Primary small cell neuroendocrine carcinoma (SNEC) of the paranasal sinuses is an extremely rare and distinctive tumor with aggressive clinical behavior. Moreover, SNECs originating in the head and neck region have been reported to be highly aggressive and to have a poor prognosis. This report describes a patient with a maxillary sinus SNEC who was successfully treated with neoadjuvant chemotherapy and concurrent chemoradiotherapy.

Oral Cancer is one of the most common malignancies of the head and neck region. Despite technological advancements and improvements in Oral cancer diagnosis and treatment modalities, the 5-year survival rate remains low and is associated with poor prognosis and high mortality rate especially when detected at a later stage. The empirical therapy followed for the treatment of oral cancer includes surgery, radiotherapy and chemotherapy. The treatments are not equally efficacious for all patients, are associated with side effects and poor prognosis. The need of the hour is early diagnosis and tailored treatment therapies for individual patients. With the advent of immunotherapy, the cancer treatment has moved toward personalised precision medicine which tailors’ treatments to each individual. Personalised precision medicine incorporates, molecular profiling of tumours with OMICS technology, biomarkers and companion diagnostics to build databases of patients and devise tailor made treatment approaches for individual patients. This article discusses the role of precision medicine in OSCC prevention, detection, and management by reviewing our understanding of OC from both genetic and OMICS perspectives.

Malignant peripheral nerve sheath tumours (MPNSTs) are rare, aggressive softtissue sarcomas associated with poor prognosis, that most commonly affect patients aged20 to 50 years, but have also been reported in children. The tumour is usually found in lowerextremities and only 10% to 20% of all lesions occur in head and neck region thus making ita rare entity. Central involvement, particularly in the jawbones is quite unusual. There is littlereported in literature on these tumors in Africa. Here we report a rare case of intraosseousMPNSTS occurring in the mandible in a 12-years old girl. The biological behavior and diagnosticchallenges of this rare malignancy are discussed.

Osteoradionecrosis of the skull base (SB-ORN) is a serious potentially lethal complication after radiotherapy in the head and neck region and is therefore considered one of the most challenging clinical entities among skull base lesions. Management options includes nasal irrigation, medications such as systemic antibiotics and anti-inflammatory drugs, hyperbaric oxygen therapy, and surgical debridement. Despite these efforts treatment outcomes are associated with a poor prognosis. Recent studies have shown an increased disease control rate with a more aggressive surgical approach including sequestrectomy. One of the main factors associated with poor prognosis is involvement of the internal carotid artery near the skull base. Spontaneous carotid rupture is frequently associated with death and/or severe morbidity and also poses risk of profuse bleeding during surgery. In this article we will deliver a comprehensive review of SB-ORN, including pathophysiology, clinical features, and an update in treatment strategy.

Fibromyalgia is a common syndrome (FM) that is prevalent all over the world with different prevalence rates in each country. The etiology and pathogenesis of fibromyalgia are still unknown, unclear, and with uncertain pathophysiology. Diagnosis of fibromyalgia is not that easy because the disease is described as multifactorial, and very little information is known about the causes of the disease. Treatment of fibromyalgia is not totally curative; treatment aims to improve and alleviate the symptoms as easing pain, improving sleep, develop the physical function. The main problem with fibromyalgia is not only difficulty of diagnosis or poor prognosis, but also the treatment is not adequate or satisfactory, so it is of great importance to educate the patients about their illness and giving them advice on how to limit the risk factors that aggravate the case, besides educating patients to use non-pharmacological strategies for better management.
