Abstract Primary progressive aphasia (PPA) is a relatively new diagnostic entity, for which few behavioral treatments have been investigated. Recent work has helped to clarify the nature of distinct PPA variants, including a nonfluent variant (NFV-PPA), a logopenic variant (LV-PPA), and a semantic variant (SV-PPA). This paper reviews treatment research to date in each subtype of PPA, including restitutive, augmentative, and functional approaches. The evidence suggests that restitutive behavioral treatment can result in improved or stabilized language performance within treated domains. Specifically, sentence production and lexical retrieval have been addressed in NFV-PPA, whereas lexical retrieval has been the primary object of treatment in LV and SV-PPA. Use of augmentative communication techniques, as well as implementation of functional communication approaches, also may result in improved communication skills in individuals with PPA. The ideal treatment approach may be one that combines restitutive, augmentative, and functional approaches to treatment, in order to maximize residual cognitive-linguistic skills in patients. Additional research is warranted to determine which modes of treatment are most beneficial in each type of PPA at various stages of severity.

Background: Previous studies explored the use of noninvasive biomarkers of speech and language for the detection of mild cognitive impairment (MCI). Yet, most of them employed single task which might not have adequately captured all aspects of their cognitive functions. Objective: The present study aimed to achieve the state-of-the-art accuracy in detecting individuals with MCI using multiple spoken tasks and uncover task-specific contributions with a tentative interpretation of features. Methods: Fifty patients clinically diagnosed with MCI and 60 healthy controls completed three spoken tasks (picture description, semantic fluency, and sentence repetition), from which multidimensional features were extracted to train machine learning classifiers. With a late-fusion configuration, predictions from multiple tasks were combined and correlated with the participants’ cognitive ability assessed using the Montreal Cognitive Assessment (MoCA). Statistical analyses on pre-defined features were carried out to explore their association with the diagnosis. Results: The late-fusion configuration could effectively boost the final classification result (SVM: F1 = 0.95; RF: F1 = 0.96; LR: F1 = 0.93), outperforming each individual task classifier. Besides, the probability estimates of MCI were strongly correlated with the MoCA scores (SVM: –0.74; RF: –0.71; LR: –0.72). Conclusion: Each single task tapped more dominantly to distinct cognitive processes and have specific contributions to the prediction of MCI. Specifically, picture description task characterized communications at the discourse level, while semantic fluency task was more specific to the controlled lexical retrieval processes. With greater demands on working memory load, sentence repetition task uncovered memory deficits through modified speech patterns in the reproduced sentences.

AbstractLexical retrieval requires selecting and retrieving the most appropriate word from the lexicon to express a desired concept. Few studies have probed lexical retrieval with tasks other than picture naming, and when non-picture naming lexical retrieval tasks have been applied, both convergent and divergent results emerged. The presence of a single construct for auditory and visual processes of lexical retrieval would influence cognitive rehabilitation strategies for patients with aphasia. In this study, we perform support vector regression lesion-symptom mapping using a brain tumor model to test the hypothesis that brain regions specifically involved in lexical retrieval from visual and auditory stimuli represent overlapping neural systems. We find that principal components analysis of language tasks revealed multicollinearity between picture naming, auditory naming, and a validated measure of word finding, implying the existence of redundant cognitive constructs. Nonparametric, multivariate lesion-symptom mapping across participants was used to model accuracies on each of the four language tasks. Lesions within overlapping clusters of 8,333 voxels and 21,512 voxels in the left lateral prefrontal cortex (PFC) were predictive of impaired picture naming and auditory naming, respectively. These data indicate a convergence of heteromodal lexical retrieval within the PFC.

AbstractThis article presents the results of a pilot study carried out based on texts from 15 immigrant children aged 6 to 9 years, who are learning Spanish in situations of immersion in the Communities of Madrid and Castilla-La Mancha. The aim is to understand how these students try to integrate into the school context and especially to determine whether the development of written expression during the early years of primary education allows them to carry out more complex linguistic actions aimed at communication, such as expressing positive attitudes towards the recipient. These actions may reveal the need to communicate and, therefore, the need to learn the language in order to integrate. The texts were taken from the ESCONES Corpus and were collected in a prior study on lexical retrieval and auditory perception in the development of communicative skills in children aged 6 to 9. The analysis carried out considered the vocabulary used, syntactic complexity and the use of linguistic actions in the different grades and found that the development of written expression may allow students to better express actions related to manifesting positive feelings and attitudes towards their interlocutor.

The development of verbal fluency is associated with the maturation of executive function skills, such as the ability to inhibit irrelevant information, shift between tasks and hold information in working memory. Some evidence suggests that multilinguistic upbringing may underpin disadvantages in verbal fluency and lexical retrieval, but can also afford executive function advantages beyond the language system including possible beneficial effects in older age. This study examined the relationship between verbal fluency and executive function in 324 individuals across the lifespan by assessing the developmental trajectories of English monolingual and multilingual children aged 7 to 15 years (N=154) and adults from 18 to 80 years old (N=170). The childhood data indicated patterns of improvement in verbal fluency and executive function skills as a function of age. Multilingual and monolingual children had comparable developmental trajectories in all linguistic and non-linguistic measures used in the study with the exception of planning, for which monolingual children showed a steeper improvement over the studied age range relative to multilingual children. For adults, monolinguals and multilingual participants had comparable performance on all measures with the exception of non-verbal inhibitory control and response times on the Tower of London task: monolinguals showed a steeper decline associated with age. Exploratory factor analysis indicated that verbal fluency was associated with working memory and fluid intelligence in monolingual participants but not in multilinguals. These findings raise the possibility that early acquisition of an additional language may impact on the development of the functional architecture serving high-level human cognition.

Abstract This study investigated the unresolved issue of potential sources of heritage language attrition. To test contributing effects of three learner variables – age of second language acquisition, length of residence, and language input – on heritage children's lexical retrieval accuracy and speed, we conducted a real-time word naming task with 68 children (age 11–14 years) living in South Korea who spoke either Chinese or Russian as a heritage language. Results of regression analyses showed that the participants were less accurate and slower in naming target words in their heritage language as their length of residence in Korea and the amount of Korean input increased. The age of Korean acquisition did not significantly influence their performance. These findings support the claim that heritage speakers’ language experience is a more reliable predictor of first language attrition than age of acquisition. We discuss these findings in light of different approaches to explaining language attrition.
