Abstract Researchers’ ability to measure the impact of adult day centers (ADCs) on participants’ health has been hampered by a lack of large-scale data. We examined categories of data ADCs across the United States are collecting related to patients’ health and health outcomes with the idea of developing a future national cohort of centers. We distributed an electronic survey to ADCs in 50-states on current data collection efforts. Forty states were represented (N=250). Only 32% of ADCs collect patient level data for research and analysis. Vital signs, nutritional risk, falls, and activities of daily living data were most commonly collected. However, validated assessment tools were used in less than 50% of cases. Researchers’ ability to pool data on clinical outcomes among ADC users is limited by lack of data collection and use of uniform outcome measures across ADCs. Standardizing data collection is critical to strengthening ADC programs and demonstrating their effectiveness.

An understanding of adult day service centers’ (ADC) impacts on clients’ health and well-being has been hampered by a lack of large-scale data. Standardizing data collection is critical to strengthening ADC programs, demonstrating their effectiveness, and enabling them to leverage additional funding streams beyond Medicaid. We distributed an electronic survey on current data collection efforts to ADCs nationally to determine categories of data ADCs are collecting related to clients’ health. In our sample ( N = 248), only 32% of ADCs collected patient-level data for research and analysis—most commonly on activities of daily living, cognition, nutrition, and caregiver strain. However, validated assessment tools were used in less than 50% of the cases. ADCs are willing to collect data: More than 70% reported a willingness to participate in future studies. National studies piloting data collection protocols with uniform outcome measures are needed to advance the understanding of ADCs’ capabilities and impacts.

This book provides an account of how and why routine interactions break down and how such situational breakdowns lead to protest violence and other types of surprising social outcomes. It takes a close-up look at the dynamic processes of how situations unfold and compares their role to that of motivations, strategies, and other contextual factors. The book discusses factors that can draw us into violent situations and describes how and why we make uncommon individual and collective decisions. Covering different types of surprise outcomes from protest marches and uprisings turning violent to robbers failing to rob a store at gunpoint, it shows how unfolding situations can override our motivations and strategies and how emotions and culture, as well as rational thinking, still play a part in these events. The first chapters study protest violence in Germany and the United States from 1960 until 2010, taking a detailed look at what happens between the start of a protest and the eruption of violence or its peaceful conclusion. They compare the impact of such dynamics to the role of police strategies and culture, protesters’ claims and violent motivations, the black bloc and agents provocateurs. The analysis shows how violence is triggered, what determines its intensity, and which measures can avoid its outbreak. The book explores whether we find similar situational patterns leading to surprising outcomes in other types of small- and large-scale events: uprisings turning violent, such as Ferguson in 2014 and Baltimore in 2015, and failed armed store robberies.

Background: The first shelter-in-place (SIP) order in the United States was issued across six counties in the San Francisco Bay Area to reduce the impact of COVID-19 on critical care resources. We sought to assess the impact of this large-scale intervention on emergency departments (ED) in Marin County, California. Methods: We conducted a retrospective descriptive and trend analysis of all ED visits in Marin County, California from January 1, 2018 to May 4, 2020 to quantify the temporal dynamics of ED utilization before and after the March 17, 2020 SIP order. Results: The average number of ED visits per day decreased by 52.3% following the SIP order compared to corresponding time periods in 2018 and 2019. Both respiratory and non-respiratory visits declined, but this negative trend was most pronounced for non-respiratory admissions. Conclusions: The first SIP order to be issued in the United States in response to COVID-19 was associated with a significant reduction in ED utilization in Marin County.

Abstract This study tackles the contribution of soil moisture feedback to the development of extreme summer precipitation anomalies over the conterminous United States using a regional climate model. The model performs well in reproducing both the mean climate and extremes associated with drought and flood. A large set of experiments using the model are conducted that involve swapped initial soil moisture between flood and drought years using the 1988 and 2012 droughts and 1993 flood as examples. The starting time of these experiments includes 1 May (late spring) and 1 June (early summer). For all three years, the impact of 1 May soil moisture swapping is much weaker than the 1 June soil moisture swapping. In 1988 and 2012, replacing the 1 June soil moisture with that from 1993 reduces both the spatial extent and the severity of the simulated summer drought and heat. The impact is especially strong in 2012. In 1993, however, replacing the 1 June soil moisture with that from 1988 has little impact on precipitation. The contribution of soil moisture feedback to summer extremes is larger in 2012 than in 1988 and 1993. This may be because of the presence of strong anomalies in large-scale forcing in 1988 and 1993 that prohibit or favor precipitation, and the lack of such in 2012. This study demonstrates how the contribution of land–atmosphere feedback to the development of seasonal climate anomalies may vary from year to year and highlights its importance in the 2012 drought.

<p>Cultures of Light is set within a period that stretches from the late nineteenth to the mid-twentieth century in the United States, an era in which nearly every aspect of American life was impacted to a lesser or greater degree by the introduction, distribution and integration of electric power and light. By no means attempting to comprehensively examine the impact and effects of this expansive transformation, this thesis has a narrow but meaningful target, defined by key intersections of electric lighting and American culture. Primarily concerned with the investigation of culturally bound ideas and practices as mediated through electric light and its applications, my thesis is focused on particular instances of this interplay. These include its role in supporting nationalizing narratives and agendas through large-scale demonstrations at world’s fairs and exhibitions, in the search for and expression of modernism and its variations in the United States. Similarly electricity and electric light throughout the better part of the twentieth century was scaled to the level of the individual through a number of mechanisms and narratives. Most prominently the electric light industry employed gendered discourses, practices and beliefs in their efforts to grow the market, calling upon the assistance of a host of cultural influencers, from movie stars to architects to interior designers, instigating a renegotiation of established approaches to the design of architecture and the visual environment. Connecting common themes and persistent concerns across these seemingly disparate subject areas through the examination of cultural beliefs, practices, rituals and traditions, Cultures of Light seeks to illustrate the deep and lasting significance of electric light within American society in the twentieth century.</p>

The outbreak of COVID-19 from late 2019 not only threatens the health and lives of humankind but impacts public policies, economic activities, and human behavior patterns significantly. To understand the impact and better prepare for future outbreaks, socioeconomic factors play significant roles in (1) determinant analysis with health care, environmental exposure and health behavior; (2) human mobility analyses driven by policies; (3) economic pressure and recovery analyses for decision making; and (4) short to long term social impact analysis for equity, justice and diversity. To support these analyses for rapid impact responses, state level socioeconomic factors for the United States of America (USA) are collected and integrated into topic-based indicators, including (1) the daily quantitative policy stringency index; (2) dynamic economic indices with multiple time frequency of GDP, international trade, personal income, employment, the housing market, and others; (3) the socioeconomic determinant baseline of the demographic, housing financial situation and medical resources. This paper introduces the measurements and metadata of relevant socioeconomic data collection, along with the sharing platform, data warehouse framework and quality control strategies. Different from existing COVID-19 related data products, this collection recognized the geospatial and dynamic factor as essential dimensions of epidemiologic research and scaled down the spatial resolution of socioeconomic data collection from country level to state level of the USA with a standard data format and high quality.

This paper investigates the impact of the emergence of large-scale enterprises on industrial structure in America in the mid-nineteenth century and concludes that their impact was ambiguous. In cottons and irons, average scale increased dramatically, but inequality in the size distribution of plants declined and economic concentration showed no clear trend. In other industries, changes in average scale were much smaller and inequality increased, but again there was no clear trend in concentration.

Abstract The NOAA Science Advisory Board appointed a task force to prepare a white paper on the use of observing system simulation experiments (OSSEs). Considering the importance and timeliness of this topic and based on this white paper, here we briefly review the use of OSSEs in the United States, discuss their values and limitations, and develop five recommendations for moving forward: national coordination of relevant research efforts, acceleration of OSSE development for Earth system models, consideration of the potential impact on OSSEs of deficiencies in the current data assimilation and prediction system, innovative and new applications of OSSEs, and extension of OSSEs to societal impacts. OSSEs can be complemented by calculations of forecast sensitivity to observations, which simultaneously evaluate the impact of different observation types in a forecast model system.

Introduction: The drug pharmacovigilance system in Japan is similar to those in the European Union (EU) and the United States. As a unique Japanese pharmacovigilance program, postmarketing all-case surveillance (PMACS) is required. PMACS plays a key role for postmarketing activities, but there are challenges that place much burden on PMACS conduct. This study investigates the impact of PMACS on postmarketing activities in Japan and proposes its potential improvement. This study also seeks the possibility to expand PMACS beyond Japan. Materials and Methods: Reexamination reports issued from 2017 to 2019 were identified in September 2020 by searching ‘reexamination report’ and ‘201701’ to ‘201912’ on the Pharmaceuticals and Medical Devices Agency website. The corresponding Package Insert (PI) change orders and premarketing review reports were also identified. Reviewing these regulatory documents allowed for investigation of the PMACS impact on postmarketing activities. Results: More than half (57%) of the drugs with PMACS had ‘Limited dosing experience in Japan’ as a reason for the PMACS requirement. As a safety measure, no PI change orders were imposed on 33% and 28% of drugs with and without PMACS, respectively. The means of the number of PI change orders were 2.23 and 2.14 for drugs with and without PMACS, respectively. There were no reexamination reports mentioning any concerns related to efficacy. Discussion and Conclusion: PMACS should not be imposed only because of limited dosing experience in Japan at the premarketing stage. Rather, PMACS should focus on (1) collection of safety data (not efficacy), (2) necessity of distribution control, and/or (3) collection of case details for drugs with a limited treated population. PMACS also has the potential to be utilized in the EU and the United States, as their regulatory frameworks are acceptable for PMACS. Naglazyme (galsulfase) is a case where the PMACS-like studies have been required in each region. Plain Language Summary Effectiveness of data collection for all patients who receive a new drug as a safety measure in Japan Introduction: In Japan, a drug company is obligated to conduct data collection after a new drug launch as an approval condition. The obligation is a unique Japanese requirement where a company must collect data from all patients receiving the drug in Japan in cooperation with hospitals. This is expected to contribute to intensive data collection and better drug distribution control and could potentially be useful in countries beyond Japan. However, no clear criteria have been established for decision making, despite the significant burden for companies and hospitals. Therefore, this study aimed to investigate the impact of the obligation on safety measures and efficacy data collection and propose a potentially improved drug scope to impose the obligation. Materials and Methods: Reexamination of reports issued by the Pharmaceuticals and Medical Devices Agency between 2017–2019. Results: More than half (57%) of the included drugs had ‘Limited dosing experience in Japan’ as a reason for the obligation being required. However, regulatory order to change drug label, an action based on safety signal identification, was imposed on 33% and 28% of drugs with and without the obligation, respectively. The means of the number of the label change orders were 2.23 and 2.14 for drugs with and without obligation, respectively. Meanwhile, some drugs were highlighted as potential factors for better application of the obligation. Conclusion: According to these results, the obligation should be imposed on a limited number of drugs by focusing not on dosing experience in Japan but on safety (not efficacy) data collection, necessity of distribution control, and/or collection of case details for drugs with a limited treated population. The obligation also has the potential to be utilized in the EU and the United States, as their regulatory frameworks are acceptable for the obligation.
