An overview of the current state of outcome measurement after hand surgery is presented. The paper focuses on the development, application and strengths and weaknesses of subjective assessment techniques. It also reviews the existing questionnaires and suggests recommendations for use in research or clinical practice.

Several key issues impact the clinical practice of stroke rehabilitation including a patient’s medical history, stroke experience, the potential for recovery, and the selection of the most effective type of therapy. Until clinicians have answers to these concerns, the treatment and rehabilitation are rather intuitive, with standard procedures carried out based on subjective estimations using clinical scales. Therefore, there is a need to find biomarkers that could predict brain recovery potential in stroke patients. This review aims to present the current state-of-the-art stroke recovery biomarkers that could be used in clinical practice. The revision of biochemical biomarkers has been developed based on stroke recovery processes: angiogenesis and neuroplasticity. This paper provides an overview of the biomarkers that are considered to be ready-to-use in clinical practice and others, considered as future tools. Furthermore, this review shows the utility of biomarkers in the development of the concept of personalized medicine. Enhancing brain neuroplasticity and rehabilitation facilitation are crucial concerns not only after stroke, but in all central nervous system diseases.

AbstractProgrammed death ligand-1 (PD-L1) expression is a key biomarker to screen patients for PD-1/PD-L1-targeted immunotherapy. However, a subjective assessment guide on PD-L1 expression of tumor-infiltrating immune cells (IC) scoring is currently adopted in clinical practice with low concordance. Therefore, a repeatable and quantifiable PD-L1 IC scoring method of breast cancer is desirable. In this study, we propose a deep learning-based artificial intelligence-assisted (AI-assisted) model for PD-L1 IC scoring. Three rounds of ring studies (RSs) involving 31 pathologists from 10 hospitals were carried out, using the current guideline in the first two rounds (RS1, RS2) and our AI scoring model in the last round (RS3). A total of 109 PD-L1 (Ventana SP142) immunohistochemistry (IHC) stained images were assessed and the role of the AI-assisted model was evaluated. With the assistance of AI, the scoring concordance across pathologists was boosted to excellent in RS3 (0.950, 95% confidence interval (CI): 0.936–0.962) from moderate in RS1 (0.674, 95% CI: 0.614–0.735) and RS2 (0.736, 95% CI: 0.683–0.789). The 2- and 4-category scoring accuracy were improved by 4.2% (0.959, 95% CI: 0.953–0.964) and 13% (0.815, 95% CI: 0.803–0.827) (p < 0.001). The AI results were generally accepted by pathologists with 61% “fully accepted” and 91% “almost accepted”. The proposed AI-assisted method can help pathologists at all levels to improve the PD-L1 assay (SP-142) IC assessment in breast cancer in terms of both accuracy and concordance. The AI tool provides a scheme to standardize the PD-L1 IC scoring in clinical practice.

AbstractWhile the benefits of routine outcome measurement have been extolled and to some degree researched, it is surprising that service user opinions on this common therapy practice have largely not been investigated. This study aimed to assess service users’ experiences of completing measures during psychological therapy, with a view to exploring how therapists can maximize how helpful measures are in therapy. Fifteen participants completed surveys about the use of measures in their current episode of care. Ten clinicians also completed a survey about their use of, and views about, measures. Results showed that despite mixed experiences in how measures were explained and used, service users showed generally favourable attitudes towards their use in therapy, with them being perceived as most helpful when well integrated into sessions by their therapists. Clinicians reported using a wide range of measures, and generally endorsed positive beliefs about measures more strongly than negative ones. Implications for clinical practice, service development, and further research are discussed.

Collagen materials are widely applied in medicine due to optimal handling characteristics, biocompatibility, controlled biodegradation, the ability to form complexes with drugs and facilitate regeneration. Researchers from Sechenov University developed, studied in experiments and introduced into medicine a variety of materials based on collagen — a protein of connective tissue. At the same time, new collagen materials were launched into clinical practice abroad. In this review of Russian and world literature, we described how scientific and applied studies of collagen materials developed over time and tried to illustrate the current state and trends of collagen application for a variety of medical purposes — from hemostatic sponges to tissue-engineered constructs. The range of available collagen-based medical products and the emergence of new collagen materials indicate the keen interest in this biomaterial from the medical community and the potential of future discoveries.

SummaryThe article is dedicated to the current state and prospects of cell technologies in the treatment of hip osteoarthritis. The material is based on the analysis of clinical trials on this topic submitted to international registry СlinicalTrials.gov. It has been founded 5 clinical trials relevant to the theme. Only one of these trials was completed with the results pub-lished. It has been stated that the use of cell technologies for the treatment of hip osteoarthritis is still in the stage of a pilot clinical experiment. Thus, it can not yet be the subject of any translation into common clinical practice. At the same time, there is a reason to expect in the upcoming years a serious progress in this direction. This is indicated by the fact that, as of March 1, 2020, the СlinicalTrials.govregistry lists 4 active clinical trials in which the usage of cell technologies in the treatment of hip osteoarthritis is tested. Some of these trials have been already recruiting patients or are ready to begin the recruitment.

Nutritional genomics is a fast-growing area of precision medicine. It is expected that dietitians will be key players in this area. This chapter discusses the current state of industry, from research to the availability of commercial products, their regulation, and consumer perceptions. Current issues and controversies surrounding the readiness, availability, and delivery of this technology to consumers are examined, and finally, an education framework describing core competencies is presented. The framework aims to provide dietitians with the necessary tools to evaluate and make decisions on the potential for translation of this technology to clinical practice for disease prevention and management.

This chapter analyzes the role played by technology in undergraduate medical education (UME) using two perspectives: how technology is used as a tool to facilitate teaching and how medical students are taught to use technology in the clinical setting. For each perspective, a survey of literature, published from 2009 to 2019, was conducted to understand the current state. Authors critically examine the current state and describe and analyze issues with it. Recommendations are made for improving the blending of medical education, technology, pedagogy, and clinical practice. The narrative in this chapter is at the intersection of digital technology, educational theories, and medical settings (educational and practice).

Background: Digital health is rapidly entering clinical practice in cardiology. Estonia is one of the leading nations in implementing digital nationwide solutions. Recently, the European Society of Cardiology organized the 1st Summit on Digital Health in Tallinn, which provided the opportunity to discuss various aspects concerning the digitalization of cardiology. Summary: The current review focuses on the advancements of Estonian digital health and digital cardiology as well as possible barriers and solutions for implementing digital innovations in cardiology. Key Messages: The authors have included aspects from the recent summit, personal communications, and literature reviews to express the current state and future possibilities of digital health in ­cardiology from the Estonian perspective.
