Excessive daytime sleepiness (EDS) is a significant public health concern, with obstructive sleep apnea (OSA) being a common cause, and a particular relationship exists with the severity of depression. A literature search on OSA, depression, and EDS was performed in PubMed. The chosen evidence was limited to human studies. Available evidence was systematically reviewed to ascertain the association of EDS with depression and OSA according to the general population and some specific population subgroups. In addition, effectiveness of continuous positive airway pressure (CPAP) was analyzed as a standard therapy for improving EDS and depression in patients with OSA. In the general population, patients with OSA, and some other subpopulations, the review contributed to: (1) delineating the prevalence of EDS; (2) substantiating the relationship of EDS and depression; (3) presenting the relationship between EDS and OSA; and (4) revealing that the duration of CPAP is crucial for its therapeutic effects in improving EDS and depressive symptoms in patients with OSA.

Introduction Approximately 80% of patients with chronic kidney disease complain about sleep disorders, which is a much higher percentage than in the general population. Excessive daytime sleepiness is the third most frequent complaint in these patients, and it is significantly associated with a higher risk of sleep apnea. The aim of this study was to assess the presence of daytime sleepiness in patients with end stage renal disease (ESRD) who were undergoing diurnal hemodialysis. Materials and methods The Epworth Sleepiness Scale (ESS) was applied to patients with ESRD who underwent diurnal hemodialysis in the Centro de Nefrologia da Zona Norte in Sao Paulo, Brazil. Results A total of 168 patients were included in the study. According to the ESS, 31% presented with a mild propensity to sleep, 22% with a moderate propensity, and 17% with a severe propensity. Conclusion Our study concluded that 70% of the sample of patients with ESRD who were undergoing hemodialysis presented with a propensity to sleep in inappropriate circumstances on the ESS. When excessive daytime sleepiness is associated with sleep apnea and other comorbidities, it is essential to carefully assess patients’ complaints.

SynopsisThis paper is part of a series dealing with the role of life events in the onset of depressive disorders. Women who developed depression in a general population sample in Islington in North London are contrasted with a National Health Service-treated series of depressed patients in the same area. Findings among the latter confirm the importance of a severely threatening provoking event for onset among the majority of depressed women patients. The results for the two series are similar except for a small subgroup of patients characterized by a melancholic/psychotic condition with a prior episode.The severe events of importance have been recognized for some time by the traditional ratings of the Life Events and Difficulty Schedule (LEDS). However, the full descriptive material collected by the LEDS has been used to develop a new refined measure reflecting the likelihood of feelings of humiliation and being trapped following a severely threatening event, in addition to existing measures of loss or danger. The experience of humiliation and entrapment was important in provoking depression in both the patient and non-patient series. It proved to be associated with a far greater risk of depression than the experience of loss or danger without humiliation or entrapment.

Background: Obstructive sleep apnea (OSA) is a condition resulting in disrupted sleep and excessive daytime sleepiness/somnolence (EDS) with demonstrable deficits in cognitive and psychomotor performance. Indian studies have shown the prevalence of 4.9–19.7% in males and 2.1–7.4% in females. Prevalence among military aviators is unknown. Materials and Methods: EDS was determined by employing Epworth Sleepiness Scale (ESS) among the 54 male aircrews in an airbase. The risk for OSA was also determined among these individuals using STOP-BANG questionnaire. Results and Discussion: It was found that none of the aircrews in the study group is found to have dangerously sleepy (DS). About 11.11% of the studied population were found to have EDS which is less than the prevalence rate in India as per a study conducted by the National Institute of Mental Health and Neuro Sciences on general population and the Western world (prevalence rate of 16.7%) as found by Gislason et al. OSA risk among the study population was (n = 9/54) 16.16%. The prevalence of OSA in India male is reported as 4.9–17.9%. Thus, percentage of aircrew showing the risk for OSA in this cross-sectional study is comparable with the general population in India. However, no correlation was found between the risk factors of OSA with ESS. The reasons for EDS could be attributed to factors such as personal lifestyle, sleeping habits, and poor work-rest schedules. It is interesting to note that none of the aircrew, who is positive for OSA risk, is showing EDS/DS. This further confirms that OSA risk individuals may not have EDS/DS and this could be a reason why, OSA largely remains underdiagnosed and untreated in clinical practice. Conclusion: This cross-sectional study has brought the conclusion that both EDS and risk for OSA coexists even in a fit and healthy population of aircrew.

Background. The aim of this study was to analyze daytime sleepiness in a sample of patients diagnosed with sarcoidosis. Methods. A sample of 1197 German sarcoidosis patients was examined with the Epworth Sleepiness Scale (ESS), the Fatigue Assessment Scale, the Hospital Anxiety and Depression Scale, the Pittsburgh Sleep Quality Index, and the Short-Form Health Survey (SF-8). The patients’ ESS mean scores were compared with those obtained from a large general population sample. Results. Exactly 50% of the patients reached the criterion (ESS > 10) for excessive daytime sleepiness, compared with only 22.1% in the general population. The effect size for the mean score difference between both samples was d=0.62. The number of affected organs and the number of concomitant diseases proved to be significant independent predictors of daytime sleepiness. Sleepiness was associated with fatigue (r=0.45), anxiety (r=0.23), depression (r=0.28), sleep problems (r=0.23), and detriments in physical (r=−0.29) and mental (r=−0.28) quality of life. Conclusions. The issue of excessive daytime sleepiness should be considered in the management of sarcoidosis.
