Introduction: Systemic lupus erythematosus (SLE) is a complex autoimmune disease characterized by presence of nucleus autoantibody and affected multiple organ. Systemic lupus erythematosus is more common in women than men with ratio 2:1 to 15:1. Men with SLE often have a more aggressive clinical course, lead to a poorer prognosis compared with women with SLE. Case Report: A man, 29 years old came to hospital with main complain joint pain increased since 1 week ago, accompanied with red spot on face, trunk, hands, foot, and back, hair loss, swollen leg, mouth ulcer, and fatique. Malar rash and discoid rash were identified from physical examination. From laboratorium, ANA profile was positive for RNP/Sm, Sm, dsDNA, and histone. Skin biopsy showed a lupus discoid. Conclusion: The patient was treated with pulse-dose methylprednisolone for 3 days and showed a good response clinically.

Background Carotid artery stent (CAS) placement may be used as an alternative to carotid endarterectomy in selected patients with symptomatic and asymptomatic carotid artery disease. Perhaps because stroke is an uncommon event after CAS, the angiographic predictors of stroke have been incompletely described. Methods A total of 1070 catheter-based angiograms were centrally reviewed in patients undergoing CAS enrolled in the Carotid Revascularization Endarterectomy versus Stenting Trial (CREST). Of these patients, 43 (4%) experienced periprocedural stroke. Angiograms were reviewed using qualitative and quantitative methodology before and after CAS. Reviews were done blinded to patient outcomes and strokes were adjudicated by a central committee blinded to results of angiography. The view showing the greatest carotid stenosis severity was selected. Extreme tortuosity was defined as 2 severe bends >90 degrees (maximum angulation/20 mm segment) beyond the target lesion, sequential lesions as lesions separated by ≥10 mm, and narrow mouth ulcer as a discrete area of contrast extension beyond the normal arterial lumen with a narrow inlet into the ulceration. Results The internal carotid artery reference diameter was 4.2 ± 0.8 mm, the minimal lumen diameter was 1.1 ± 0.6 mm, and the baseline % diameter stenosis was 74.1 ± 11.6%. There were no differences in vessel size or severity of the stenosis in patients with and without stroke. Morphologic predictors significantly related to the risk of subsequent stroke are listed in the Table . Calcification, lesion eccentricity, baseline flow, and distance from the bifurcation were not predictive of periprocedural stroke (p > 0.05). Conclusion In CREST, four angiographic variables were found to increase risk for stroke during and within 30 days after CAS: severe distal tortuosity, sequential lesions, lesion length > 20 mm, and narrow mouth ulcer. Angiographic characteristics beyond percent stenosis may help in patient selection in terms of best revascularization option.

Emblica officinalis, commonly known as Amla belongs to family Euphorbiaceae, is widely used for medicinal purposes in Indian traditional system of medicine (Unani, Ayurveda, and Siddha). It is well known that all parts of Amla are useful in the treatment of various diseases. Various studies on Amla suggest that it has antiviral, antibacterial, and antifungal actions. It is one among those traditional plants, which have a long history of usage as a fruit and remedy. It is amazingly effective as natural antiaging drug. It is a very effectual plant in the treatment of acidity and peptic ulcer. According to Unani literature, it possesses nutritional as well as therapeutic values, and thus, it is one of the herbal nutraceuticals. Modern literature and research studies also prove its medicinal importance. Its fruit is used traditionally as an antioxidant, immunomodulator, antipyretic, analgesic, antitussive, anticancer, and gastroprotective. It is also useful in diarrhea, dysentery, diabetes, fever, headache, mouth ulcer, hair growth, scurvy, and constipation. Phytochemical studies on amla disclosed major chemical constituents including tannins, alkaloids, polyphenol, fatty acid, glycosides, phosphatides, vitamins, and minerals. Gallic acid, ellagic acid, phyllembein, and ascorbic acid are also found to be biologically effective. Various reports show the presence of catechol, β-carotene, flavonoids, pyrogallol, superoxide, and dismutase enzyme in Emblica fruit. In this review, an attempt is made to explore the complete information of E. officinalis including its phytochemistry and pharmacology.

Mukha (mouth) is one of the important parts of body and it is exposed to various risk factors due to the bad habits like tobacco chewing and smoking etc. Mukhapaka is pathological condition related to Mukha which occurs due to the simple cut, eating hot food items, unhygienic cleaning, vitamins deficiencies and adverse reaction of antibiotics. Lalasrava, Glani, Dugdha Dwesha, Vyatha and Nasa Shwasa etc. are general symptoms of Mukhapaka. As per ayurveda Mukha Rogas is Pittaja Nanatmaja and Rakta Pradoshaja Vikara and around 20% of population is suffering with this problem. Ayurveda described various treatment modalities for the management of Mukhapaka; Ashwattha Choorna Lepa is one of them, present article established efficacy of Ashwattha Choorna Lepa in Mukhapaka. The study concluded that Lepan-Karma with ayurveda formulation Ashwattha Choorna offers beneficial effects in mouth ulcer.
