BACKGROUND No studies have been conducted on smart devices that provide integrated nursing for patients' urinary & bowel elimination needs. OBJECTIVE To evaluate the feasibility of a smart excretion care system tethered to Electronic Medical Record (EMR) in a tertiary hospital and community care setting and discuss the expectations of the reducing burden of nursing care. METHODS Focus group interviews were conducted based on the Consolidated Criteria for Reporting Qualitative Research Guidelines. The interviews were conducted in March, 2021, involving 67 participants who had worked at Seoul National University Bundang Hospital for more than one year and had experience assisting patients with excretion care. Data were collected using both purposive and snowball sampling methods. RESULTS This research found four themes relevant to the Smart Excretion Care System: expectations of reducing burdens of nursing care, applicable indications (by departments and diseases), preferred features/functions, and expected benefits of using the Smart Excretion Care system at the clinical facilities. Nurses from the Comprehensive Nursing Service wards were found to have the highest burden of excretion care. It was a common opinion that the Smart Excretion Care System would be very useful in the Intensive Care Unit, and it should be applied first to patients with stroke or dementia. CONCLUSIONS Excretion care is one of the most bothersome tasks for nurses that increase their workload. The further development of the Smart Excretion Care System, as a digital health intervention, can help improve nurses’work efficiency and reduce their burden as well as expand to caregivers and guardians.

LANGUAGE NOTE | Document text in Chinese; abstract also in English. Developments in research have made culturing human embryos beyond the 14-day limit seem technologically feasible. In the article “Emerging Human Embryo Research Technologies, the 14-day Rule, and the Special Status of the Embryo,” the authors examine a proposal for new human embryo and embryoid research guidelines by reviewing the history of the 14-day limit and emerging areas of research that are impacted by these guidelines. However, as noted by the authors, changes in science policy should not be developed solely by scientists. Instead, policy development should reflect the reality of science as a public endeavor. After 40 years of consensus, any attempts to revoke the 14-day limit on the in vitro culturing of human embryos should rely on public and stakeholder engagement.

Abstract Background Heart failure (HF) is a global epidemic affecting the elder globally. It is uncertain what care patients with heart failure receive at their end of life and what care trends are in the last month of life. OBJECTIVES: This study's objective was to investigate the changes in the use of intensive and supportive procedures for Taiwanese patients with heart failure in their last month of life during 2001-2013. Methods Analysis of claims data of 25,375 patients with heart failure obtained from the National Health Insurance Research Database was performed to investigate the changes in the use of intensive and supportive procedures for Taiwanese patients with heart failure in their last month of life during 2001-2013. Results Over the whole study period, 53.3% of patients with heart failure were admitted to intensive care units in their last month of life. The percentages of patients receiving mechanical ventilation (54.3%-41.5%), cardiopulmonary resuscitation (41.5%-16.7%), decreased over time. The percentages of patients receiving artificial hydration and nutrition (52.5.9%-56.8%) and extracorporeal membrane oxygenation(ECMO) (0.52%-1.78%) increased over time. Patients under 75 years old were more likely to be admitted to intensive care units. Conclusion Over time, supportive procedures increased, and intensive procedures decreased in patients with heart failure in the last month of life. This study highlights a need for research, guidelines, and training in how to provide palliative care for end-stage patients with heart failure.

Background. Worldwide, 47 million people suffer from dementia. Despite recognizing the importance of spirituality within dementia care, it is still unclear how this should be integrated into dementia services. Aim. To explore the perspective of health professionals regarding the spiritual care of people with advanced dementia. Methods. A qualitative systematic review was performed following the Enhancing Transparency in Reporting the Synthesis of Qualitative Research guidelines for the study design. The inclusion criteria included original articles published from January 2008 to March 2019, using either qualitative or mixed methods. The quality of the articles included was evaluated using the consolidated criteria for reporting qualitative research, Standards for Reporting Qualitative Research, and the Critical Appraisal Skills Programme. Synthesis of findings was performed using thematic analysis. Results. Twelve studies were included in the review. Seventeen categories were identified, grouped into four themes: (1) the perception of spirituality, including the failure to address the same, (2) the spiritual needs of people with advanced dementia, (3) spiritual needs from health care providers, and (4) addressing spirituality, with the following categories: music, significant activities, among others. Conclusions. Spirituality is not formally addressed in this population, and professionals do not feel confident enough to be able to integrate spirituality in their care. It is necessary to identify and record the spiritual needs of people with advanced dementia, as well as to design specific care programs.

The paper aims to present a systematic literature review in the field of human resource management in times of crisis since 2008. In that way gaps in the current body of knowledge can be established that justify future research guidelines. The study consists of an analysis of 56 articles published in journals indexed in the Web of Science database. The majority of the papers were published in recent years, indicating a growing interest in the field. To be included, the papers had to be (1) peer-reviewed papers, (2) empirical papers that report on HRM practices in firms, (3) written in English, and (4) published in the period 2008–2021. Four themes were identified: economic crisis and HRM, health crisis and HRM, natural disasters and HRM, and political instability and HRM. The findings provide valuable knowledge and understanding of the present situation of HRM in crises. A number of future research guidelines are presented, which may encourage more research in this crucial field of study.

Newcomer adolescents make up a large minority of Canada’s population and their positive integration experiences with education systems across the country are critical for both their development and the country’s long-term success. The current study examined newcomer adolescents’ ( n = 4, between 16 and 18 years old) integration experiences using an arts-based engagement ethnography to understand what influences their positive integration into the school system. Artifacts, interview, and focus group data were analyzed systematically using ethnographic research guidelines. Five structures were identified: (1) barriers to advancement at individual, school, and macro levels, (2) fluctuating relationship with cultural identity, (3) limited trust in systems, (4) resilience through independent learning, and (5) facilitating factors to positive integration experiences at the family and school level. In keeping with a relational developmental systems theory framework, each structure accounts for multiple inter- and intra-individual factors at multiple environmental levels. These findings outline considerations for systemic issues in academic institutions and offer suggestions for how institutions can better support newcomer adolescents.

Aims: There are several advantages to pooling survey data from individual studies over time or across different countries. Our aim is to share our experiences on harmonizing data from 13 Finnish health examination surveys covering the years 1972–2017 and to describe the challenges related to harmonizing different variable types using two questionnaire variables – blood pressure measurement and total cholesterol assessment – as examples. Methods: Data from Finnish national population-based health surveys were harmonized as part of the research project ‘Projections of the Burden of Disease and Disability in Finland – Health Policy Prospects’, including variables from questionnaires, objective health measurements and results from the laboratory analysis of biological samples. The process presented in the Maelstrom Research guidelines for data harmonization was followed with minor adjustments. Results: The harmonization of data from objective measurements and biomarkers was reasonably straightforward, but questionnaire items proved more challenging. Some questions and response options had changed during the covered time period. This concerned, for example, questionnaire items on the availability and use of medication and diet. Conclusions: The long time period – 45 years – made harmonization more complicated. The survey questions or response options had changed for some topics due to changes in society. However, common core variables for topics that were especially relevant for the project, such as lifestyle factors and certain diseases or conditions, could be harmonized with sufficient comparability. For future surveys, the use of standardized survey methods and the proper documentation of data collection are recommended to facilitate harmonization.

Introduction: Sleep is necessary for maintaining adequate health and well-being. However, several behavioral and environmental factors, known as sleep hygiene, could affect sleep quality, contributing to the development of insomnia. Instruments to measure sleep hygiene such as the sleep hygiene index (SHI) have been developed, but only a few are available in the Arabic language. Therefore, this study was designed to translate and culturally adapt the SHI from English to Arabic. Methods: The SHI was translated from English to Arabic using the forward–backward translation method recommended by the International Society for Pharmacoeconomics and Outcomes Research guidelines. Nine Arabic-speaking individuals of diverse educational and cultural backgrounds reviewed the Arabic SHI. The internal consistency reliability of the items contained in the Arabic SHI was determined. Results: The SHI was successfully translated and culturally adapted into the Arabic language. Minimal changes in the wording of some questions were required to ensure the cultural adaptability of the instrument. The Arabic version of the SHI was found to be simple, clear, and brief as reported by the participants during cognitive debriefing. The Arabic SHI has moderate internal consistency reliability with a Cronbach's alpha of 0.589. Conclusion: The Arabic SHI is a brief and easy-to-understand instrument for assessing sleep hygiene practices and behavior in Arabic-speaking population. Further assessment of the psychometric properties of the Arabic SHI is necessary to ensure the validity of this instrument in different populations.
