Purpose – This article aims to analytically review the Plan-Do-Study-Act (PDSA) model used in the Pacesetters national programme evaluation. Design/methodology/approach – The evaluation team's two-fold role is outlined: supporting project teams to develop PDSA plans and collect evaluation data. Four case studies are used to show the PDSA model's application and effect in a participatory action learning approach. Despite limitations, it is clear that all four case studies illustrate the PDSA model's potential benefits in a participatory evaluation approach, which involves public and patients. Findings – The model's effectiveness is premised on several enabling factors such as: teams appreciating the model; a climate that values all learning and open to re-planning; engaging any external evaluators whose role is clearly communicated to all project stakeholders in a timely fashion. Practical implications – There are clear intentions to promote evidence-based commissioning in the UK that values patient and public involvement. The PDSA model has the potential to test and implement changes in real work-place settings and to involve the public in evaluation. Originality/value – Case study analysis identifies new reflections on the PDSA model's use to support innovative NHS practice development with public involvement in a participatory approach.

Purpose – Remote monitoring technology (RMT) is widely acknowledged as an important enabler of servitisation however, there is a dearth of understanding about how RMT is used by manufacturing firms to support servitised strategies. The purpose of this paper is to contribute to this important yet somewhat ignored topic in servitisation research. It attempts to address the following questions: what has constrained, and what has enabled the exploitation of RMT in the context of servitised strategies? Design/methodology/approach – The research adopts an exploratory multiple-case study design. Four in-depth descriptive case studies of companies operating in aerospace, industrial equipment, marine, and transport sectors were conducted. The collected data were analysed and synthesised, drawing out conclusions. Findings – The study describes how four manufacturers are using RMT and identifies ten factors that have enabled and constrained the realisation of expected outcomes. The enabling factors identified include: skills, experience, and knowledge; support from customers and other complementary data sources, processes, and structures; operations centres; historical data; and presence of in-house knowledge and capabilities. While the constraining factors include: defining benefits of RMT; limitations of RMT; limited understanding about true capabilities of RMT; knowledge management; and lack of alignment between services and manufacturing strategies. Research limitations/implications – While considerable attention and effort have been invested in designing and conducting the research and analysing the data from the case studies, more empirical work is required to validate and enrich findings and conclusions. For this purpose several research questions to guide further theory development in this area are formulated. Originality/value – This paper is an in-depth study examining the role of RMT in supporting servitised strategies. In particular, it explores how this technology is used in practice to support service-oriented value propositions of manufacturers and identifies the factors that are key to successfully executing this strategy. As such it qualifies as one of the first studies of this kind.

IntroductionPressure continues to grow on emergency departments in the UK and throughout the world, with declining performance and adverse effects on patient outcome, safety and experience. One proposed solution is to locate general practitioners to work in or alongside the emergency department (GPED). Several GPED models have been introduced, however, evidence of effectiveness is weak. This study aims to evaluate the impact of GPED on patient care, the primary care and acute hospital team and the wider urgent care system.Methods and analysisThe study will be divided into three work packages (WPs). WP-A; Mapping and Taxonomy: mapping, description and classification of current models of GPED in all emergency departments in England and interviews with key informants to examine the hypotheses that underpin GPED. WP-B; Quantitative Analysis of National Data: measurement of the effectiveness, costs and consequences of the GPED models identified in WP-A, compared with a no-GPED model, using retrospective analysis of Hospital Episode Statistics Data. WP-C; Case Studies: detailed case studies of different GPED models using a mixture of qualitative and quantitative methods including: non-participant observation of clinical care, semistructured interviews with staff, patients and carers; workforce surveys with emergency department staff and analysis of available local routinely collected hospital data. Prospective case study sites will be identified by completing telephone interviews with sites awarded capital funding by the UK government to implement GPED initiatives. The study has a strong patient and public involvement group that has contributed to study design and materials, and which will be closely involved in data interpretation and dissemination.Ethics and disseminationThe study has been approved by the National Health Service East Midlands—Leicester South Research Ethics Committee: 17/EM/0312. The results of the study will be disseminated through peer-reviewed journals, conferences and a planned programme of knowledge mobilisation.Trial registration numberISRCTN51780222.

Purpose – The purpose of this paper is to review the latest management developments across the globe and pinpoint practical implications from cutting-edge research and case studies. ObjectivesTo design, deliver and evaluate a programme of training workshops for biomedical researchers aimed at building confidence and skills in actively involving patients and the public (PPI) in research.DesignA bespoke programme of training workshops in PPI aimed at researchers.SettingA large National Institute for Health Research Biomedical Research Centre in London and several partner organisations.Participants721 scientists, clinicians and research managers attending dedicated training in PPI at a major London NHS (National Health Service)–university partnership.InterventionsA programme of 72 training workshops, designed to build practical skills and confidence for researchers working with patients and the public in research, was delivered at a major research-active NHS:university partnership. An iterative approach was taken to the programme, with the content of the workshops continually reviewed and refreshed to respond to the needs of researchers. Surveys before, immediately following and 6 months after training investigated the impact on researchers’ confidence and skills in PPI work, and the kind of PPI they subsequently carried out.ResultsTraining brought about immediate marked increases in researchers’ self-reported confidence to carry out PPI activities within their research, and in their knowledge of good practice. The evaluation indicates that workshop attendees were more likely to involve patients in their research following training. Researchers tended to involve patients and the public in a range of areas, including input to study design and patient information, in particular.ConclusionsWhen positioned within a broader organisational strategy for PPI in research, such training has an important role to play in progressing PPI in a major research partnership. Training appeared to provide the confidence needed to carry out PPI which enabled further development of confidence and skills. Involving researchers who have attended the training in the ongoing development of the programme and bringing in patients to the training programme are key next steps.

Purpose The energy retrofit of the existing building stock, and specifically the thermal upgrading of the buildings’ envelopes, has been identified as a key action for both the decarbonisation of the built environment and the reduction in fuel poverty. When considering the energy retrofit of heritage buildings it is, however, important to recognise both the technical issues that this entails and the potential impact on their cultural value and the emotional responses to it. The purpose of this paper is to focus on the thermal upgrading of historic timber-framed buildings in the UK. Design/methodology/approach The paper begins by exploring the cultural significance of this form of building construction, before examining three case studies using both quantitative and qualitative methodologies. Findings The results show that whilst the application of energy retrofit actions to this emblematic typology may have limited success, the emotional connection of the buildings’ occupants often results in the work resulting in higher user satisfaction than would otherwise be expected. Research limitations/implications Although limited in number, the three case studies provide an insight into the complex issues surrounding the low energy retrofit of historic timber-framed buildings. Further research into this area is encouraged. Practical implications The paper contains the monitoring of specific retrofit details, the results of which should inform future projects. Social implications The review of the cultural significance of historic timber-frame buildings in the UK underlines the importance of the conservation and continuing survival of these buildings. Originality/value Previous heritage retrofit research in the UK has focussed on solid wall construction with little investigation into the issues surrounding the retrofit of historic timber-frame buildings. This paper explores this previously under-researched area. Additionally, this paper begins to explore the possible links between occupants’ emotional connection to historic buildings and their perceived levels of comfort.

BackgroundA cross-sectional service evaluation within South Central Ambulance Service (SCAS) revealed deficiencies in the assessment and management of paediatric patients’ pain resulting from trauma. This suggested a need for further work to identify facilitators and barriers to pain assessment and management amongst this patient group. Studies looking into the barriers and facilitators to paramedics’ assessment and management of paediatrics in pain have been conducted internationally and not solely in traumatic events. These studies are not transferable to the UK setting due to prehospital emergency care differences within these countries.MethodsThis study will utilise qualitative research methods.The study population will be qualified paramedics. In-depth semi structured interviews will be conducted using a guide developed from previous studies and refined at patient and public involvement events. The expected numbers of participants will be 12–15 but will continue to the point of data saturation. Thematic analysis of data will be performed using an open coding technique guided by Braun and Clarke’s six step framework. Once complete member checking will take place with each participant.ResultsIt is anticipated that both positive and negative aspects of paramedics’ role during these incidents will be identified. Previous literature has identified support from colleagues, interaction with parents, and the ability to alleviate pain as facilitators. Barriers have included current educational practices, lack of exposure, guidelines, the prehospital environment, difficulty in assessing young children and difficulty in providing pain relief. It is therefore likely that these themes may emerge but identification of UK-specific barriers and facilitators is anticipated.ConclusionsWith no previous study of this nature conducted before in the UK this could valuably inform the education and teaching provision of ambulance services and universities as well as a platform for policy makers and guideline producers to enhance the tools available to paramedics.

Purpose – A growing literature reports the benefits and challenges of patient and public involvement (PPI) in research; nevertheless, understanding PPI in research design remains under-developed. The purpose of this paper is to report learning experiences from involving service users as research partners in two projects that developed and evaluated guidelines for good practice in this regard. The main objective was to evaluate these guidelines. Design/methodology/approach – PPI research guidelines were developed through five workshops involving service users/patients, carers, health and social care professionals/managers and academics. Using a participatory qualitative approach, these guidelines were evaluated through mapping them against the two service user research partners’ experience within another project. Findings – The guidelines were found to be fit for purpose, as they allowed problems to be easily identified and reassurance that required standards were being met. Both academic and service user research partners learned and gained relevant skills. Two service user research partners also found their daily living skills unexpectedly enhanced by project participation. Originality/value – The PPI guidelines, the authors developed were produced by consensus involving several stakeholders. Service users involved as research partners in the project experienced unanticipated personal benefits.
