This special issue of Sexual Health presents a collection of articles that addresses issues facing transgender individuals that are particularly challenging. Issues covered range from sexual health education in schools, the need for accurate epidemiological measurements, the perils of inaccurate diagnostic labels of transgender children, legal issues, the disproportionately high prevalence of HIV and sexually transmissible infections (STIs), and the role of primary care. We need to think critically, constructively and compassionately about transgender people. Particularly, we must look beyond the hype and objectively consider the evidence, without forgetting the people who are trying to cope with feelings that may be causing them great distress.

The goal of research is to understand our world and the people in it. This understanding comes through a variety of methods and strategies that each uncover a different layer of knowledge that can contribute to this overall understanding. As researchers, we are much like a young child seeking to understand their surroundings by first touching a new toy, then lifting it, then shaking it, then even tasting it! Each of these strategies tells the child a little more about the object. Similarly, as we seek to understand and influence our world, we need the knowledge gained from a wide variety of scholarly methods. The articles that make up this special issue are intended to bring that mission and goal closer together.

Grief is a natural and self-limited process of adaptation to a new reality that arises after a significant loss, real or perceived, with a wide range of manifestations that produce an impact on the health of the mourner. Objective: To know and identify, broken down by diagnostic gradient, the prevalence of mourning in the Canary Islands, as well as the needs of care before the diagnosis of mourning of the mourners. Results: Significantly, more women than men are identified in the three diagnostic labels under study. Patients with bereavement complications previously presented depression, anxiety, and risk of loneliness. The result of the pattern assessment is paradoxical. Conclusion: Grief is a multidimensional phenomenon that can hardly be assessed from a reductionist tool such as functional patterns. The high prevalence of complications in women’s grief should be studied in-depth, associating it with their previous burdens of care and vulnerability. The association between loneliness and complicated grief is evident, and the role of community nurses in these patients may be essential. Further research is needed on the care needs of bereaved persons, both complicated and uncomplicated, as well as on their predictive and protective factors.

Background Pre-exposure prophylaxis (PrEP) was introduced in Sexual Health Services of the Welsh National Health Service (NHS Wales) in July 2017 as a 3-year pilot service. Methods: Data were collected through the pre-existing Sexual Health in Wales Surveillance System, to which codes were added to capture PrEP eligibility, outcome of offer of PrEP, reasons for declining and adherence. Eligibility categories were defined based on nationally agreed criteria: men who have sex with men (MSM) and transgender people at high risk of HIV acquisition; partners of HIV-positive individuals not known to be virally suppressed; and heterosexuals reporting condomless intercourse with a HIV-positive individual not known to be virally suppressed. Results: During the first 6 months, 516 people were eligible, 96% of which were MSM. Overall, 57% of those eligible (296/516) started PrEP. Reasons for declining PrEP were given by 88 (56%) of 157 people; 50 (57%) of whom did not believe themselves to be at risk. Of the available adherence assessments, 89% considered that all risk episodes had been covered. Persistence at 3 months was assessed for 141 people, of which 93 (66%) were still using PrEP. There were no HIV diagnoses in people taking PrEP during the first 6 months. Twenty-nine people were diagnosed with 37 episodes of sexually transmissible infections (STIs) while on PrEP. STI incidence was 105.7 per 100 person-years. Conclusions: The early trend indicates that implementation of PrEP is progressing as planned, and the service has been utilised by clients. This analysis can help refine implementation, inform planning and research around uptake, use and effect in Wales and internationally.

Understanding the role of youth workers and promoting an accurate representation of these professionals and their work requires a focus on current research, practice, and policies that capture the challenges and opportunities of the youth development workforce. An overview of this special issue of the Journal of Youth Development examines efforts to support individual youth workers, strategies organizations use to prepare and assist youth workers, and system approaches to building the capacity of the workforce in key areas such as quality and social emotional learning, and concludes with an encapsulation of interviews with 10 key leaders nationwide. They identified the overarching task as one of identification of the elements that can make the field cohesive across the different settings and programs, while supporting youth in their growth through experiences that are rooted in the science of learning and development. Finally, this special issue offers the opportunity to better understand the youth workers, examine different types of professional development pathways, explore the role that systems can play in support of these workers, and reflect on the challenges and opportunities raised by key leaders in the profession.

Background Young men are vulnerable when it comes to sexual health. They attend the general practitioner (GP) less often than females and are less likely to be offered testing for sexually transmissible infections. Access to accurate health information and education is a cornerstone of primary prevention, yet we know very little about how, where and why young people obtain information about sexual health. Methods: One-on-one semi-structured interviews were conducted with 35 male students aged 16–19 years from two Victorian educational institutions for trade skills until data saturation was reached. Interviews were audio-recorded, transcribed and thematically analysed. Results: The young men were poorly informed about sexual health. Their existing knowledge mainly came from school-based sexual health education, which while valued, was generally poorly recalled and provided only a narrow scope of physiological information. Young men seek sexual health information from various sources including family, the Internet, friends and pornography, with information from the latter three sources perceived as unreliable. GPs were seen as a source of trust-worthy information but were not accessed for this purpose due to embarrassment. Young men preferred the GP to initiate such conversations. A desire for privacy and avoidance of embarrassment heavily influenced young men’s preferences and behaviours in relation to sexual health information seeking. Conclusions: The current available sources of sexual health information for young men are failing to meet their needs. Results identify potential improvements to school-based sexual education and online resources, and describe a need for innovative technology-based sources of sexual health education.

Purpose The purpose of this paper is to introduce the special issue about entrepreneurial universities. The paper focuses on the reasons for their importance in global communities. Design/methodology/approach The literature about entrepreneurial universities is reviewed with a focus on the role of communities, people and places. Findings Entrepreneurial universities need to consider the people in society in terms of how they relate to learning, education and teaching about new ideas and business practices. Research limitations/implications It is important in the increasingly connected global economy that universities be seen as the drivers of the entrepreneurial society, which helps place people at the centre of educational communities. Practical implications This paper has suggestions for educational managers and business people interested in connecting with entrepreneurial universities and their place in the society. Originality/value This value of this paper is that it includes the main ideas from the special issue in terms of how entrepreneurial universities are fostering a connected society that places people, places and communities at the forefront of the change.

Background: Previous studies have concluded that transgender people are a marginalised and stigmatised group, with high rates of sexually transmissible infections (STI), sex work, injecting drug use and multiple sexual partners. To our knowledge, this is the first study in Australia to focus on the sexual behaviour and sexual health needs of transgender people attending an urban sexual health clinic. Methods: A retrospective case note review was undertaken of the transgender attendees of the Sydney Sexual Health Centre between 1990 and 2006. Demographics, gender characteristics, risk behaviours, sexual health morbidity, psychosocial information and other significant features of the transgender population were assessed. Results: Forty clients were identified as transgender, of whom 36 (90%) were male-to-female and four (10%) were female-to-male. Seventeen (43%) had a history of sex work, 16 (40%) had injected drugs, 14 (35%) had had unprotected anal or vaginal sex in the past 3 months. Twenty (50%) clients had histories of a STI, including three (7.5%) that were HIV positive, and two were co-infected with hepatitis C. Genital warts and chlamydia were the most common diagnoses made during the study period. Physical examination was inadequately documented in 53% of first visits. Psychosocial morbidity was common. Conclusions: Transgender clients presented infrequently at this clinic. Although half reported few risks, the other half reported multiple risk behaviours and had most STI. These findings suggest that there needs to be improved sexual health service for transgender clients at our clinic.

Background: Sexually transmissible infections (STIs) among young Australians increased dramatically between 1997 and 2007 with rates of chlamydia increasing by 528% and rates of gonorrhoea by 169% among 15–19 year olds. High notification rates of STIs and teenage pregnancy point to the need to investigate sexual health education (SHE) in Australian schools. This first quantitative study investigated the attitudes and experiences of parents to SHE in Australian schools. Methods: One hundred and seventeen (117) Australian parents were recruited through purposive sampling and snowballing methods to complete an online questionnaire in 2007. Results: Most respondents (97.4%) support SHE in schools and 95.7% advocate schools and parents sharing responsibility. A majority (82.9%) believe SHE should begin in primary school with discrepancy as to when specific topics should be introduced. There is consensus for a comprehensive curriculum, including topics potentially seen as controversial such as ‘masturbation’. ‘Abstinence’ was chosen by 15.4% of parents to not be included in SHE curricula. Most parents rate the SHE their children have received in school as ‘fair’ in quality, and want access to resources to help them educate their children, including workshops at schools, information about school SHE, literature, and trained sexual health educators. Conclusions: Parents generally support SHE in schools. They want programs to begin in primary school. They consider the current school programs to be ‘fair’ in quality. Parents also point out the need for a more comprehensive curriculum and they want to be involved with schools in the development and delivery of SHE.
