Deficiencies in our understanding of the hydro-ecology of several native Australian fish: a rapid evidence synthesis

Environmental flows aim to mitigate the impacts of modification of riverine flow regimes by restoring components of the natural flow regime. Explicit evidence-based cause–effect relationships between flow regimes and fish responses are required for defensible flow recommendations. However, flow recommendations are typically based on expert opinion rather than a rigorous test of the available evidence. We used rapid evidence synthesis to systematically review the literature on responses of several native Australian fish to flow-related and other important environmental variables, and tested 13 explicit hypotheses. The hypotheses were related to the condition, reproduction and survival of pygmy perches, carp gudgeons, blackfish and flat-headed gudgeons. The evidence was insufficient to reach strong conclusions for all but three of our hypotheses. Late-spring high flows are associated with increased survival of pygmy perches and carp gudgeons, and exotic fish are associated with decreased survival of these fishes. The evidence that warmer spring water temperatures enhance their reproduction was inconsistent. The dearth of evidence to support or refute most hypotheses points to substantial gaps in our understanding of hydro-ecology of these native Australian fish, highlighting the need for targeted monitoring and research to further understand key flow relationships for these fish species.

Abstract. Worldwide, flow regimes are being modified by various anthropogenic impacts and climate change induces an additional risk. Rising temperatures, declining snow cover and changing precipitation patterns will interact differently at different locations. Consequently, in distinct climate zones, unequal consequences can be expected in matters of water stress, flood risk, water quality, and food security. In particular, river ecosystems and their vital ecosystem services will be compromised as their species richness and composition have evolved over long time under natural flow conditions. This study aims at evaluating the exclusive impacts of climate change on river flow regimes in Europe. Various flow characteristics are taken into consideration and diverse dynamics are identified for each distinct climate zone in Europe. In order to simulate present-day natural flow regimes and future flow regimes under climate change, the global hydrology model WaterGAP3 is applied. All calculations for current and future conditions (2050s) are carried out on a 5' × 5' European grid. To address uncertainty, bias-corrected climate forcing data of three different global climate models are used to drive WaterGAP3. Finally, the hydrological alterations of different flow characteristics are quantified by the Indicators of Hydrological Alteration approach. Results of our analysis indicate that on the European scale, climate change can be expected to modify flow regimes remarkably. This is especially the case in the Mediterranean (due to drier conditions with reduced precipitation across the year) and in the boreal climate zone (due to reduced snowmelt, increased precipitation, and strong temperature rises). In the temperate climate zone, impacts increase from oceanic to continental. Regarding single flow characteristics, strongest impacts on timing were found for the boreal climate zone. This applies for both high and low flows. Flow magnitudes, in turn, will be predominantly altered in the Mediterranean but also in the Northern climates. At the end of this study, typical future flow regimes under climate change are illustrated for each climate zone.

Low-grade inflammation and oxidative stress underlie chronic osteoarthritis. Although best-practice guidelines for osteoarthritis emphasize self-management including weight control and exercise, the role of lifestyle behavior change to address chronic low-grade inflammation has not been a focus of first-line management. This paper synthesizes the literature that supports the idea in which the Western diet and inactivity are proinflammatory, whereas a plant-based diet and activity are anti-inflammatory, and that low-grade inflammation and oxidative stress underlying osteoarthritis often coexist with lifestyle-related risk factors and conditions. We provide evidence-informed recommendations on how lifestyle behavior change can be integrated into “first-line” osteoarthritis management through teamwork and targeted evidence-based interventions. Healthy living can be exploited to reduce inflammation, oxidative stress, and related pain and disability and improve patients’ overall health. This approach aligns with evidence-based best practice and holds the promise of eliminating or reducing chronic low-grade inflammation, attenuating disease progression, reducing weight, maximizing health by minimizing a patient’s risk or manifestations of other lifestyle-related conditions hallmarked by chronic low-grade inflammation, and reducing the need for medications and surgery. This approach provides an informed cost effective basis for prevention, potential reversal, and management of signs and symptoms of chronic osteoarthritis and has implications for research paradigms in osteoarthritis.

Abstract. Extreme low and high flows can have negative economic, social, and ecological effects and are expected to become more severe in many regions due to climate change. Besides low and high flows, the whole flow regime, i.e., annual hydrograph comprised of monthly mean flows, is subject to changes. Knowledge on future changes in flow regimes is important since regimes contain information on both extremes and conditions prior to the dry and wet seasons. Changes in individual low- and high-flow characteristics as well as flow regimes under mean conditions have been thoroughly studied. In contrast, little is known about changes in extreme flow regimes. We here propose two methods for the estimation of extreme flow regimes and apply them to simulated discharge time series for future climate conditions in Switzerland. The first method relies on frequency analysis performed on annual flow duration curves. The second approach performs frequency analysis of the discharge sums of a large set of stochastically generated annual hydrographs. Both approaches were found to produce similar 100-year regime estimates when applied to a data set of 19 hydrological regions in Switzerland. Our results show that changes in both extreme low- and high-flow regimes for rainfall-dominated regions are distinct from those in melt-dominated regions. In rainfall-dominated regions, the minimum discharge of low-flow regimes decreases by up to 50 %, whilst the reduction is 25 % for high-flow regimes. In contrast, the maximum discharge of low- and high-flow regimes increases by up to 50 %. In melt-dominated regions, the changes point in the other direction than those in rainfall-dominated regions. The minimum and maximum discharges of extreme regimes increase by up to 100 % and decrease by less than 50 %, respectively. Our findings provide guidance in water resource planning and management and the extreme regime estimates are a valuable basis for climate impact studies. Abstract STUDY QUESTION What is the recommended assessment and management of infertile women with polycystic ovary syndrome (PCOS), based on the best available evidence, clinical expertize and consumer preference? SUMMARY ANSWER International evidence-based guidelines, including 44 recommendations and practice points, addressed prioritized questions to promote consistent, evidence-based care and improve the experience and health outcomes of infertile women with PCOS. WHAT IS KNOWN ALREADY Previous guidelines on PCOS lacked rigorous evidence-based processes, failed to engage consumer and multidisciplinary perspectives or were outdated. The assessment and management of infertile women with PCOS are inconsistent. The needs of women with PCOS are not being adequately met and evidence practice gaps persist. PARTICIPANTS/MATERIALS, SETTING, METHODS Governance included a six continent international advisory and a project board, a multidisciplinary international guideline development group (GDG), consumer and translation committees. Extensive health professional and consumer engagement informed the guideline scope and priorities. The engaged international society-nominated panel included endocrinology, gynaecology, reproductive endocrinology, obstetrics, public health and other experts, alongside consumers, project management, evidence synthesis and translation experts. Thirty-seven societies and organizations covering 71 countries engaged in the process. Extensive online communication and two face-to-face meetings over 15 months addressed 19 prioritized clinical questions involving nine evidence-based reviews and 10 narrative reviews. Evidence-based recommendations (EBRs) were formulated prior to consensus voting within the guideline panel. STUDY DESIGN, SIZE, DURATION International evidence-based guideline development engaged professional societies and consumer organizations with multidisciplinary experts and women with PCOS directly involved at all stages. A (AGREE) II-compliant processes were followed, with extensive evidence synthesis. The Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) framework was applied across evidence quality, desirable and undesirable consequences, feasibility, acceptability, cost, implementation and ultimately recommendation strength. The guideline was peer-reviewed by special interest groups across our partner and collaborating societies and consumer organizations, was independently assessed against AGREE II criteria and underwent methodological review. This guideline was approved by all members of the GDG and has been approved by the NHMRC. MAIN RESULTS AND THE ROLE OF CHANCE The quality of evidence (QOE) for the EBRs in the assessment and management of infertility in PCOS included very low (n = 1), low (n = 9) and moderate (n = 4) quality with no EBRs based on high-quality evidence. The guideline provides 14 EBRs, 10 clinical consensus recommendations (CCRs) and 20 clinical practice points on the assessment and management of infertility in PCOS. Key changes in this guideline include emphasizing evidence-based fertility therapy, including cheaper and safer fertility management. LIMITATIONS, REASONS FOR CAUTION Overall evidence is generally of low to moderate quality, requiring significantly greater research in this neglected, yet common condition. Regional health systems vary and a process for adaptation of this guideline is provided. WIDER IMPLICATIONS OF THE FINDINGS The international guideline for the assessment and management of infertility in PCOS provides clinicians with clear advice on best practice based on the best available evidence, expert multidisciplinary input and consumer preferences. Research recommendations have been generated and a comprehensive multifaceted dissemination and translation program supports the guideline with an integrated evaluation program. STUDY FUNDING/COMPETING INTEREST(S) The guideline was primarily funded by the Australian National Health and Medical Research Council of Australia (NHMRC) supported by a partnership with ESHRE and the American Society for Reproductive Medicine (ASRM). GDG members did not receive payment. Travel expenses were covered by the sponsoring organizations. Disclosures of conflicts of interest were declared at the outset and updated throughout the guideline process, aligned with NHMRC guideline processes. Conservative treatment is the basis for diabetic foot ulcer (DFU) management, whereas surgical treatment is usually reserved for patients with failed, recurrent, or nonresponsive infected wounds. However, many reports demonstrated good to excellent results following surgery. Evidence synthesis on surgical offloading techniques and clear guidelines regarding the timing of surgery are lacking. The present study aimed to investigate the evidence behind surgical offloading techniques and propose a cutoff time for surgical indication following failed conservative treatment of neuropathic diabetic forefoot ulcers. Electronic databases were searched from inception to identify the best evidence level articles related to non-vascular surgical treatment of DFUs, such as metatarsal head resection, resection arthroplasty, metatarsal osteotomy, Achilles tendon lengthening, gastrocnemius recession, and flexor tenotomy, that have been employed for managing DFUs. Based on the highest level of evidence available, surgery was found to generate better values than standard conservative care for all outcomes except for the transfer rate. In particular, surgical bony offloading procedures demonstrated significantly better outcomes than standard conservative nonsurgical care in terms of higher healing rates, shorter healing durations, and lower recurrence rates. Moreover, 96% of DFUs healed in <1 month following surgical bony offloading, whereas 68% of ulcers healed within 3 months after standard care. The findings could challenge the classical guidelines of DFU management. This evidence-based review indicates that surgical offloading could be used more often and be proposed earlier during the course of ulcer management. The results imply that a period of 12 weeks could be considered a reasonable cutoff value to consider surgical treatment for patients with nonhealing DFUs.
