Gastroesophageal reflux disease (GERD) is the most common of all acid-related diseases, it is recognized as the leading cause of esophageal adenocarcinoma. The natural factor of protection against aggressive refluxate components is the integrity of the esophageal mucosa, which performs a barrier function with the participation of a number of mechanical, chemical and immunological mechanisms. Their damage under the regular influence of acidic or mixed reflux causes the development of the pathological process. The review was prepared to systematize knowledge of the main components of mucosal barrier of the esophagus providing resistance of mucosa under conditions of GERD. The literature was searched in Embase, PubMed, and Google Scholar using the keywords: gastroesophageal reflux disease, mucosal protection, esophageal mucosa epithelium, dense contact proteins, epithelial protection, esophagoprotection. The main structural and functional components of esophageal mucosal protection were emphasized

Dexlansoprazole delayed release is a newly developed class of Proton Pump Inhibitor (PPI). The drug is effective in the treatment of erosive esophagitis, maintenance of eosinophilic esophagitis and relief of heartburn. Additionally, it is also beneficial in the treatment of heartburn with symptomatic gastroesophageal reflux disease. The drug offers the advantage of no dosage adjustment in patients with hepatic impairment. Dexlansoprazole inhibits the stomach acid secrection by specific inhibition of the hydrogen-potassium–ATPase in parietal cells of stomach.

Aim. A generalized analysis of changes in functional activity of macrophages on the basis of phagocytic activity, cytokine profile, changes in the level of expression of surface markers characteristic of pro - or anti-inflammatory phenotype of the cells when exposed to reluctate. Materials and methods. Developed in vitro model of co-peritoneal macrophages of mice With57/BL6 (n=65) and reluctate patients with gastroesophageal reflux disease (GERD; n=65) having different pH values (three group comparison). Took into account the standard criteria phagocytic ability (absorption Staphylococcus aureus 9198, light microscopy), secretory activity (cytokine profile Th1/Th2, flow cytometry) and receptor characterization of macrophages (expression of CD25/80/163/206, flow cytometry). Results. The phagocytic activity of macrophages, calculated on the basis of the average number of bacteria ingested by one phagocyte, is not associated with the pH value of the added reluctate. It is established that the alkalinisation of reluctate leads to significant alteration in the expression of CD receptors - decrease M1 and increase M2. The index of total production of Th1/Тһ2 in groups progressively decreased with increasing pH of reluctate and amounted to 3.6 units in the group pH from 4.6 to 6.6; 2.8 units group a pH of 6.7-7.2 and 1.6 units in the group pH of 7.3 to 8.1, due to increased production of Th2 cytokines at offset reluctate pH to slightly alkaline side. The data obtained indicate the increase of expression and secretion of anti-inflammatory markers at an alkaline pH shift of reluctate. Analysis of the studied characteristics of the activity profile of macrophages in the proposed in vitro model justifies the need for considering the peculiarities of the functional activity of macrophages under the influence of reluctate different nature. The special importance of studying the cytokine profile and characteristics of the functional activity of macrophages in patients with GERD, given the nature of reluctate.

BACKGROUND: It is well known the association between gastroesophageal reflux disease and asthma. The hyperreactivity of the airways is a characteristic of an asthmatic. Many studies associate the increase of the airways reactivity with gastroesophageal reflux disease. AIM: In this study we have evaluated the effect of the intraluminal exposition to gastric juice of trachea on the reactivity to methacholine from rats submitted to a pulmonary allergic inflammation. METHODS: Group of rats were sensitized and challenged with ovalbumin. After 24 hours the animals were sacrificed, and their tracheae were removed to be cultured with gastric juice. The gastric juice was obtained from a donor rat. Subsequently the segments were placed into plastic plates with RPMI-1640 for incubation, under suitable atmosphere and time. After the period of incubation the segments were put into chambers for the analysis of the contractile response to methacholine. RESULTS: We observed reduction in the contractile response of trachea cultured with gastric juice from allergic rats. This result was confirmed by the pharmacological treatments with compound 48/80 and dissodium cromoglicate (mast cells blockade), L-NAME (nitric oxide inhibitor, NO), capsaicin (neuropeptides depletion) and indomethacin (ciclooxigenase inhibitor). CONCLUSIONS: Our results highlight to the existence of a complex interaction between pulmonary allergy and gastric juice in the airways. The involvement of the non-adrenergic non-cholinergic system, NO, prostanoids and mast cells are directly related to this interaction. We suggest that the reduced contractile response observed in vitro may represent a protector mechanism of the airways. Despite its presence in the human body it can not be observed due to the predominant effects of excitatory the non-adrenergic non-cholinergic system.

Barrett’s esophagus (BE) is a premalignant condition caused by gastroesophageal reflux disease (GERD), where physiological squamous epithelium is replaced by columnar epithelium. Several in vivo and in vitro BE models were developed with questionable translational relevance when implemented separately. Therefore, we aimed to screen Gene Expression Omnibus 2R (GEO2R) databases to establish whether clinical BE molecular profile was comparable with animal and optimized human esophageal squamous cell lines-based in vitro models. The GEO2R tool and selected databases were used to establish human BE molecular profile. BE-specific mRNAs in human esophageal cell lines (Het-1A and EPC2) were determined after one, three and/or six-day treatment with acidified medium (pH 5.0) and/or 50 and 100 µM bile mixture (BM). Wistar rats underwent microsurgical procedures to generate esophagogastroduodenal anastomosis (EGDA) leading to BE. BE-specific genes (keratin (KRT)1, KRT4, KRT5, KRT6A, KRT13, KRT14, KRT15, KRT16, KRT23, KRT24, KRT7, KRT8, KRT18, KRT20, trefoil factor (TFF)1, TFF2, TFF3, villin (VIL)1, mucin (MUC)2, MUC3A/B, MUC5B, MUC6 and MUC13) mRNA expression was assessed by real-time PCR. Pro/anti-inflammatory factors (interleukin (IL)-1β, IL-2, IL-4, IL-5, IL-6, IL-10, IL-12, IL-13, tumor necrosis factor α, interferon γ, granulocyte-macrophage colony-stimulating factor) serum concentration was assessed by a Luminex assay. Expression profile in vivo reflected about 45% of clinical BE with accompanied inflammatory response. Six-day treatment with 100 µM BM (pH 5.0) altered gene expression in vitro reflecting in 73% human BE profile and making this the most reliable in vitro tool taking into account two tested cell lines. Our optimized and established combined in vitro and in vivo BE models can improve further physiological and pharmacological studies testing pathomechanisms and novel therapeutic targets of this disorder.

Pendahuluan: Gastroesophageal reflux disease (GERD) didefinisikan sebagai reflex otot esophagus atau spincter, yang memungkinkan asam lambung bergerak naik melalui kerongkongan hingga masuk kedalam rongga mulut. Asam lambung memiliki pH berkisar 1 hingga 1.5, berada di bawah pH kritis email sebesar 5.5 yang dapat menyebabkan terjadinya demineralisasi email, dentin, dan sementum. Tujuan penelitian adalah untuk membedakan nilai kekasaran permukaan semen glass ionomer (GIC) dan modifikasi resin semen glass ionomer (RMGIC) akibat efek cairan lambung buatan.  Metode: Penelitian yang dilakukan merupakan penelitian eksperimental murni secara in vitro. Sampel dipersiapkan sebanyak 40 buah, yang terdiri dari 20 sampel semen glass ionomer (GIC) dan 20 sampel modifikasi resin semen glass ionomer (RMGIC). Seluruh sampel dibagi ke dalam 4 kelompok, masing-masing terdiri dari 10 sampel yaitu (1) kelompok GIC direndam dalam saliva buatan, (2) kelompok RMGIC direndam dalam saliva buatan, (3) kelompok GIC direndam dalam cairan lambung buatan selama 3 kali 7 menit dan setelahnya direndam kembali dalam saliva buatan, dan (4) kelompok RMGIC direndam dalam cairan lambung buatan selama 3 kali 7 menit dan setelahnya direndam kembali dalam saliva buatan. Perendaman dilakukan selama 9 hari. Nilai kekasaran diukur dengan menggunakan surface roughness tester (Profilometri). Hasil dianalisis secara statistik dengan menggunakan uji ANAVA dan analisis post-hoc dengan menggunakan t-test. Hasil: Terdapat perbedaan nilai kekasaran permukaan semen glass ionomer (GIC) dan modifikasi resin semen glass ionomer (RMGIC) akibat efek cairan lambung buatan. Simpulan: Paparan cairan asam lambung pada pasien GERD dapat mempengaruhi kekasaran permukaan bahan restorasi GIC dan RMGIC. The gastric acid has pH between 1 and 1.5, far below the critical pH of 5.5 at which tooth enamel will dissolve. Gastric juice has been shown to demineralize enamel, dentin, and root cementum. Methods: Fourthy samples of each restorative material, a conventional glass ionomer cement (20 samples) and a resin modified glass ionomer cement (20 samples), were prepared and divided into four groups, each group consist of 10 samples. Group (1) group of GIC immersed in simulated saliva and group (2) group of RMGIC immersed in simulated saliva, both control groups immersed for 9 days. Group (3) were group GIC and group (4) were RMGIC, both groups immersed in simulated saliva for 9 days and in between both groups immersed in gastric juice every 3 times a day for 7 minute. Each group subjected to profilometric analysis. The profilometric values were statistically analyzed using ANOVA and 2-way analysis of variance (post-hoc). Results: There are differences between GIC and RMGIC after immersion in gastric juice.
