Viele Menschen mit einem akuten Koronarsyndrom (AKS) leiden komorbid an einer depressiven Symptomatik und Demoralisierung. Dies geht mit einem niedrigen therapeutischen Commitment, häufigeren Rückfallquoten und erhöhter Mortalität einher. Wissenschaftler aus Italien überprüften nun, ob eine Kombinationsbehandlung aus kognitiver Verhaltenstherapie (KVT) und Well-Being Therapie (WBT) sowohl depressive als auch kardiovaskuläre Symptome nachhaltig reduzieren kann.

Purpose Hearing loss self-management refers to the knowledge and skills people use to manage the effects of hearing loss on all aspects of their daily lives. The purpose of this study was to investigate the relationship between self-reported hearing loss self-management and hearing aid benefit and satisfaction. Method Thirty-seven adults with hearing loss, all of whom were current users of bilateral hearing aids, participated in this observational study. The participants completed self-report inventories probing their hearing loss self-management and hearing aid benefit and satisfaction. Correlation analysis was used to investigate the relationship between individual domains of hearing loss self-management and hearing aid benefit and satisfaction. Results Participants who reported better self-management of the effects of their hearing loss on their emotional well-being and social participation were more likely to report less aided listening difficulty in noisy and reverberant environments and greater satisfaction with the effect of their hearing aids on their self-image. Participants who reported better self-management in the areas of adhering to treatment, participating in shared decision making, accessing services and resources, attending appointments, and monitoring for changes in their hearing and functional status were more likely to report greater satisfaction with the sound quality and performance of their hearing aids. Conclusion Study findings highlight the potential for using information about a patient's hearing loss self-management in different domains as part of clinical decision making and management planning.

Because of the increasing number of transgender people requesting speech-language pathology services, because having gender-incongruent voice and communication has major negative impacts on an individual's social participation and well-being, and because voice and communication training is supported by an improving evidence-base, it is becoming more common for universities to include transgender-specific theoretical and clinical components in their speech-language pathology programs. This paper describes the theoretical and clinical education provided to speech-language pathology students at La Trobe University in Australia, with a particular focus on the voice and communication training program offered by the La Trobe Communication Clinic. Further research is required to determine the outcomes of the clinic's training program in terms of student confidence and competence as well as the effectiveness of training for transgender clients.

Current research on feeding outcomes after discharge from the neonatal intensive care unit (NICU) suggests a need to critically look at the early underpinnings of persistent feeding problems in extremely preterm infants. Concepts of dynamic systems theory and sensitive care-giving are used to describe the specialized needs of this fragile population related to the emergence of safe and successful feeding and swallowing. Focusing on the infant as a co-regulatory partner and embracing a framework of an infant-driven, versus volume-driven, feeding approach are highlighted as best supporting the preterm infant's developmental strivings and long-term well-being.

Die Koronare Herzkrankheit kann sich akut oder chronisch-rezidivierend mit meist belastungsabhängigen pektanginösen Beschwerden oder Atemnot manifestieren. Die Unterscheidung zwischen stabiler und instabiler Verlaufsform ist prognostisch wichtig, instabile Patienten müssen wie ein akutes Koronarsyndrom stationär abgeklärt werden, bei stabiler Symptomatik kann die weitere Diagnostik mehrheitlich ambulant erfolgen. Differentialdiagnostisch kommen eine Vielzahl anderer kardialer und extrakardialer Ursachen für Thoraxbeschwerden in Frage. Wichtigste initiale diagnostische Schritte sind eine kardiovaskuläre Risikostratifizierung sowie der Nachweis einer Ischämie (bzw. Narbe, Nekrose) in Ruhe oder meist unter Belastung, allenfalls ergänzt durch eine bildgebende Methode. Die Beurteilung der Leistungsfähigkeit erfolgt anhand physiologischer Parameter (Watt, VO2max. bzw. MET, Distanz) mittels Ergometrie, Spiroergometrie oder 6-Minuten-Gehtest (z.B. bei Herzinsuffizienz). Für die Beurteilung der Arbeitsfähigkeit sind zusätzliche Faktoren ausschlaggebend.

A high-quality diet is one of the foundations of health and well-being. For a long time in human history, diet was chiefly a source of energy and macronutrients meant to still hunger and give the strength for work and activities that were in general much harder than nowadays. Only few persons could afford to emphasize enjoyment. In the assessment of quality, organoleptic properties were major criteria to detect spoilage and oxidative deterioration of food. Today, food hygiene is a quality aspect that is often taken for granted by consumers, despite its lack being at the origin of most food-borne diseases. The discovery of micronutrients entailed fundamental changes of the concept of diet quality. However, non-essential food components with additional health functions were still barely known or not considered important until recently. With the high burden of obesity and its associated diseases on the rise, affluent, industrialized countries have developed an increased interest in these substances, which has led to the development of functional foods to optimize special body functions, reduce disease risk, or even contribute to therapeutic approaches. Indeed, nowadays, high contents of energy, fat, and sugar are factors associated with a lower quality of food, and products with reduced amounts of these components are valued by many consumers. At the same time, enjoyment and convenience are important quality factors, presenting food manufacturers with the dilemma of reconciling low fat content and applicability with good taste and appealing appearance. Functional foods offer an approach to address this challenge. Deeper insights into nutrient-gene interactions may enable personalized nutrition adapted to the special needs of individuals. However, so far, a varied healthy diet remains the best basis for health and well-being.
