Abstract Introduction Home court advantage (HCA) in the National Basketball Association (NBA) is well-documented, yet the co-occurring drivers responsible for this advantage have proven difficult to examine in isolation. The Coronavirus disease (COVID-19) pandemic resulted in the elimination of crowds in ~50% of games during the 2020/2021 NBA season, whereas travel remained unchanged. Using this ‘natural experiment’, we investigated the impact of crowds and travel-related sleep and circadian disruption on NBA HCA. Methods 1080 games from the 2020/2021 NBA regular season were analyzed using mixed models (fixed effects: crowds, travel; random effects: team, opponent). Results In games with crowds, home teams won 58.65% of the time and outrebounded (M=2.28) and outscored (M=2.18) their opponents. In games without crowds, home teams won significantly less (50.60%, p = .01) and were outrebounded (M=-0.41, p < .001) and outscored (M=-0.13, p < .05) by their opponents. Further, the increase in home rebound margin fully mediated the relationship between crowds and home points margin (p < .001). No significant sleep or circadian effects were observed. Discussion Taken together, these results suggest that HCA in the 2020/2021 NBA season was predominately driven by the presence of crowds and their influence on the effort exerted by the home team to rebound the ball. Moreover, we speculate that the strict NBA COVID-19 policies may have mitigated the travel-related sleep and circadian effects on the road team. These findings are of considerable significance to a domain wherein marginal gains can have immense competitive, financial, and even historical consequences.

Abstract Introduction Elite athletes are often required to travel for National and International competitions. However, the direction (westwards or eastwards), time zones crossed, and recovery days relative to their opponents may influence team success. The aim of this study was to determine whether differences in jetlag-induced circadian misalignment and number of recovery days between National Basketball Association (NBA) teams influenced the subsequent game result. Methods A total of 11,598 games from the 2011/2012 to the 2020/2021 seasons were analysed using mixed models with two fixed effects (travel, recovery) and three random effects (team, opponent, game time). Mediation modelling was also performed to determine if any influence of the fixed effects were mediated by another variable. The data is presented from the home team’s perspective. Results Teams with more recovery days between games, won by larger margins (F = 5.0, p < 0.001). Compared to one fewer recovery day (1.45 ± 13.92), one more recovery day (3.53 ± 13.51) advantaged the home team by 2.08 points (d = 0.15). The effect of travel on greater home team margins was completely mediated via recovery day differences (95% CI -0.11 to -0.03, p = 0.002). Discussion Using 10 seasons of data, our findings show that regardless of travel, recovery days between games significantly impact game margins. An advantage in recovery days should be considered for teams who travel more time zones westwards relative to their opponent. This suggests inequalities of the NBA schedule may be minimised for future seasons.

Mixed models are useful tools for analyzing clustered and longitudinal data. These models assume that random effects are normally distributed. However, this may be unrealistic or restrictive when representing information of the data. Several papers have been published to quantify the impacts of misspecification of the shape of the random effects in mixed models. Notably, these studies primarily concentrated their efforts on models with response variables that have normal, logistic and Poisson distributions, and the results were not conclusive. As such, we investigated the misspecification of the shape of the random effects in a Weibull regression mixed model with random intercepts in the two parameters of the Weibull distribution. Through an extensive simulation study considering six random effect distributions and assuming normality for the random effects in the estimation procedure, we found an impact of misspecification on the estimations of the fixed effects associated with the second parameter σ of the Weibull distribution. Additionally, the variance components of the model were also affected by the misspecification.

Abstract Introduction Travel fatigue and circadian disruptions are known factors that can hinder performance in professional athletes. The present exploratory study focused on investigating the impact of travel distance and direction on back-to-back games over the 2013–2020 seasons in the National Basketball Association (NBA). Methods Data from away and home games of back-to-back sequences, in two different cities, from the 2013 to 2020 seasons in the National Basketball Association were included in this study. Information from every selected game was retrieved from the official website of the NBA (www.nba.com). The outcomes were based on winning percentage with additional covariates including the direction of travel (eastward or westward) and the distance travelled (0-500km – 501-1000km – 1001-1500km – 1501km and more). If a team played both games of a back-to-back sequence on the road, they were considered Away-Away; if a team played the first game of a back-to-back sequence at home they were considered Home-Away; if a team played the first game of a back-to-back sequence on the road they were considered Away-Home. Results The sequence Away-Home significantly increases the likelihood of winning compared to the Away-Away and Home-Away sequences 54.4% (95%CI: 54.4,54.5); 39.2% (95%CI: 37.2,41.2), and 36.8%, (95%CI: 36.7,36.8), respectively. Following a road game, when teams travel back home, every additional 500km reduces the likelihood of winning by approximately 4% (p = 0.038). Finally, after withdrawing the Away-Home sequence, travelling eastward significantly increases the chance of winning (p = 0.024) compared to westward travel but has no significant impact on the probability of winning compared to neutral time zone travel (p = 0.091). Conclusion The accumulation of travel fatigue and the chronic circadian desynchronization that occurs over the NBA season can acutely disturb sleep and recovery. It appears that tailored sleep and recovery strategies need to be dynamically developed throughout the season to overcome the different challenges of the NBA schedule. Support (if any):

This study examined the home advantage effect in decisive National Basketball Association Conference Finals and Finals series playoff games from 1979 to 2019 (the 3-point shot era). We also examined the potential contribution of various offensive- and defensive-based skills and whether these skills mediated the relationship between game status (decisive vs. nondecisive) and outcome (win vs. loss). Overall, we found evidence of a home court advantage with the home team winning 63% of the decisive playoff games and 66% of the nondecisive playoff games. After adjusting for multiple comparisons and regular season win percentage, the home team had significantly more defensive rebounds and steals in Game 5 when trailing 3–1 going into that game. Mediation analyses did not reveal any significant findings when examining the impact of decisive game status on performance through offensive and defensive skills, thus suggesting there are other explanations for the home advantage effect.

Abstract PURPOSE: This meta-analysis aims to evaluate the impact of delaying surgery in operable breast tumor patients after neoadjuvant chemotherapy (NAC) on survival. METHODS:An electronic literature retrieval was conducted on PubMed/Medline and EMBASE((between January 2000 and June 2020).The primary end point was overall survival(OS),secondary end points included disease-free survival (DFS) or recurence-free survival (RFS).The HR with 95% confidence intervals were calculated using a random-effects or fixed-effects model. RESULTS:The combined HR for OS was 1.51 (95% CI:1.30-1.76; P=0.000) by fixed effects model.No statistically significant heterogeneity was found (P=0.168, I 2 =31.3%).The pooled HR for RFS/DFS was 1.59 (95%CI:1.30-1.95,I 2 = 66.0%) by random-effects model,with significant heterogeneity. CONCLUSION:Our meta-analysis revealed a significant adverse association between longer TTS after NAC and more inferior OS and RFS/DFS in patients with breast cancer.Clinicians and patients should minimize surgical delay after NAC as much as possible.

Gelendzhik is the resort town, there aren't a lot of industrial enterprises here. The main pollutant is transport. The work purpose is the assessment of a condition of the artificial landings of a pine located along the Federal highway "Don" and landings, which is nearly the sea coast. Researches were conducted to a standard technique of the General vital state (A. S. Bogolyubov). The assessment of a condition of pines was carried out during 6 years: from 2010 to 2015. For carrying out research we used 6 experimental grounds on the Markotkhsky spine and 2 control grounds within the town. We investigated 24 trees on each platform, middle age of the trees were 30 - 40 years. Results. 1. The condition of trees in the pine forests located in immediate proximity with the Federal highway "Don" (No. 1, 2, 3) is unsatisfactory. As even weak influences of the majority of atmospheric gaseous pollutants (sulphurous gas, nitrogen oxides, etc.) give effect of a necrosis and hloroz of pine needles, the condition of pines is connected with technogenic pollution. So near the Federal highway "Don" the air environment is strongly polluted by exhaust gases. Information of 2012 confirm that negative influence of the route on Markotkh's vegetation decreases at reduction of load of the route. 2. On the sites located above on a slope (No. 4,5,6) thanks to remoteness and the wind mode intensity of influence of pollutants is lower and a condition of pines the quite satisfactory. 3. Trees on the sites located near the sea (No. 7,8) are in a good shape. Small deterioration of a state is noted in very droughty years. Conclusion. Results of six years' research show that the condition of the plantings which are in close proximity with the road worsens. It is explained by increase in intensity of the movement on the road, especially during a resort season. Gelendzhik is the city with a good ecological shape, but the damage to environment is already caused. If not to take measures, we can lose a unique part of the nature in the future, recreate it will be impossible. Measures of reduction of negative impact of exhaust gases were offered. Results of researches are transferred to ecological department of the City administration of Gelendzhik.

Abstract Public health professionals (PHP) have a dual task in climate change. They should persuade their colleagues in clinical medicine of the importance of all the issues covered by the GD. The fact that the health sector contributes to the overall emissions of 4.4% speaks to the lack of awareness within the health sector itself. The issue of providing adequate infrastructure for the health sector is essential. Strengthening the opportunities and development of the circular economy within healthcare is more than just a current issue. The second task of PHP is targeting the broader population. The public health mission is being implemented, inter alia, through numerous activities related to environmental monitoring and assessment of the impact on health. GD should be a roadmap for priorities and actions in public health, bearing in mind: an ambitious goal of climate neutrality, an insistence on clean, affordable and safe energy, a strategy for a clean and circular economy. GD provides a framework for the development of sustainable and smart transport, the development of green agriculture and policies from field to table. It also insists on biodiversity conservation and protection actions. The pursuit of zero pollution and an environment free of toxic chemicals, as well as incorporating sustainability into all policies, is also an indispensable part of GD. GD represents a leadership step in the global framework towards a healthier future and comprises all the non-EU members as well. The public health sector should consider the GD as an argument for achieving goals at national levels, and align national public health policies with the goals of this document. There is a need for stronger advocacy of health and public-health interests along with incorporating sustainability into all policies. Achieving goals requires the education process for healthcare professionals covering all of topics of climate change, energy and air pollution to a much greater extent than before.
