified hyperinvasive lineages were present: 26 isolates (17%) belonged to the ST-41 complex (lineage 3); 4 (2.6%) belonged to the ST-11 (electrophoretic type [ET-37]) complex, and 1 (0.6%) belonged to the ST-32 (ET-5) complex. The data were consistent with the view that most nucleotide sequence diversity resulted from the reassortment of alleles by horizontal genetic exchange.

Salmonella prevalence was assessed in 816 pigs from fifteen herds which were slaughtered in ten slaughterhouses from June 2001 to December 2002. No Salmonellae were isolated in pigs from eight herds in four slaughterhouses. Salmonella prevalence in pigs originating from the other seven herds ranged from 2.0% to 12.0%. The most frequent site of Salmonella isolation was caecum (2.45%). This finding is statistically significant (P &lt; 0.01) as compared to those obtained with mesenteric lymph nodes (0.73%) and carcass swabs (0.12%). Salmonellae were not found in samples from the environments (n = 197). A total of 27 Salmonella isolates were classified into serotypes S. infantis (n = 8), S. typhimurium (n = 5), S. agona (n = 4), S. kaapstad (n = 4), S. derby (n = 3), S. bredeney (n = 2), and S. london (n = 1). All five S. typhimurium DT 104 were resistant to the phenotype ACSSuT. Resistance genes bla<sub>PSE-1</sub>, floR, aadA2, sul1, and tetG were identified in all pentaresistant strains. One strain of S. derby was resistant to gentamicin, streptomycin and sulphonamides. The other Salmonella isolates were sensitive to all antibiotics tested. &nbsp;
