Hydroxylapatite (HA) has been shown to be an efficacious bio-implant material in a number of surgical procedures[l]. A problem in the use of this material is that of particle migration away from the implant site. One method to resolve this problem is to mix the HA with calcium sulfate hemihydrate, CaSO4·0.5H2O, and allow the material to set to a gypsum/HA composite monolith[2]. The gypsum is resorbed by the body over a period of weeks, leaving the hydroxylapatite surrounded by newly generated tissue.

Abstract Background: Following the implementation of the health transformation plan in Iran and the significant increase in medical tariffs, it seems there is still a considerable gap between the actual cost of the Global Surgical procedures (GSPs) and the approved tariffs. The aim of this study was to compare the actual costs of GSPs with approved tariffs in hospitals affiliated to Qazvin University of Medical Sciences in 2016. Methods: In this cross-sectional study, data of 6,126 GSPs performed in three hospitals were extracted from Hospital Information System and approved tariffs were also obtained from the Supreme Council of Health Insurance. The difference between the average actual costs of a GSP and its tariff was considered as the tariff-actual cost gap. Regression test was used to determine the factors affecting the tariff-actual cost gap using SPSS.21. Results: The average actual cost of GPSs was 503 USD while the average tariff was 361 USD, indicating a significant negative gap between the approved tariffs and the actual cost of GPSs. In fact, for each procedure, the hospital suffered an average of 142 USD losses. In 54 of 63 GSPs, the average actual cost was 0.3% to 307.4% higher than the tariff. The highest negative gap was related to the “femoral fracture fixation" with -307.4%. The gap was higher in older patients, females, and patients with a longer stay, orthopedic specialty, and full-time surgeons (p<0.05). Conclusions: The findings of this study showed that tariffs covered only 71.8% of actual costs. It is suggested that in the tariff setting of GSPs, factors such as inflation rate, patient's age, comorbidity, disease severity, and hospital function (teaching or referral center) be considered. In addition, hospital management can also reduce the gap by modifying processes and managing resource consumption, especially medicines and consumables items.

AbstractBackground:Deformities of the carotid artery are rare. Tortuosity, kinking and coiling of the internal carotid artery may be observed with advancing age. A tortuous internal carotid artery may cause an abnormal sensation in the throat. In the early twentieth century, there were several reported cases of fatal haemorrhage during pharyngeal surgical procedures, because this condition went undetected.Method and results:We present two cases of tortuosity of the right internal carotid artery. Both women complained of abnormal throat sensations. Endoscopic studies and radiological examinations revealed tortuous right internal carotid arteries presenting as pulsatile masses. A literature review revealed that, in most reported cases, this deformity occurred on the right side. We believe that the defect and its right-sided predominance can be attributed to anatomical influences and factors affecting blood pressure.Conclusion:In most reported cases of tortuous internal carotid artery, the defect occurred on the right side and patients complained of an abnormal sensation in the throat. This information is useful in the diagnosis of this condition. It is important for otolaryngologists to recognise this anomaly, because fatal haemorrhage can occur in patients with this condition during surgical procedures on the pharynx.

Abstract Introduction. In gynecology, the proper selection of diagnostic and therapeutic methods, including conservative and/or surgical procedures, plays an important role. An outline of these issues is presented in the context of static disorders of female genital organs. The period of a woman’s life predisposing to pelvic organ prolapse is menopause, especially the period of senium, which is associated with hypoestrogenism. An analysis of the factors affecting the risk of disorders of genital statics indicates that pelvic floor exercises and a proper diet, which does not lead to constipation or obesity, are important in the prevention of genital static disorders. Aim. The aim of the study is to analyze the problem of disorders of the statics of genital organs, which is significant in nursing and medicine, in the light of the literature data.

The experience of the surgeon for all types of surggeries has a great influence on the patients' preference. In some surgical procedures patients prefer gender concordant surgeons like gynecological operations. These factors have been evaluated in many studies regarding different types of operations.

Factors affecting the expulsion resistance of an anterior cervical discectomy and fusion (ACDF) device in a cadaver model were investigated. Two specimen specific properties were considered; bone mineral density (BMD), and donor age. Also, two surgical procedures were used, representing two different degrees of destabilization of the intervertebral joint. Expulsion forces averaged 176 N (S.D. 60 N) for all specimens. No statistically significant difference in expulsion force was found between the specimens in the two surgical groups. However, expulsion force showed significant correlation to both BMD and donor age, with correlation coefficients of 0.81 and −0.83 respectively.
