Obesity and Health Care: indicators, measures and tools in the context of Rio de Janeiro, Brazil

This research aimed to describe the frequency of parasites in stool samples in the environment of Ilha da Marambaia, Rio de Janeiro, Brazil. One hundred and five stool samples were collected and processed by the coproparasitological techniques ethyl acetate sedimentation and centrifuge-flotation using saturated sugar solution. Parasites were detected in 81.9% of the samples, hookworm being the most prevalent, followed by Trichuris vulpis. Ascaris sp. eggs were also found. A high level of evolutive forms of parasites with public health risk was found in stool samples of the environment studied. We propose that health education programs, allied to an improvement of human and animal health care, must be employed to reduce the environmental contamination.

To reduce inequity in maternal and child health care indicators among socio-economically different regions, strategic location-specific policies should be designed. In this research work, a knowledge-discovery-based interactive decision support system has been developed on a web platform which would assist health care policymakers to design evidence-based decisions. Two modules have been prepared under this system to find out key influential Maternal and Child Healthcare (MCH) interventions for socio-economically different regions which had high impact on health care indicators. Data of 284 districts of nine high-focus states of India have been provided into the system to find out the efficiency of the system. Those data have been collected from district- level household survey part three (DLHS-3). The first module of the system has segregated all 284 districts into three segments based on their educational, social and economic conditions, and the second module has found out key influential health care interventions for all three segments separately which had high impact on health care indicators. It has been observed that adolescent health care intervention like female sterilization and childhood health care interventions such as DPT (diphtheria, pertussis, and tetanus) vaccine and measles vaccine were key influential health care interventions. The improvement of coverage of these interventions would help to reduce inequity and improve health care indicators of regions. Further research should be done to understand how the coverage of these interventions can be improved, especially in socio-economically poor regions.

Abstract Taking into consideration issues such as stigma and the mental health gap, this study explores narratives of anxious and depressed women treated in a community-based primary care service in a Rio de Janeiro favela about their suffering and care. We analysed 13 in-depth interviews using questions from Kadam's study. Framework analysis studied Access, Gateway, Trust, Psychosocial Issues, and Primary Mental Health Care, as key-concepts. Vulnerability and accessibility were the theoretical references. Thematic analysis found “suffering category”, highlighting family and community problems, and “help seeking category”, indicating how these women have coped with their emotional problems and addressed their needs through health services, community resources and self-help. Women's language patterns indicated links between implicit social rules and constraints to talk about suffering, especially if related to local violence. High medical turnover and overload are barriers for establishing a positive relationship with family physicians and continuity of care is a facilitator that promotes trust, security and adherence. Concluding, to plan community-based primary mental health care of this population, cultural and social factors must be comprehended as well as the work health teams conditions.
