Can Behavioral Speech-In-Noise Tests Improve the Quality of Hearing Aid Fittings?

The purpose of this article is to propose 4 dimensions for consideration in hearing aid fittings and 4 tests to evaluate those dimensions. The 4 dimensions and tests are (a) working memory, evaluated by the Revised Speech Perception in Noise test (Bilger, Nuetzel, & Rabinowitz, 1984); (b) performance in noise, evaluated by the Quick Speech in Noise test (QSIN; Killion, Niquette, Gudmundsen, Revit, & Banerjee, 2004); (c) acceptance of noise, evaluated by the Acceptable Noise Level test (ANL; Nabelek, Tucker, & Letowski, 1991); and (d) performance versus perception, evaluated by the Perceptual–Performance test (PPT; Saunders & Cienkowski, 2002). The authors discuss the 4 dimensions and tests in the context of improving the quality of hearing aid fittings.

Purpose: The present study examines auditory training outcomes in terms of speech perception in noise, working memory, reasoning ability, and subjective hearing aid satisfaction. Methods: Ten older adults who wear hearing aids (mean age: 71.6) voluntarily participated in the study. For the training material, 78 sentence sets composed of 3 to 5 sentences were used. During the auditory training, participants tried to remember the order of sentences. Among the 78 sentence sets, 45 contained clues and the rest did not. Each participant underwent assessments including speech perception in noise and digit spans and reasoning test and completed a self-report hearing aid satisfaction questionnaire both pre- and post-auditory training. The participants completed eight sessions of the auditory training. Results: The results showed a statistically significant increase in speech recognition ability in noise, short-term memory, and working memory, as well as satisfaction with hearing aid use. Despite a slight increase on the reasoning test, there was no statistically significant improvement.Conclusion: The results of this study suggested that the auditory training in remembering the order of sentences improved speech perception in noise, sensory and working memory, and subjective satisfaction with hearing aids. Future research can investigate more effective auditory training tools to improve the various cognitive skills and communication ability of older adults who wear hearing aids.

Background noise is a significant factor influencing hearing-aid satisfaction and is a major reason for rejection of hearing aids. Attempts have been made by previous researchers to relate the use of hearing aids to speech perception in noise (SPIN), with an expectation of improved speech perception followed by an increased acceptance of hearing aids. Unfortunately, SPIN was not related to hearing-aid use or satisfaction. A new measure of listener reaction to background noise has been proposed. The acceptable noise level (ANL), expressed in decibels, is defined as a difference between the most comfortable listening level for speech and the highest background noise level that is acceptable when listening to and following a story. The ANL measure assumes that speech understanding in noise may not be as important as is the willingness to listen in the presence of noise. It has been established that people who accept background noise have smaller ANLs and tend to be "good" users of hearing aids. Conversely, people who cannot accept background noise have larger ANLs and may only use hearing aids occasionally or reject them altogether. Because this is a new measure, it was important to determine the reliability of the ANL over time with and without hearing aids, to determine the effect of acclimatization to hearing aids, and to compare the ANL to well-established measures such as speech perception scores collected with the SPIN test. Results from 50 listeners indicate that for both good and occasional hearing aid users, the ANL is comparable in reliability to the SPIN test and that both measures do not change with acclimatization. The ANLs and SPIN scores are unrelated. Although the SPIN scores improve with amplification, the ANLs are unaffected by amplification, suggesting that the ANL is inherent to an individual and can be established prior to hearing aid fitting as a possible predictor of hearing-aid use. Background and Aim: In most everyday sett­ings, speech is heard in the presence of com­peting sounds and speech perception in noise is affected by various factors, including cognitive factors. In this regard, bilingualism is a pheno­menon that changes cognitive and behavioral processes as well as the nervous system. This study aimed to evaluate speech perception in noise and compare differences in Kurd-Persian bilinguals versus Persian monolinguals. Methods: This descriptive-analytic study was performed on 92 students with normal hearing, 46 of whom were bilingual Kurd-Persian with a mean (SD) age of 22.73 (1.92) years, and 46 other Persian monolinguals with a mean (SD) age of 22.71 (2.28) years. They were examined by consonant-vowel in noise (CV in noise) test and quick speech in noise (Q-SIN) test. The obtained data were analyzed by SPSS 21. Results: The comparison of the results showed differences in both tests between bilingual and monolingual subjects. In both groups, the reduc­tion of signal-to-noise ratio led to lower scores, but decrease in CV in noise test in bilinguals was less than monolinguals (p < 0.001) and in the Q-SIN test, the drop in bilinguals’ score was  more than monolinguals (p = 0.002). Conclusion: Kurd-Persian bilinguals had a bet­ter performance in CV in noise test but had a worse performance in Q-SIN test than Persian monolinguals.

Objective The study attempted to track speech-in-noise perception and auditory long latency responses (ALLRs) over a period of hearing aid use in naïve hearing aid users. The primary aim was to investigate the relationship of change in speech-in-noise perception with the change in ALLRs. Method Thirty adults with mild-to-moderate sensorineural hearing loss (clinical group) and 17 adults with normal hearing (control group) in the age range of 23–60 years participated in the study. Syllable identification in noise (SIN) and ALLRs in noise were measured three times (three sessions) over a period of 2 months of hearing aid use. Results Results showed a significant increase in SIN and a decrease in the latency of ALLRs in the later sessions compared to the baseline session in the clinical group. However, the changes seen across the three sessions in the control group were not statistically significant. The magnitude of change in ALLRs seen in the clinical group did not significantly correlate with the change in SIN scores seen in them. Conclusions The study provides evidence for improvements in speech perception in noise and in processing time of auditory cortical areas with hearing aid acclimatization. However, it is important to note that the improvement in ALLRs does not assure improvement in speech perception in noise.

Abstract Although bilinguals benefit from semantic context while perceiving speech-in-noise in their native language (L1), the extent to which bilinguals benefit from semantic context in their second language (L2) is unclear. Here, 57 highly proficient English–French/French–English bilinguals, who varied in L2 age of acquisition, performed a speech-perception-in-noise task in both languages while event-related brain potentials were recorded. Participants listened to and repeated the final word of sentences high or low in semantic constraint, in quiet and with a multi-talker babble mask. Overall, our findings indicate that bilinguals do benefit from semantic context while perceiving speech-in-noise in both their languages. Simultaneous bilinguals showed evidence of processing semantic context similarly to monolinguals. Early sequential bilinguals recruited additional neural resources, suggesting more effective use of semantic context in L2, compared to late bilinguals. Semantic context use was not associated with bilingual language experience or working memory.

Abstract Background The objective of this study was to evaluate the effect of “Spatially separated speech in noise” auditory training on the ability of speech perception in noise among bimodal fitting users. The assumption was that the rehabilitation can enhance spatial hearing and hence speech in noise perception. This study was an interventional study, with a pre/post-design. Speech recognition ability was assessed with the specific tests. After performing the rehabilitation stages in the intervention group, the speech tests were again implemented, and by comparing the pre- and post-intervention data, the effect of auditory training on the speech abilities was assessed. Twenty-four children of 8–12 years who had undergone cochlear implantation and continuously used bimodal fitting were investigated in two groups of control and intervention. Results The results showed a significant difference between the groups in different speech tests after the intervention, which indicated that the intervention group have improved more than the control group. Conclusion It can be concluded that “Spatially separated speech in noise” auditory training can improve the speech perception in noise in bimodal fitting users. In general, this rehabilitation method is useful for enhancing the speech in noise perception ability.

The perception of speech in noise is challenging for children with cochlear implants (CIs). Singing and musical instrument playing have been associated with improved auditory skills in normal-hearing (NH) children. Therefore, we assessed how children with CIs who sing informally develop in the perception of speech in noise compared to those who do not. We also sought evidence of links of speech perception in noise with MMN and P3a brain responses to musical sounds and studied effects of age and changes over a 14–17 month time period in the speech-in-noise performance of children with CIs. Compared to the NH group, the entire CI group was less tolerant of noise in speech perception, but both groups improved similarly. The CI singing group showed better speech-in-noise perception than the CI non-singing group. The perception of speech in noise in children with CIs was associated with the amplitude of MMN to a change of sound from piano to cymbal, and in the CI singing group only, with earlier P3a for changes in timbre. While our results cannot address causality, they suggest that singing and musical instrument playing may have a potential to enhance the perception of speech in noise in children with CIs.

PurposeThe aim of the study was to investigate the effect of 3 hearing aid fitting procedures on provided gain of the hearing aid in bimodal cochlear implant users and their effect on bimodal benefit.MethodThis prospective study measured hearing aid gain and auditory performance in a cross-over design in which 3 hearing aid fitting methods were compared. Hearing aid fitting methods differed in initial gain prescription rule (NAL-NL2 and Audiogram+) and loudness balancing method (broadband vs. narrowband loudness balancing). Auditory functioning was evaluated by a speech-in-quiet test, a speech-in-noise test, and a sound localization test. Fourteen postlingually deafened adult bimodal cochlear implant users participated in the study.ResultsNo differences in provided gain and in bimodal performance were found for the different hearing aid fittings. For all hearing aid fittings, a bimodal benefit was found for speech in noise and sound localization.ConclusionOur results confirm that cochlear implant users with residual hearing in the contralateral ear substantially benefit from bimodal stimulation. However, on average, no differences were found between different types of fitting methods, varying in prescription rule and loudness balancing method.

Background and Aim: Migraine is a relatively common neurovascular disease. Audiology studies have shown some ways of influencing migraine by the auditory pathways from cochlea to the auditory cortex. Considering that one of the most important functions of the central auditory system is speech perception in challenging conditions, the purpose of this study was to evaluate the ability to understand speech in noise in migraineurs without aura, and compare it with normal subjects.Methods: In this cross-sectional study, 30 migraineurs without aura aged 17 to 41 years (mean=31.9, SD=6.89) and 30 normal individuals who were matched for age and sex with the migraine group were evaluated by quick speech-in-noise test (Q-SIN). The correlation between duration of the disease and the frequency of attacks per month and signal-to-noise ratio (SNR) loss, as well as the role of headache severity on the scores were assessed.Results: In Q-SIN test, the mean SNR loss in migraineurs without aura was greater than that in controls (p<0.05). But this ability did not differ between males and females (p>0.05). There was no correlation between the duration of migraine, frequency of attacks per month and the severity of headache with SNR loss (p>0.05).Conclusion: Migraineurs without aura sometimes have difficulties in speech perception in noise which is not affected by duration of disease, its frequency and the severity of the attacks.
