Reward can improve motor learning and the consolidation of motor memories. Identifying the features of reward feedback that are critical for motor learning is a necessary step for successful integration into rehabilitation programs. One central feature of reward feedback that may affect motor learning is its timing, that is, the delay after which reward is delivered following movement execution. In fact, research on associative learning has shown that short and long reward delays (e.g., 1 and 6 s following action execution) activate preferentially the striatum and the hippocampus, respectively, which both contribute with varying degrees to motor learning. Given the distinct functional role of these two areas, we hypothesized that reward timing could modulate how people learn and consolidate a new motor skill. In sixty healthy participants, we found that delaying reward delivery by a few seconds influenced motor learning dynamics. Indeed, training with a short reward delay (i.e., 1 s) induced slow, yet continuous gains in performance, while a long reward delay (i.e., 6 s) led to initially high learning rates that were followed by an early plateau in the learning curve and a lower endpoint performance. Moreover, participants who successfully learned the skill with a short reward delay displayed overnight consolidation, while those who trained with a long reward delay exhibited an impairment in the consolidation of the motor memory. Overall, our data show that reward timing affects motor learning, potentially by modulating the engagement of different learning processes, a finding that could be exploited in future rehabilitation programs.

Motor memories become resistant to interference by the process of consolidation, which leads to long-term retention. Studies have shown involvement of the somatosensory cortex in motor learning-related plasticity, but not directly in motor memory consolidation. This Neuro Forum article reviews evidence from a continuous theta-burst transcranial magnetic stimulation (cTBS) study by Kumar and colleagues (Kumar N, Manning TF, Ostry DJ. PLoS Biol 17: e3000469, 2019) that demonstrates the role of somatosensory, rather than motor, cortex in human motor memory consolidation during implicit motor learning.

Cerebellar climbing fiber activity encodes performance errors during many motor learning tasks, but the role of these error signals in learning has been controversial. We compared two motor learning paradigms that elicited equally robust putative error signals in the same climbing fibers: learned increases and decreases in the gain of the vestibulo-ocular reflex (VOR). During VOR-increase training, climbing fiber activity on one trial predicted changes in cerebellar output on the next trial, and optogenetic activation of climbing fibers to mimic their encoding of performance errors was sufficient to implant a motor memory. In contrast, during VOR-decrease training, there was no trial-by-trial correlation between climbing fiber activity and changes in cerebellar output, and climbing fiber activation did not induce VOR-decrease learning. Our data suggest that the ability of climbing fibers to induce plasticity can be dynamically gated in vivo, even under conditions where climbing fibers are robustly activated by performance errors.

Misfolding of prion protein (PrP) into amyloid aggregates is the central feature of prion diseases. PrP has an amyloidogenic C-terminal domain with three α-helices and a flexible tail in the N-terminal domain in which multiple octapeptide repeats are present in most mammals. The role of the octapeptides in prion diseases has previously been underestimated because the octapeptides are not located in the amyloidogenic domain. Correlation between the number of octapeptide repeats and age of onset suggests the critical role of octapeptide repeats in prion diseases. In this study, we have investigated four PrP variants without any octapeptides and with 1, 5 and 8 octapeptide repeats. From the comparison of the protein structure and the thermal stability of these proteins, as well as the characterization of amyloids converted from these PrP variants, we found that octapeptide repeats affect both folding and misfolding of PrP creating amyloid fibrils with distinct structures. Deletion of octapeptides forms fewer twisted fibrils and weakens the cytotoxicity. Insertion of octapeptides enhances the formation of typical silk-like fibrils but it does not increase the cytotoxicity. There might be some threshold effect and increasing the number of peptides beyond a certain limit has no further effect on the cell viability, though the reasons are unclear at this stage. Overall, the results of this study elucidate the molecular mechanism of octapeptides at the onset of prion diseases.

An overview of the areas of sibling research that laid the foundation for the modern understanding of sibling issues is presented. Attention is focused on the importance of sibling relationships for personal development, socialization and adaptation, and on the existing shortage of relevant work at the same time. The main stages of the development of sibling theory in their continuity are considered, starting from the first studies devoted to the search for a connection between the order of birth and achievements and dated to the end of the 19th century, to the term “individual environment” developed by the genetics of behavior in the second half of the 20th century. It emphasizes the role of A. Adler, who has made sibling a central feature of family life and personal development and has long determined the future direction of empirical family research. It shows a gradual shift in the interest of researchers from studying the influence of birth order, gender, and age intervals between siblings on personal characteristics. In this connection, an analysis of the nature of sibling relations on the basis of reciprocity and complementarity, undertaken by Dunn, is given. Early works on the jealousy and rivalry of children in the family, the study of the impact of parental differential treatment on them, as well as the influence of child characteristics on siblings in families with sick children are considered.

The role of comprehensive cardiac rehabilitation (CCR) is well established in the secondary prevention of cardiovascular diseases such as coronary artery disease and heart failure. Many clinical trials demonstrated effectiveness of CCR in improving exercise capacity, quality of life, and in reducing cardiovascular mortality and morbidity. However, even before the era of the COVID-19 pandemic comprehensive cardiac rehabilitation program’s implementation, especially the second phase, had many barriers. One of the main reasons for not attending in second phase of CCR was lack of transportation from patient’s home to rehabilitation centers. Additionally, in recent months COVID-19 pandemic has led to closure of many cardiac rehabilitation centres resulting in many eligible patients unable to participate in the optimisation of secondary prevention. During the coronavirus disease-2019 pandemic, hybrid telerehabilitation has become the leading solution in the cardiac rehabilitation programs. The present paper contains key information about structures, effectives and safety of hybrid telerehabilitation during the COVID-19 era.

Introduction: The declaration of COVID 19 pandemic by WHO on 11th March 2020, is the dening global health crisis of the past 100 years. The following Nationwide survey was conducted to ascertain the sea changes this pandemic has brought in the physiatrist practice, patient management and preparing oneself to the challenges of rehabilitation in the POST COVID world. The survey covers the role and importance of social media in medical education and consultation in a POST COVID world. It also sheds light on the nancial implications and personal lifestyle changes that physiatrists are witnessing. A Google form comprising of 74 questions, divided in 3 se Material &Methods: ctions, was used to gather the requisite data on :- A) PRECOVID practice, management and lifestyle; B) Life during COVID pandemic; and C) Anticipated changes in POST COVID era. The survey was circulated to 400 physiatrist via various interactive groups who have been practicing either independently or in govt setups and associated hospitals nationwide. They were requested to submit their responses in a period of 30 days. In PRECOVID era an average Result: physiatrist was nely balancing his profession, personal lifestyle, learning and recreation. The LOCKDOWN clamped their practice which lead to nancial drought but hardly made a dent in productivity by engaging in webinars, reading and researching. The POST COVID time demands implementation of newer safety protocols like changes in setup, regulating the patient trafc, limiting daily procedures, engaging in distance learning through virtual conferences, maintaining a healthy lifestyle and constructing proper protocols for pulmonary, neurological and musculoskeletal rehabilitation programs for patients post recovery from COVID19, especially the patients weaning from ventilators. Conclusion: The effect of COVID19 demands major change in physiatrist professional practice like limiting consultation, following safety protocols, COVID testing, preparing consolidated program for POSTCOVID sequel. Social media is rightly poised to be a major tool for education, consultation, marketing and awareness. The role of teleconsultation needs to be reprised, recognised and regularised. Webinars and virtual conferences will nd more takers in future.
