Placebo and Nocebo Effects in Chronic Pain Patients

Placebo, as well as nocebo, effects have been primarily investigated in studies with healthy volunteers exposed to acute experimental pain. Yet with regard to chronic pain patients, there is emerging evidence for significant placebo effects but not for nocebo effects. Expectations of pain relief are known to contribute to placebo effects, and lately the influence of emotional feelings has also been investigated. In this line of research, an experiential method has been applied to capture the emotional feelings that chronic pain patients experience during placebo and nocebo interventions. The findings indicate that the patients’ expectations of treatment effects are highly embedded in their emotional feelings. Hence, in order to optimize placebo factors in the clinical treatment of patients, it may be pivotal to investigate and enhance both expectancies and emotional feelings about treatments.

© 2019 Copyright held by the owner/author(s). Chronic pain is ongoing pain lasting for long periods of time after the initial injury or disease has healed. Chronic pain is difficult to treat and can affect the daily lives of patients. Distraction therapy is a proven way of relieving pain by redirecting the focus of patients' attention. Virtual reality is an effective platform for distraction therapy as it immerses the user visually, aurally, and even somewhat physically in a virtual world detached from reality. There is little research done on the effects that physical interactions have on pain management. This project aims to evaluate different types of extended reality (XR) interactions, including full body movement, for chronic pain patients to determine which is the best for pain relief. We are building a prototype for participants to interact both mentally and physically and measuring the reduction in subjective pain ratings at various points of the XR experience.

Experimental pain testing requires specific equipment and may be uncomfortable for patients. The Pain Sensitivity Questionnaire (PSQ) was developed to assess pain sensitivity, based on the pain intensity ratings (range: 0–10) of painful situations that occur in daily life. The main objective of this study was to carry out a cross-cultural adaptation and validation of the Spanish version of the PSQ (PSQ-S). A total of 354 subjects (296 healthy and 58 chronic pain patients) filled in the PSQ-S. A subgroup of 116 subjects performed experimental pain testing, including two modalities (cold and pressure), with different measures: pain intensity rating, pressure pain threshold, and tolerance. The validation results showed two factors: PSQ-S-moderate and PSQ-S-minor and, for the total scale and the two factors, an excellent internal consistency (Cronbach’s alpha coefficient > 0.9) and a substantial reliability (Intraclass Correlation Coefficient > 0.8). We obtained strong correlations with all the experimental pain rating parameters, catastrophizing, and depression variables, as well as moderate correlations with anxiety, central sensibilization, and impact on the quality of life. Chronic pain patients received elevated PSQ-S scores compared to healthy controls, and three cut-off values (PSQ-S-total = 7.00, PSQ-S-moderate = 7.57, and PSQ-S-minor = 6.29) based on ROC curve analyses were shown to be able to discriminate between healthy adults and adults with chronic pain. Therefore, PSQ-S may be a simple alternative to experimental pain procedures for clinical and experimental pain research.

Abstract. A growing body of literature suggests that chronic-pain patients suffer from problems in various neuropsychological domains, including executive functioning. In order to better understand which components of executive functioning (inhibition, shifting and/or updating) might be especially affected by pain and which mechanisms might underlie this association, we conducted a systematic review, including both chronic-pain studies as well as experimental-pain studies. The chronic-pain studies (N = 57) show that pain is associated with poorer executive functioning. The findings of experimental-pain studies (N = 28) suggest that this might be a bidirectional relationship: Pain can disrupt executive functioning, but poorer executive functioning might also be a risk factor for higher vulnerability to pain.
