Abstract MindfulGarden (MG) is a digital device resembling a flat screen TV, with touchless sensors that react to voice and motion. In this study 13 long-term care home residents aged 74-100 exhibiting Behavioural and Psychological Symptoms of Dementia (BPSD) were randomized to treatment and control groups. On days 1-3 the treatment group received usual care plus exposure to MG during morning and evening care - events well documented to be problematic for residents and care staff; controls received usual care only. On day 4 both groups were exposed to MG with verbal cueing. A 26-item checklist was used to record frequency and types of disruptive BPSD exhibited; care duration was recorded in minutes. There was a trend toward reduction of BPSD and duration of care during morning care. Findings suggest that verbal cueing may be important for successful implementation of MG in calming residents with dementia during routine care.

Background: Dental schools are considered to be a very stressful environment; the stress levels of dental students are higher than those of the general population. The aim of this study was to assess the level of stress among dental students while performing specific dental procedures. Methods: A survey was conducted among 257 participants. We used an original questionnaire, which consisted of 14 questions assigned to three categories: I—Diagnosis, II—Caries Treatment, and III—Endodontic Treatment. Each participant marked their perceived level of stress during the performed dental treatment procedures. The scale included values of 0–6, where 0 indicates no stress, while 6 indicates high stress. Results: Third- (p=0.006) and fourth-year (p=0.009) women were characterized by a higher level of perceived stress during dental procedures related to caries treatment. Caries treatment procedures were the most stressful for 18.3% of third-year students, 4.3% of fourth-year students, and 3.2% of fifth-year students. Furthermore, 63.4% of third-year students, 47.3% of fourth-year students, and 17.2% of fifth-year students indicated that they felt a high level of stress when performing endodontic procedures. Conclusion: Third- and fourth-year female students are characterized by a higher level of stress during caries and endodontic treatment procedures. The most stressful treatments for participants were endodontic treatment procedures.

Background: The prosthetic preparation of the teeth for ceramic laminate veneers has to follow the minimally invasive concept brought by the modern Conservative Dentistry and Prosthodontics. However, during the cementation phase under the rubber dam, the loss of the esthetics landmarks could lead to errors in the future positioning of the laminate veneers. Methods: In this article the authors show an accurate operative prosthetic protocol using different fine intraoperative maneuvers and tricks for the realization of ceramic laminates in order to solve the problems of the cementation phase. Results: The treatment of the anterior sector of the upper maxilla with porcelain laminate veneers was realized in a 30 years old woman with aesthetic issues. Conclusion: Different fine intraoperative maneuvers and tricks during teeth preparation, master impression and rubber dam positioning could reduce errors occurring in the cementation phase and increase the predictability of the results.

The COVID-19 pandemic has posed a serious challenge to the lactating women to practice optimal infant and young child feeding. Although international and national agencies developed appropriate evidence-based guidelines early in the pandemic, availability of this information to the mothers and their caregivers needs to be enhanced. This becomes important in view of apprehension about the risk of a decline in breastfeeding practices during the pandemic due to various factors. Any decrease in the breastfeeding rates may lead to increased childhood morbidity, mortality, and malnutrition. This article provides a glimpse of available evidence-based guidelines on breastfeeding by Covid-19 positive mothers and attempts by the baby food industry to exploit the situation by promoting their products. The article also deals with infection prevention and control measures to be observed by the mother while caring and breastfeeding her baby and other action required to protect breastfeeding from commercial influence.

Social care is the broad term for the support provided to people living at home and in care homes. Major changes have taken place to this sector in the past few years and this chapter describes the key policy developments that have impacted upon the kind of support provided to older people with mental health problems and how that support is funded. The policy of personalization is intended to increase choice and control, but as yet it is unclear whether this will lead to improved outcomes in terms of quality of life or independence. Some long-standing issues, such as the high prevalence of people with unidentified mental health problems in long-term care, continue to provide challenges for organizations providing social care support.

Background Nurses certified in wound, ostomy, and continence monitored an increasing incidence of hospital-acquired pressure injury of the nares due to medical devices, specifically nasogastric tubes, in a metropolitan hospital. A majority of these pressure injuries occurred in patients in the intensive care unit. The organization lacked formal guidelines for preventing such injuries. Objective To decrease the incidence of nasogastric tube–related hospital-acquired pressure injury. Methods The organization’s process improvement model, comprising steps to define, measure, analyze, improve, and control, guided the project. The incidence rate of nasogastric tube–related hospital-acquired pressure injury before the intervention was determined for calendar year 2015 and compared with data obtained after the intervention, for calendar year 2016. An interprofessional team created, implemented, and evaluated the effectiveness of evidence-based guidelines and surveillance strategies for preventing nasogastric tube–related hospital-acquired pressure injury. The team implemented guidelines using the simple mnemonic “CLEAN”: correct tube position, stabilize tube, evaluate area under/near tube, alleviate pressure, note date and time. Results The incidence rate of nasogastric tube–related hospital-acquired pressure injury (0.13 per 1000 patient days in 2015) decreased 100% (0.0 per 1000 patient days in 2016) after the guidelines were implemented in the organization. This rate was sustained for a full year, after which it increased slightly because temporary and new staff lacked knowledge of the guidelines. Conclusions The creation and implementation of clear and specific guidelines for assessing and securing nasogastric tubes successfully reduced nasogastric tube–related hospital-acquired pressure injury.

The article presents the results of the preliminary survey and pilot application of the methodological toolkit for the improvement of certain competencies, which are part of the soft skills of professionals in the long-term care sector in Bulgaria. Based on the survey the authors define 2 target competencies, namely conflict management and empathic interaction. These skills are considered a part of the scope of emotional intelligence and its constituent dimensions and authors use Daniel Goleman’s ability-based model as a base for their conceptual frame and theoretical explanations. In order to present their thesis, the authors perform a soft skill analysis of the long-term care sector; define the construct emotional intelligence and justify their choice of a theoretical model for the subsequent survey. On this basis, they develop a training design for the development of these skills and present the results achieved through its pilot application. The study includes 62 participants randomly divided into a test and control groups. The level of targeted skills is measured (before and after) and the authors report a significant increase in those skills (empathic interaction, net effect 1.87; conflict management, net effect 1.75). In conclusion, the authors open a discussion on the importance of emotional management as an essential part of any profession related to health care and underline its importance for the quality of caregiving services.
