Caffeine and blood pressure: a critical review perspective

AbstractThe WHO reported that high blood pressure (BP) is one of the primary causes of death worldwide. Hypertension (HPT) is a major risk factor for CVD and related diseases as well as for diseases, leading to a considerable increase in cardiovascular risk. Since BP response could also be influenced by caffeine, which is widely consumed with coffee and other items, it is important to define the possible effects associated with caffeine intake. The most recent findings aimed at clarifying the role of caffeine consumption on BP and HPT risk/incidence are conflicting and difficult to interpret. Therefore, in the present narrative review, we aimed to examine various methodological inaccuracies/aspects and factors that make studies difficult to be compared, in order to obtain a single consensus on the effects of caffeine intake on the risk of BP and HPT. We observed that this heterogeneity in results could be due to the presence of: (i) several variables affecting BP (such as age, sex, genetic and lifestyle aspects); (ii) different caffeine content of food and beverages; and (iii) caffeine metabolism. Moreover, different methodological aspects in the evaluation of daily dietary caffeine intake and in the BP measurement could add some other bias in the interpretation of results. Therefore, it is mandatory to consider all methodological aspects and confounding factors to generate a standardised methodology in order to increase cross-study consistency and minimise confounding effects of different variables on the relationship between BP response and HPT risk/incidence after caffeine intake.

ABSTRACT The relationship between genetic variation and caffeinated beverages intake has been investigated in various human populations, but not among North Africans who have distinct caffeinated beverage dietary habits. Therefore, this study analysed the association between rs6968865 (AHR), rs382140 (NRCAM), rs9526558 (CAB39L), rs7754744 (PDSS2) and rs68157013 (TAS2R43) SNPs from previous GWASs on habitual caffeine consumption and caffeinated beverage intake in 568 healthy blood donors from the Tunisian population. The AHR caffeine metabolism gene SNP was associated with coffee intake but not with green tea decoction. However, the association of SNPs with caffeine metabolism (CAB39L and PDSS2), addiction to caffeine (NRCAM) and perceived caffeine bitterness (TAS2R43) genes was only observed or was stronger with the highest caffeine-containing beverage green tea decoction. The researchers further detected an opposite association of PDSS2 gene SNP with coffee and tea intake. The study provides additional data on caffeinated beverage intake genetics in a population with specific dietary habits

ObjectiveLarge systolic blood pressure (SBP) variability has been proposed as a novel risk factor for dementia above and beyond SBP levels, but the underlying neuropathology is largely unknown. We investigated the relationship among visit-to-visit SBP variability, cognitive deterioration and underlying neuropathological changes.MethodsWe used longitudinal data (between 2005 and 2019) from the National Alzheimer’s Coordinating Center. 13,284 dementia-free participants aged≥50 years were followed over a median of 5.0 (interquartile range: 3.1-7.6) years. Neuropathology data were available in 1,400 autopsied participants. Visit-to-visit SBP variability was quantified from repeated annual SBP measurements. Cognitive deterioration was defined as conversion from normal cognition to mild cognitive impairment (MCI) or dementia, or from MCI to dementia.ResultsLarger visit-to-visit SBP variability was associated with cognitive deterioration (adjusted odds ratio comparing extreme quintiles: 2.64; 95%CI:2.29-3.04, P <0.001). It was also associated with a higher burden of vascular pathology (including microinfarcts, white matter lesion, atherosclerosis of the circle of Willis and arteriolosclerosis) and with neurofibrillary tangle pathology assessed by Braak staging (All P < 0.05). The association with cognitive deterioration and vascular pathology appeared stronger among those with normal cognition versus MCI at baseline. These findings were observed after adjusting for age, sex, mean SBP and other confounding variables. Similar results were observed for diastolic BP variability.ConclusionLarger visit-to-visit SBP variability was associated with cognitive deterioration. It was also associated with cerebrovascular pathology and neurofibrillary tangles. These results suggest the intertwined role of vascular and Alzheimer's disease pathology in the etiology of dementia.

Purpose: To examine the sleep of rugby league athletes the night before and following an evening match and to investigate the association between caffeine intake and sleep on the night of competition. Methods: On the night prior to, night of, and night after a professional rugby league match, 15 athletes had their sleep monitored using wrist activity monitors. Additionally, saliva samples were collected 60 minutes before and 30 minutes after the competition to assess salivary caffeine concentration. Differences in sleep across the 3 nights were examined using linear mixed models, and changes were assessed using effect size (ES). Pearson correlation (r) assessed the relationship between salivary caffeine levels and sleep indices. Results: On the night of competition, athletes went to bed later than the night before (P = .00002, ES = 1.84) and night after (P = .0003, ES = 1.49) competition. Consequently, their sleep duration was reduced on the night of competition compared with the previous night (P < .0000003, ES = 2.36) and night after competition (P = .001, ES = 1.53). Postcompetition salivary caffeine concentration was substantially elevated in athletes when compared with precompetition measures (P < .00000001, ES = 4.44), and moderate, nonsignificant correlations were observed between changes in salivary caffeine concentration and delayed bedtime (r = .48, P = .07), increased sleep latency (r = .45, P = .09), decreased sleep duration (r = −.30, P = .28), and reduced sleep efficiency (r = −.34, P = .22). Conclusions: These results demonstrate that evening competition results in sleep disturbance in rugby league athletes, and caffeine supplementation prior to and during competition leads to substantial increases in postcompetition salivary caffeine concentration.

The purpose of this study was to examine if offspring physical activity may affect the relationship between maternal overweight and offspring fatness and blood pressure (BP). Subjects included 144 maternal-child pairs (n = 74 boys and 70 girls, mean age = 7.3 yrs). Maternal prepregnancy BMI was determined by self-report. Offspring characteristics included resting systolic and diastolic BP, body fatness by dual energy x-ray absorbtiometry, and moderate-to-vigorous physical activity (MVPA) using the Actigraph accelerometer. Children whose mothers were overweight or obese prepregnancy (Prepreg OW) were significantly larger and fatter than children from mothers with a normal prepregnancy BMI (Prepreg NORM). Prepreg OW children also had higher mean arterial pressure than Prepreg NORM children. BP values were not different across maternal Prepreg BMI/MVPA groups. Percent fat was significantly different across Prepreg BMI/MVPA groups. Prepreg OW children that did not meet the daily recommended value of MVPA were the fattest. Prepreg OW children that attained 360 min of MVPA/day had a mean percent body fat that was similar to Prepreg NORM children of either MVPA group.

BACKGROUND: Caffeine can affect depressive symptoms and decision-making. OBJECTIVE: This study aims to examine the relationship between caffeinated beverages consumption with depressive symptoms and decision-making styles. METHODS: This cross-sectional study was conducted with 432 adults working in office environment. The questionnaire consisting of individuals’ socio-demographic attributes, the frequency and the amount of caffeinated beverages consumption, the “Epidemiological Research Center-Depression (CES-D) Scale” and the “Decision-Making Styles Scale” were used. The frequency and amount of caffeinated beverages were determined using the food frequency questionnaire (FFQ). The participants were asked to choose which cup/mug they prefer to drink their caffeinated beverages and what amount they consume that beverage at a time. All the data were collected using online platforms. RESULTS: In the study, 76.7%of the participants were female and the mean age was 31.5±8.0 years. The average daily total caffeine intake of the participants was 425.8±461.4 mg and the total CES-D scale score was 17.7±11.2 points. It was found that as the amount of caffeine consumed increased, intuitive decision-making decreased and depressive symptoms increased (p <  0.05). In linear regression analysis, total caffeine consumption was found to be a significant predictor for the intuitive decision-making score (B: –0.151; p:0.002). When caffeine consumption is controlled, intuitive and rational decision making decreases with increasing depressive symptoms while addiction and avoidance decision making increased (p <  0.05). CONCLUSIONS: As a result, the amount of caffeine consumed daily was related to intuitive decision-making but did not effect depression. It has been observed that depressive symptoms affect decision-making styles in different ways. To our knowledge, our study is the first to examine the effects of caffeine consumption on depression and decision-making styles. Accordingly, future studies may focus on the link between caffeine consumption, depression, and decision-making styles in larger populations and the mechanisms that influence this relationship.

Abstract Introduction Excessive caffeine intake has been thought to be a contributory factor for tinnitus. However, there has been no systematic review to elucidate the causal relationship between caffeine intake and the incidence of tinnitus. Objectives We performed the current review aiming at evaluating the evidence from the current literature for the relationship between caffeine intake and the incidence of tinnitus. Data Synthesis Databases including PubMed, Scopus, and Google scholar were searched for relevant articles. A total of 142 studies were screened for eligibility, of which four articles met our inclusion criteria: two were prospective cohorts and two were cross-sectional studies. Although one study found no association between caffeine consumption and the incidence of tinnitus, an inverse relationship was reported by two population-based studies. Concerning patients with preexisting tinnitus, reduction of caffeine intake in a subset who consumed 150 ml to 300 ml/day of coffee yielded a favorable outcome in tinnitus severity. However, those with higher dose intake were less prone to have improvement in the severity of tinnitus. Conclusion Although the current review was inconclusive, it appears that the incidence of tinnitus in previously unaffected individuals might be prevented by a high dose of caffeine intake. However, in preexisting tinnitus, a high dose of caffeine may adversely interfere with the efficacy of caffeine reduction.

AbstractObjectiveDietary Ca is now being recognized to play an important role not only in skeletal integrity, but also in the regulation of energy and metabolism. The aim of the present study was to estimate the relationship of dairy Ca intake with BMI and blood pressure (BP) in a sample derived from the Polish population.DesignCa intake was calculated from an interviewer-administered semi-quantitative FFQ. BMI was calculated from measured weight and height, and BP was measured by a physician.SettingCross-sectional epidemiological study on osteoporosis risk factors in Poland.SubjectsRandomly selected healthy adult persons (n 1259; 750 women and 509 men).ResultsDairy Ca intake was significantly lower in individuals with overweight/obesity (BMI≥25·00 kg/m2) and/or with elevated BP (systolic/diastolic ≥140/≥90 mmHg) than in those with normal body mass and BP, respectively. Ca intake was negatively correlated with BMI (r=−0·12, P<0·001), systolic BP (r=−0·11, P<0·001) and diastolic BP (r=−0·08, P<0·01). Daily dairy Ca intake below 1000 mg was a predictor for BMI≥25·0 kg/m2 (OR=1·44, P<0·005). This relationship was stronger in women, particularly premenopausal women.ConclusionsThe obtained results indicate the role of low dairy Ca intake in the development of obesity and hypertension, notably in premenopausal women.

In the teaching of design in architecture courses integrated in Bologna becomes evident the central importance of the student in the teaching/learning process. If the antinomy process/product is recurrent in didactic and pedagogical atmosphere in our historical context and teaching experience, others seem to arise with greater acuity, interest and additional platforms to enriching the debate, such as the relationship between problem/solution, knowledge/skills and experience/awareness. In these scenarios of antinomic problematic is important to understand the special role of the teacher and student, especially, to know how to organize the relative weights of each component during the academic years of two different cycles. It seems clear that the importance of process and product is not the same along the different academic years. It is also a fact that the teacher's role in the construction of the solution or of the problem is variable in the progression of learning, especially because the student needs to gain greater autonomy and judgment. Also, the acquisition of skills (reinforcing the disciplinary culture and methodological aspects) must constitute themselves as nuclear (1st cycle) and the knowledge (as global problematic) can be worked when the student has more autonomy and critical consciousness (2nd cycle). Because the experience of “to do” it isn’t always simultaneous of the awareness of “to know”, the optional courses should only provide eventual specialization in the 2nd cycle of studies.
