Daily-level effects of alcohol, marijuana, and simultaneous use on young adults’ perceived sleep health

Abstract Study Objectives Simultaneous alcohol and marijuana (SAM) use is increasingly prevalent among young adults but has adverse health consequences. The current study examined daily-level associations between perceived sleep health and SAM use, relative to non-substance-use days and alcohol- or marijuana-only days. We also estimated linear associations between alcohol/marijuana use and perceived sleep health and explored whether effects were moderated by combined use of alcohol and marijuana. Methods A community sample of SAM-using young adults (N=409; Mage=21.61, SD=2.17; 50.9% female; 48.2% White; 48.9% college students) completed twice-daily surveys for five 14-day sampling bursts. Daily measurements assessed substance use and perceived sleep health in terms of subjective sleep quality, negative impact of sleep on functioning, and symptoms of insomnia. Results Multilevel models indicated that, relative to non-substance-use days, participants reported poorer perceived sleep health on alcohol-only days, better perceived sleep health on marijuana-only days, and mixed evidence regarding SAM use (i.e., fewer perceived symptoms of insomnia, but poorer perceived next day functioning attributed to sleep). Daily-level estimates showed increased alcohol use was associated with poorer perceived sleep health, while stronger effects from marijuana were associated with better perceived sleep health. Across all indices of sleep health, only one linear association was moderated by combined use: The adverse association between alcohol and next day functioning was weaker on days alcohol was combined with marijuana. Conclusions Findings provide additional evidence for daily-level effects of alcohol and marijuana use on perceived sleep health and address an important literature gap regarding potential adverse effects of SAM use.

Abstract Background Interoception is mental awareness, recognition and acknowledgement of physiological body signals. Understanding the role of sleep and interoception may provide a better understanding surrounding the sleep-health connection. Our primary objective was to examine the potential relationships between subjective sleep quality and multiple dimensions of interoceptive abilities in a large sample of young adults, a group who are vulnerable to sleep impairment and its widespread health consequences. Methods We conducted an online cross-sectional survey targeting young adults, aged 18–25 years. The Pittsburgh Sleep Quality Index (PSQI) was used to identify subjective sleep quality and the Multidimensional Assessment of Interoceptive Awareness Version 2 was used to assess eight domains of interoception. We conducted a series of Spearman’s bivariate correlations to assess the relationships between global sleep quality as well as the seven PSQI sub-components in relation to the eight interoception outcomes. We then conducted quantile regression to assess if global PSQI score was an independent predictor of interoception. Participants (n = 609) consented and provided data. Results After adjustment, the global PSQI was a significant predictor of ‘Non-Distracting’, ‘Emotional Awareness’ and ‘Trusting’, where β = − 0.10 (95% CI: − 0.14, − 0.07), β = 0.05 (0.01, 0.09), and β = − 0.10 (− 0.14, − 0.05), respectively. Conclusions Our findings reveal a small, significant relationship between sleep quality and interoceptive abilities amongst young adults. Sleep impairment may inhibit interoceptive skills, thus adding value to the mechanistic explanation of the sleep-health relationship. Experimental and prospective studies are needed to determine temporal associations.

Abstract Introduction Several studies have shown the negative impact of COVID-19-related confinement measures (spring 2020) on sleep features and quality. Here we aim to follow-up on these data by assessing self-reported sleep characteristics during the second wave of the pandemic (autumn 2020) in Italy, where the government responded with a second, partial, lockdown. Methods Using a longitudinal approach, 214 participants (Mage=36.78±14.2y; 159F) who had participated in a previous survey (April 2020 – Total Lockdown, TL) completed the same online survey from November 10th to December 1st (Partial Lockdown, PL). In each survey, participants had to respond to a set of questionnaires, including the Pittsburgh Sleep Quality Index (PSQI), referring to their current situation and, retrospectively, to their situation before the lockdown, resulting in four time-points (pre-TL, TL, pre-PL, PL). Results Linear mixed-model analysis showed that bedtime was delayed from pre-TL (23:46) to TL (24:42) and then linearly advanced in pre-PL (24:02) and PL (23:56). The same pattern emerged for rise time (pre-TL: 07:48, TL: 9:05, pre-PL: 8:28, PL: 7:58) and time in bed, which increased from pre-TL (8h4min) to TL (8h24min) and then linearly decreased (pre-PL: 8h15m, PL: 8h2m). Subjective sleep quality decreased in the two lockdowns compared to the period with no restrictions. The proportion of poor sleepers (PSQI>5) increased from 39.7% (pre-TL) to 48.6% in TL and again from 36.9% (pre-PL) to 47.7% in PL. Conclusion Sleep habits and quality showed different profiles across four time-points of the COVID-19 pandemic and related restrictive measures. Sleep timing alterations appeared during the first lockdown, recovered after the confinement period, and almost returned to baseline during the second lockdown (likely due to a normalization of working schedules). Instead, subjective sleep quality markedly worsened during both lockdowns relative to the preceding respective months. These data suggest that subjective sleep quality is particularly sensitive to changes in life habits and psychological factors, independently of sleep habits. Considering that the pandemic situation may continue for several months, there is a need for interventions targeting sleep quality. Support (if any) N/A

Studies have reported the significant economic impact of smoking cannabis and drinking alcohol In the USA. It was estimated that the costs of cannabis-related treatment, hospitalization and loss of work-related pay have amounted to $200 billion. (Andersen AM, Dogan MV, Beach SRH, Philibert RA. 2015 Genes 6 , 991–1022. ( doi:10.3390/genes6040991 )). Data from the National Epidemiologic Survey on Alcohol and Related Conditions showed that individuals with general anxiety disorder and substance use disorder (GAD-SUD) have higher psychiatric comorbidity rates than those without substance use disorder (Alegría AA, Hasin DS, Nunes EV, Liu SM, Davies C, Grant BF, Blanco C. 2010 J. Clin. Psychiatry 71, 1187–1195. ( doi:10.4088/JCP.09m05328gry )). Moreover, the criminal justice system is significantly impacted by this cost (Andersen AM, Dogan MV, Beach SRH, Philibert RA. 2015 Genes 6 , 991–1022. ( doi:10.3390/genes6040991 )). Despite the increasing use of cannabis, there are still too many obscure facts. One of the new areas that scientific evidence shows is impacted negatively by cannabis use is the epigenome, which is an understudied area that we are still learning about. In addition, over the past few decades, we have seen various social and healthcare changes that have raised critical questions about their ongoing roles in regulating marijuana and alcohol use. This is important because of the increasing popularity and usage across various ages especially young adults and teenagers. More than 97.5 million Americans over 12 years old have used cannabis for non-medical use despite the significant side effects, with 1 in 10 users developing cannabis dependence (Crean RD, Crane NA, Mason BJ. 2011 J. Addict. Med. 5, 1–8. ( doi:10.1097/ADM.0b013e31820c23fa ), Office of Applied Studies. 2006 Substance Abuse and Mental Health Services Administration, USA.). It was reported that 16% of substance abuse admissions in the USA were for cannabis-related symptoms, which is second only to alcohol-related disorders (Agalioti T, Lomvardas S, Parekh B, Yie J, Maniatis T, Thanos D. 2000 Cell 103, 667–678. ( doi:10.1016/S0092-8674(00)00169-0 ), Soutoglou E, Talianidis I. 2002 Science 295, 1901–1904. ( doi:10.1126/science.1068356 )). Today there are thirty-one states and the District of Columbia that currently have legalized marijuana for either medical or recreational use. Data about marijuana use from NIAAA's National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) indicates that ‘in total, 79 000 people were interviewed on alcohol and drug use. When examined by age young adults (ages 18–21) were found to be at highest risk for marijuana use and marijuana use disorder, with use increasing from 10.5 to 21.2% and disorder increasing from 4.4 to 7.5%’. ‘Given these facts, George Koob, PhD, director of NIAAA stated the importance for the scientific community to convey this information to the public about the potential hazards of marijuana and it's use’. On the other hand, according to the National Institute on Alcohol Abuse and Alcoholism, 16 million adults suffer from alcohol use disorders. To the best of our knowledge, epigenetic mechanisms have been previously studied in alcohol and cannabis abuse separately. Recent studies highlighted the molecular mechanisms that are linked with drug-induced transcriptional regulation, behavioural abnormalities and neurodegeneration, which has emphasized the role of chromatin modification/remodelling in the generation of drug activation of certain genes and the disabling of others, and the effect of that on addiction (Maze I, Nestler EJ. 2011 Ann. N. Y. Acad. Sci. 1216, 99–113. ( doi:10.1111/j.1749-6632.2010.05893.x ); Renthal W, Nestler EJ. 2008 Trends Mol. Med . 14, 341–350. ( doi:10.1016/j.molmed.2008.06.004 )). In this review, we will give an overview of epigenome science relevant to cannabis/the endocannabinoid system and the potential of epigenetic overlap between alcohol and cannabinergic activity at different stages, to aid further investigations that could bring more treatment options to our horizon.

Irregular sleep patterns are often a major hindrance in the life of undergraduate students, fueled by the undulating lifestyle anomalies and new vulnerabilities that come with college life. Insomnia, slapdash sleep cycles, and daytime dysfunction may affect both physical as well as mental well-being of individuals. India has the largest share of young adults in the world, which also makes it a likely epicenter for increasing sleep disorders. Students are often exposed to a significantly high level of academic burden and turn to options such as pulling all-nighters that further propel these issues. Most students realize the effects of a bad sleep schedule but nonetheless sacrifice it for the promise of better grades ignoring the double-edged sword. This study assesses the association of self-rated subjective sleep quality with habits prevalent among undergraduate students in India and its effect on their academic performance. Based on a modified PSQI questionnaire, the participants’ sleep quality was scored and a large majority of students showed an overall moderately good sleep quality. Insufficient sleep was seen to have an adverse effect on facial appearance, work productivity and enthusiasm for daily chores, among students. Unlike previous reports, the current dataset did not reveal any significant impact of sleep quality on the academic performance of the students. We also analyzed the most prevalent factors that were responsible for disruption of sleep in college goers and probed the major reasons for nighttime phone usage, which revealed social media to be a major contributor. Although the negative impact of sleep deprivation on academic performance has been studied earlier, any differential impact of stream has not been addressed thoroughly. Unlike the common belief of disparity induced due to stream-based academic pressure, our survey analysis showed insignificant contribution of stream leading to differences in sleep quality of students. Although participants’ reported mild disruption in sleep, it was observed across all streams and it did not seem to have an immediate effect on overall sleep quality of undergraduate students.

Substance use during pregnancy is not so rare as health care professionals could easily expect. Actually, 6 % of pregnant women have some misuse during pregnancy. Marijuana use has been in the news lately and when attitudes toward it have become more tolerant especially among young adults and use is becoming more popular, it is no surprise use also during pregnancy is coming more prevalent. In United States, 0.88 % of women use it during pregnancy [1].

Abstract Objectives The current study utilized mHealth technologies that were objective (e.g., sleep actigraphy and pulse oximetry) and time-sensitive (e.g., ecological momentary assessments [EMAs]) to characterize sleep in youth with sickle cell disease (SCD) and investigate the relationships between sleep variables and pain. It also investigated the influence of age on sleep and the sleep–pain relationship. Methods Eighty-eight youth with SCD (aged 8–17 years) were recruited from three regional pediatric SCD clinics. Youth completed twice daily EMAs for up to 4 weeks to assess nighttime subjective sleep quality and daily pain. They also wore a sleep actigraph for 2 weeks to assess sleep duration, sleep efficiency, and sleep latency, and a wrist-worn pulse oximeter for two nights to assess whether they had sleep apnea. Multilevel models were calculated predicting daily SCD pain using the sleep variables, age, and the interaction between age and the sleep variables. Results None of the sleep variables were related to one another. Poor subjective sleep quality during the night was related to high pain severity the next day, and high pain was related to poor subjective sleep quality that night. Older age was associated with poorer subjective sleep quality, shorter duration of nighttime sleep, and high sleep latency. Also, findings indicated that as age increased, the strength of the relationship between poor continuous subjective sleep quality and high pain severity increased. Conclusions Future research is needed to examine possible mechanisms connecting subjective sleep quality to high pain.

e18095 Background: A cancer diagnosis in adolescents and young adults (AYA) comes at a complex developmental interval that often coincides with risky behaviors, such as substance use. Modifiable risk behaviors are an important area of research to minimize morbidity for AYA. Our goal was to examine substance use, affect, and other health indicators in a diverse AYA cancer patient sample. Methods: We identified patients between the ages of 18-39 years and 6 or more months from date of cancer diagnosis. Measures included demographic, general health and substance use variables, the Center for Epidemiologic Studies Depression Scale (CES-D), and the State-Trait Anxiety Inventory (STAI-T). Linear trend analyses were performed to examine group differences. Backward-selection ordinal regression analyses identified specific factors associated with risky health behaviors. Results: Participants (60% male; Mage = 27; 70% White; 14% Black) included 100 patients with primary diagnoses of lymphoma (N = 45); leukemia (N = 37); sarcoma (N = 10); other (N = 8). Median time from cancer diagnosis was 2 years (range 6 months - 20 years). Past year alcohol, cigarette, and marijuana use was reported in 80% ,15%, and 33% of participants, respectively. We classified three subgroups based on past year alcohol and smoking, including non-smoker/non-drinker (n = 20), non-smoker/drinker (n = 65), and smoker/drinker (n = 15). Linear trend analyses indicated that the groups did not differ on time since diagnosis, but the smoker/drinker group initiated drinking younger ( p< 0.01), were more likely to use marijuana in the past year ( p< 0.01), and had increased depressive symptoms (CES-D; p< 0.01), anxiety (STAI-T; p< 0.05), body pain ( p< 0.05), and poorer sleep quality ( p< 0.05). Backward model selection treating outcome categories as ordinal indicated that frequent marijuana use and poor sleep quality were associated with being a past year smoker/drinker. Conclusions: A sizeable portion of AYA cancer patients engage in drinking and smoking, which is associated with negative affect and poorer self-reported health. Targeted interventions should be considered to reduce risky behaviors in this population.

This study examined the prospective association between peer victimization and subjective sleep quality among typically developing early adolescents. At Time 1 (T1), participants included 123 youth (50% boys; [Formula: see text] age = 12.03 years; 58.5% European American, 35% African American), along with one parent and teacher. Youth, parents, and teachers reported on youth’s experiences of peer victimization at T1. Youth reported on sleep quality at T1 and T2 (10 months later). Results from regression analyses revealed that youth-reported, but not parent- or teacher-reported, peer victimization predicted poorer sleep quality over time. Findings provide evidence that experiences of peer victimization can have a negative impact on youth’s sleep quality over time.
