Effects of Cognitive Training on Cognitive Performance of Healthy Older Adults

AbstractThe purpose of this study was to determine the immediate effects of cognitive training on healthy older adults and verify the transfer effects of targeted and non-targeted abilities. The design consisted of a semi-randomized clinical controlled trial. The final sample was composed of 80 volunteers recruited from a Brazilian community (mean age = 69.69; SD = 7.44), which were separated into an intervention group (N = 47; mean age = 69.66, SD = 7.51) and a control group (N = 33; mean age = 69.73, SD = 7.45). Intervention was characterized by adaptive cognitive training with 12 individual training sessions of 60 to 90 minutes (once a week). Eight instruments were used to assess effects of cognitive training. Five were used to assess trained abilities (near effects), including: Memorization Tests (List and History), Picture Completion, Digit Span, Digit Symbol-Coding, and Symbol Search (the last four from WAIS-III). Two instruments assessed untrained abilities (far effects): Arithmetic and Matrix Reasoning (WAIS-III). The non-parametric repeated measures ANOVA test revealed a significant interaction between group by time interaction for Picture Completion [F(74) = 14.88, p = .0002, d = 0.90, CLES = 73.69%], Digit Symbol-Coding [F(74) = 5.66, p = .019, d = 0.55, CLES = 65.21%] and Digit Span [F(74) = 5.38, p = .02, d = 0.54, CLES = 64.85%], suggesting an interventional impact on these performance tasks. The results supported near transfer effects, but did not demonstrate a far transfer effects.

Abstract Introduction: Plasticity in intellectual functioning has been the target of significant research investment. Among non-pharmacological interventions, cognitive training appears as a promising option for delaying the effects of aging on cognition. Objective: The present study describes the procedures of a cognitive training program for healthy Brazilian elderly persons (without diagnosis of dementia). Method: Cognitive tasks for the training of attention, processing speed, episodic memory and working memory were carried out. The program training was performed over 12 individual sessions, and included an Instruction Book, a Stimulus Book and Protocol Record. To assess the effects of training, five subtests of the WAIS-III test were used: Picture completion, Coding, Arithmetic, Matrix Reasoning and Digit Span. The cognitive training program was tested with 15 individuals, divided into an experimental group (EG), which received training, and a control group (CG). The EG was formed of seven participants, aged between 70 and 82 years (M=73.57, SD=4.11) and with an average schooling of 5.8 years (SD=1.02). The CG was formed by 8 participants, aged 69-77 years (M=74.00, SD=4.58), and who had an average schooling of 2.88 years (SD=2.58). Results: Repeated measures ANOVA revealed a training effect for three subtests: Coding: [F(1)=5.40, p=0.03, η2G=0.09], Arithmetic [F(1)=9.03, p=0.01, η2G=0.004] and Picture completion [F(1)=8.01, p=0.01, η2G=0.19]. There were no gain effects for Matrix Reasoning [F(1)=1.43, p=0.25] and Digit Span [F(1)=10.04, p<0.001]. Conclusion: The results of this pilot study show the importance of testing the impact of cognitive training through a randomized clinical trial to verify its effects on the mental performance of older adults. The importance of greater disclosure in literature of the construction procedures involved in cognitive training tasks, as well as the formulation of intervention strategies, is highlighted.

This paper reports the results from a three-years follow-up study to access the long-term efficacy of a cognitive training for healthy older adults and investigates the effects of booster sessions on the cognitive performance of the participants using an innovative analytical approach from information theory. Design: semi-randomized quasi-experimental controlled design. Participants: 50 healthy older adults, (M = 73.3, SD = 7.77) were assigned into an experimental (N = 25; Mean age = 73.9; SD = 8.62) and a passive control group (N = 25; mean age = 72.9; SD = 6.97). Instruments: six subtests of WAIS and two episodic memory tasks. Procedures: the participants were assessed in four occasions: after the end of the original intervention, pre-booster sessions (three years after the original intervention), immediately after the booster sessions and three months after the booster sessions. Results: the repeated measures ANOVA showed that two gains reported in the original intervention were identified in the follow-up: Coding (F(1, 44) = 11.79, MSE = 0.77, p = .001, ηˆG2 = .084) and Picture Completion (F(1, 47) = 10.01, MSE = 0.73, p = .003, ηˆG2 = .060). After the booster sessions, all variables presented a significant interaction between group and time favorable to the experimental group (moderate to high effect sizes). To compare the level of cohesion of the cognitive variables between the groups, an entropy-based metric was used. The experimental group presented a lower level of cohesion in three of the four measurement occasions, suggesting a differential impact of the intervention with immediate and short-term effects, but without long-term effects.

Abstract This quasi-experimental designed study analyzed the effects of adaptive computer-based cognitive training among community-dwelling older adults. A 6-week (5 times/week) program was implemented with an intervention group (Difficulty Adaptive Training) and control group (Difficulty Fixed Training). General cognitive, memory, executive and attention functions were evaluated before (T1), completion (T2), and one month after intervention (T3). Sixty-one participants completed data collection. (1) General cognitive function: improved in both groups at T2, and T3, intervention group had better effect; (2) Memory function: improved in both groups in immediate, short and long-delayed recalls at T2 and T3, and recognition at T2. (3) Executive function: improved in both groups. Time of simple information processing was shortened at T2 and T3 in intervention group, at T3 in control group; TMT response inhibition was shortened at T2 and T3 in both groups. (4) Attention function: digit span forward was improved at T2 in intervention group.

ABSTRACT Objectives: Reminiscence promotes the acceptance of oneself and others, a sense of meaning, and the integration of the present and the past. The currently accepted classification contains eight reminiscence functions grouped in three broader functions: self-positive functions (identity, problem-solving, and death preparation); self-negative functions (bitterness revival, boredom reduction, and intimacy maintenance); and prosocial functions (conversation and teach-inform). The main objective of this study was to investigate how the eight dimensions change over time in a sample of healthy older adults after an intervention based on simple reminiscence and subsequent follow-up. Design: Participants were randomly assigned to the treatment or control group. Setting: Participants were evaluated individually before, immediately after, and 3 months after the intervention. Participants: Participants included 139 healthy older adults (71 intervention group and 68 control group). Intervention: Ten weekly sessions lasting 2 hours each were administered, based on a simple reminiscence program. Measurements: Participants were assessed with the Reminiscence Functions Scale. Results: After the intervention, in the treatment group, there was a significant increase in the self-positive functions of problem-solving and death preparation, and these effects were maintained even at the follow-up; there was a significant reduction in the self-negative functions of bitterness revival and intimacy maintenance; and there was a significant increase in the prosocial function of conversation. Conclusions: When applying an intervention based on reminiscence, it is important to evaluate how it influences these functions because this information can help us to modify the objectives of the intervention and increase its effectiveness.

Objective: Physical functioning is closely associated with cognition. The current study assessed the impact of three cognitive training programs on objective measures of physical functioning across 5 years. Method: Older adults randomized to a processing speed ( n = 702), reasoning ( n = 694), or memory ( n = 703) training intervention were compared with those randomized to a no-contact control condition ( n = 698). Intention-to-treat (ITT) and treatment-received/dosage (time-varying number of training sessions) analyses were conducted. Results: There were no transfer effects in the ITT analyses. Treatment-received models demonstrated that training sessions (i.e., higher dosage) across all intervention arms transferred to better maintained Digit Symbol Copy and Turn 360 performance relative to the control group. More reasoning training transferred to better grip strength. Discussion: This is the first study to demonstrate differential longitudinal cognitive training transfer effects to three performance-based physical functioning measures. Future research should investigate mechanisms of far-transfer effects.

Abstract Objective The natural learning experience from infancy to emerging adulthood, when considerable cognitive and functional growth is observed, mandates learning multiple real-world skills simultaneously. The present studies investigated whether learning multiple real-world skills simultaneously is possible in older adults and also whether it improves both their cognitive abilities (working memory, episodic memory, and cognitive control) and functional independence. Method Over two studies (15 and 27 participants), older adults learned at least three new skills (e.g., Spanish, drawing, music composition) simultaneously for 3 months. Participants completed cognitive and functional assessments before, during, and after the intervention in both studies. Participants were recruited sequentially for an intervention or no-contact control group in Study 1, and Study 2 included only an intervention group, who also completed assessments 4–6 weeks prior to the start of the intervention (i.e., they served as their own control group). Results Results from both studies show that simultaneously learning multiple skills is feasible and potentially beneficial for healthy older adults. Learning multiple skills simultaneously increased cognitive abilities in older adults by midpoint of the intervention, to levels similar to performance in a separate sample of middle-aged adults, 30 years younger. Discussion Our findings demonstrate the feasibility and potential of conducting a real-world skill-learning intervention involving learning three novel skills with older adults. Our multiskill intervention may provide broad cognitive gains, akin to the benefits experienced earlier in the life span.

Objective: The purpose of this randomized-controlled pilot study was to explore the effectiveness of a home-based computerized cognitive training intervention in improving cognitive function in a population of older adults with mild cognitive impairment who are living with HIV. Methods: In all, 24 participants were enrolled in this study. All study participants were impaired [defined as Montreal Cognitive Assessment (MoCA) score < 26]; 12 were randomly assigned to a computer-training intervention group and 12 to a control group. The intervention group used a home-based computerized cognitive training program for 8 weeks, while the control group received health-related newsletter via email and follow-up phone calls. Cognitive function was measured at study entry, immediately post intervention, and 8 and 16 weeks post intervention Results: This study achieved a 92% retention rate, losing two persons from the intervention group. Participants in the intervention group scored significantly higher on cognitive testing immediately post intervention compared to the control group: F(1, 19) = 4.92, p = 0.04. The partial Eta squared of 0.32 indicates a small to moderate effect size. Discussion: Cognitive improvement was seen immediately after the intervention, and cognitive improvement was still evident 16 weeks post intervention. Cognitive training could be considered as an option for older adults with HIV experiencing mild cognitive impairment.

Objective. To examine plasticity of inhibition, as indexed by practice effects of inhibition tasks and the associated transfer effects, using a multiple task approach in healthy older adults.Method. Forty-eight healthy older adults were evenly assigned to either a practice group or a no-contact control group. All participants completed pretest (2.5 hours) and posttest (2 hours) sessions, with a 2-week interval in between. During the 2-week interval, only the practice group completed six 30-minute practice sessions (three sessions per week for two consecutive weeks) of three lab-based inhibition tasks.Results. All three inhibition tasks demonstrated significant improvement across practice sessions, suggesting practice-induced plasticity. The benefit, however, only transferred to near-near tasks. The results are inconclusive with regard to the near-far and far-far transfer effects.Discussion. This study further extends literature on practice effects of inhibition in older adults by using a multiple task approach. Together with previous work, the current study suggests that older adults are able to improve inhibition performance through practice and transfer the practice gains to tasks that overlap in both target cognitive ability and task structure (i.e., near-near tasks).

ACKGROUND: Cognitive interventions have the potential to enhance cognition among healthy older adults. However, little attention has been paid to the effect of cognitive training (CT) on mood and activities of daily living (ADL). OBJECTIVES: To assess the effectiveness of a multicomponent CT using a training program of executive functions, attention, memory and visuospatial functions (TEAM-V Program) on cognition, mood and instrumental ADL. DESIGN: A randomized, single-blinded, treatment-as-usual controlled trial. SETTING: Geriatric clinic in Bangkok, Thailand. PARTICIPANTS: 77 nondemented community-dwelling older adults (mean age 65.7±4.3 years). INTERVENTION: The CT (TEAM-V) program or the treatment-as-usual controlled group. The TEAM-V intervention was conducted over 5 sessions, with a 2-week interval between each session. Of 77 participants randomized (n=40 the TEAM-V program; n=37 the control group). MEASUREMENTS: The Thai version of Montreal Cognitive Assessment (MoCA), The Alzheimer’s Disease Assessment Scale-Cognitive Subscale (ADAS-cog), Thai version of Hospital Anxiety and Depression Scale (HADS) and The Chula ADL were used to assess at baseline, 6 months and 1 year. RESULTS: Compared with the control arm, the TEAM-V Program was associated with reducing anxiety (P = 0.004). Compared with the baseline, participants receiving the TEAM-V Program were associated with significantly improved general cognition (MoCA, P < 0.001), immediate recall (word recall task, P = 0.01), retrieval and retention of memory process (word recognition task, P = 0.01), attention (number cancellation part A, P < 0.001) and executive function (maze test, P = 0.02) at 1 year. No training effects on depression (P = 0.097) and IADL (P = 0.27) were detected. CONCLUSIONS: The TEAM-V Program was effective in reducing anxiety. Even though, the program did not significantly improve cognition, depression and ADL compared with the control group, global cognition, memory, attention and executive function improved in the intervention group compared with baseline. Further studies incorporating a larger sample size, longitudinal follow-up and higher-intensity CT should be conducted.

The purpose of this present study was to examine the effect of Taichi softball (TCSB) on physical function in Chinese older adults. Eighty Chinese older adults were randomly assigned into either an experimental group experiencing four 90-minute TCSB sessions weekly for seven consecutive weeks or a control group. At baseline and 7 weeks later, all participants were asked to perform physical functional tests for both lower and upper limbs. Multiple separate Analyses of Variance (ANOVA) with repeated measures were applied to evaluate the effects of TCSB on function-related outcomes between baseline and postintervention in the two groups. The findings indicate that a short-term and intensive TCSB training program does not only improve low limb-related physical function such as dynamic balance and leg strength, but also strengthen upper limb-related physical function (e.g., arm and forearm strength, shoulder mobility, fine motor control, handgrip strength, and fine motor function). Health professionals could take into account TCSB exercise as an alternative method to help maintain or alleviate the inevitable age-related physical function degeneration in healthy older adults. In addition, researchers could investigate the effect of TCSB exercise on physical function in special populations such as patients with different chronic diseases or neurological disorder (e.g., Parkinson’s disease).
