Severe carotid stenosis or occlusion may cause insufficient blood flow and lead to distal artery wall collapse and extensive lumen contraction. Whether this ‘adaptive narrowing' can restitute after carotid recanalization is unclear. We report a patient with global ischemia due to occlusions of bilateral carotid and right vertebral arteries. The occluded left carotid was recanalized successfully with angioplasty and stenting. The adaptively narrowed distal carotid did not restitute immediately but regained its morphology 1 week after the procedure. Carotid adaptive narrow distal occlusion or stenosis may not regain its original morphology immediately but several days after recanalization. This knowledge is instructive for treating occlusive carotid diseases.

Human papillomavirus (HPV) has been directly related to acuminate warts and cervical cancer, the second most common neoplasia among women. Given the lack of treatment against the virus itself, many medications have been utilised, mainly aiming in modifying the host’s immunological response. We present the case of a 54 years old postmenopausal patient with a history of vaginal cuff wart and HPV persistence that we managed in our clinic for 6 months with a mix of curcumin, aloe vera, amla and other natural ingredients. As the patient was found to be intolerant to imiquimod (one of the most common conservative methods of treatment) we attempted the use of curcumin, which was applied to the area of the wart three times per week for 6 months. Both clinical and colposcopical improvement was noted in regular clinic visits with regression of the lesion. The outcome of this case encourages our view that curcumin should be considered as a significant treatment modality against HPV infection and acuminate warts.

Lutembacher syndrome is a rare combination of congenital Atrial Septal Defect (ASD) and acquired Rheumatic Mitral Stenosis (MS).It is usually treated by surgical repair with potential risk of cardiopulmonary bypass. With the advancement of interventional methods of treatment it is amenable to nonsurgical transcatheter management. We are reporting a case of Lutembacher syndrome that was successfully treated with percutaneouos Inoue balloon mitral valvuloplasty and device closure of atrial septal defect. Studies are going worldwide to develop an effective method for treatment against the global pandemic of COVID-19 but unfortunately no treatment method has been declared as a specific therapeutic approach against COVID-19. History stands as evidence where homeopathy intervention has been beneficial in both preventive and curative aspects of various epidemics. Holistic symptomatological aspect based on generalities enables Homeopathy to be effective in any disease condition. Selection of homeopathic remedy is always on basis of the totality of symptoms and in most cases the changes occurring in cellular and molecular level are not required except in few occasions of dose and potency determination. This concept of healing through homoeopathy is different from other conventional methods of treatment where molecular pathogenesis is the fulcrum of the drug protocols. Hence, in the era of fight to discover vaccine against COVID-19, the following case report shows how a COVID-19 positive individual with mild symptomatology of vomiting, fever, malaise, heaviness of head and loss of taste, recovers with prescription of Bryonia alba 200, followed by Tuberculinum 200, 1M and 10M and at the same time RT-PCR for SARS-CoV-2 becomes negative in the duration of two weeks. Therefore, this case report demonstrates why Homoeopathic intervention should be emphasized in the management of this international crisis. The severe course of chronic pancreatitis (CP) in children is rare. More often it is caused by hereditary factors and abnormalities of the bile and pancreatic ducts. Cases of the development of pancreatitis against the background of systemic inflammatory diseases are described. Therapy of a systemic disease can aggravate the course of pancreatitis, and the general serious condition of the patient does not allow for the necessary surgical treatment. The article describes the clinical case report of a complicated course of CP against the background of juvenile idiopathic arthritis in a 17-year-old girl. The effectiveness of intraluminal endoscopic methods of treatment (wirsungotomy and stenting of the main pancreatic duct) for relieving exacerbation and complications of CP has been demonstrated.

Congenital pseudarthrosis of the ulna is an extremely rare condition for which current surgical techniques have been unsatisfactory in restoring a normal two-bone forearm. We report a case which was treated by excision of the ulnar pseudarthrosis and interposition of a free vascularized fibular graft with a skin island to monitor anastomotic patency. Forearm rotation has been restored and the potential for normal forearm growth has been preserved.

Head and neck cancer accounts for 3.5% of annual worldwide cancer diagnoses with the larynx being part of it. The main methods of treatment include laryngectomy, but mainly a combination of radiation with chemotherapy and then combinations of chemotherapy with immunotherapy in more advanced stages of the disease. Histologically it is mostly squamous cell carcinoma, but cases of glandular differentiation have been reported that favor the entry of these patients into clinical trials. In our case we discuss a patient diagnosed with adenocarcinoma to the larynx area treated initially with laryngectomy and radiotherapy. In metastatic setting, he received multiple treatment lines with chemotherapy and immunotherapy and a later biopsy from a paraspinal mass which pathology resulted in an adenocarcinoma from the larynx, with hepatocellular differentiation. We performed upper endoscopy which revealed a high-grade dysplasia and the initiation of irinotecan led to a rapid clinical deterioration.

Vertical patellar dislocations (VPDs) are a rare event, and even more so among pediatric female patients. There have been less than 30 vertical patellar dislocations reported in the literature since the first in 1844. In this type of dislocation, the patella rotates about its vertical axis with the articular surface facing either medially or laterally. The mechanism of injury for a VPD can be broadly divided into two themes: a twisting injury or direct impact to the medial or lateral edge of the patella. We present a 10-year-old girl with a VPD after experiencing a twisting injury when descending a playground slide. The purpose of this study is to present a case report and review of the literature on vertical patella dislocations, including mechanisms of injury and suggested methods of treatment. We aim to provide a comprehensive understanding of the various categories of patella dislocations to alleviate confusion when classifying patellar dislocations. Furthermore, we provide clear suggestions for reduction methods and techniques with regards to vertical patellar dislocations, including a suggested protocol for an irreducible patella.

Relevance. According to the latest data the distal occlusion of the dentition is considered first in the structure of dental anomalies on the sagittal plane reaching 40-45% of the total number of deformations of the dental system.Purpose. Study of information about management and methods of treatment and intended indications for their use in patients with distal occlusion in different periods of bite formation.Materials and methods. We studied the publications of recent years by both Russian and foreign authors which were devoted to the choice of management and methods of treatment of growing patients with distal occlusion.Results. The variability in the manifestations of distal occlusion indicates the urgency of improving the management and methods of orthodontic treatment in patients with this pathology. In this case the determining factors are the severity of the pathology and the age of the patient. It is considered that the potential growth of the dento-maxillofacial area of the patient should be determined according to its individual characteristics. They are based on the stage of formation of the cervical vertebrae. It is noted that treatment during the second dentition has significant effects which are associated with the possibility of modifying the growth of the dental complex. This allows you to achieve a stable result in the long term.Conclusions. Different opinions of specialists determine the relevance of further study of methods of orthodontic treatment and analysis of the efficiency of their application, the necessity to improve the tactics of comprehensive rehabilitation of patients with distal occlusion during the formation of the bite taking into account the direction of the type of growth of the facial skeleton in growing patients.
