Rheumatoid arthritis (RA) and periodontitis are common chronic inflammatory diseases and periodontitis is known to be more common and more severe in patients with RA. Based on a paucity of studies about the relationship between common conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) and periodontitis, this prospective study aimed to evaluate the adjunctive effect of csDMARDs on response to nonsurgical periodontal treatment in patients with RA. Thirty-two patients with RA (RA group) and 32 systemically healthy patients (control group) with periodontitis were included in this study. The RA group patients were treated with csDMARDs, such as methotrexate, hydroxychloroquine, and sulfasalazine. Conventional nonsurgical periodontal treatment with scaling and root planing was performed in both groups. The extent and severity of periodontitis were evaluated by plaque index (PI), gingival index (GI), probing depth (PD), clinical attachment level (CAL), and bleeding on probing (BOP) at baseline and 4 weeks after periodontal treatment. There was no statistically significant difference of periodontal parameters between the RA and control groups at baseline. Four weeks after scaling and root planing, PD reduction, and CAL gain were higher in the RA group treated with csDMARDs compared to the control group, and the difference was statistically significant (P = 0.006 and 0.003, respectively). A post hoc analysis of the RA group showed no statistically significant difference on the response to nonsurgical periodontal treatment in multiple csDMARDs therapy and addition of NSAIDs and/or steroids to csDMARDs. In patients with RA, csDMARDs showed beneficial effect on periodontal clinical parameters following the nonsurgical periodontal treatment.

Objectives: Photodynamic therapy applies to the elimination of target cells by reactive oxygen species using a photosensitizing agent and a sufcient wavelength of light. It is a new, non-invasive approach to preventing infection. It is now an upcoming alternative treatment modality for periodontitis patients as a supplement to traditional scaling and root planing. The objective of the present study was to assess and compare the effect of photodynamic therapy with conventional Scaling and Root planing in patients with chronic periodontitis. Methods: The study included a minimum of 5 non-smoking males with mild to moderate periodontitis. Clinical parameters such as plaque index, gingival index, gingival bleeding index, pocket probing depth, clinical attachment level; microbiological parameter – anaerobic bacterial CFU count in subgingival plaque samples and biochemical parameter – GCF SOD levels were evaluated at baseline and 28 days postoperatively. The obtained data were analyzed using SPSS (SYSTAT 12) statistical software. The Wilcoxon signed Rank test was used to compare the mean values of various parameters before and after treatment. Results: The results of the study show that all clinical parameters improved from baseline to day 28. Following therapy, the mean levels of the anaerobic bacterial CFU and GCF SOD have decreased dramatically. Conclusion: The results of this study suggest a signicant improvement in clinical, microbiological and Biochemical parameters with the use of PDT as an adjunct to conventional SRP in chronic periodontitis patients

Objectives: The objectives of the study were to evaluate changes in 25(OH) Vitamin D levels and some biochemical parameters in rheumatoid arthritis (RA) patients compared with healthy controls and assess the correlation of 25-hydroxy Vitamin D, calcium, magnesium, and disease activity. Study the effects of anti-RA drugs on these biochemical parameters and also the role of supplements calcium and 25-OH Vitamin D in RA patients. Methods: This study conducted between 60 patients for RA and 20 healthy controls according to the American College of Rheumatology standards in 2010. In this study, 25-hydroxy Vitamin D was measured using an enzyme-linked immunosorbent assay, and also some biochemical parameters were measured with a spectrophotometer (Humalyzer 2000). Results: Serum 25(OH) Vitamin D, calcium, magnesium, and albumin levels were significantly lower in RA patients compared with healthy controls. Serum alanine aminotransferase aspartate aminotransferase levels were significantly increased in RA patients compared with healthy controls. The correlation was non-significantly among 25-hydroxy Vitamin D and clinical disease activity index (CDAI), while the results showed significantly inverse correlation calcium and magnesium concentrations with CDAI. Conclusion: 25-OH Vitamin D, calcium, albumin, and magnesium deficiency appear to be widespread in patients with RA. Thus, biochemical changes in RA are reflected in the pathogenesis of RA. Furthermore, in these results, there is no relationship between Vitamin D and the disease activity, while there is a relationship between calcium and magnesium with disease activity.
