INTRODUCTION: The Brazilian National Health System may reduce inequalities in access to health services through strategies that can reach those most in need with no access to care services. OBJECTIVE: To identify factors associated with the use of health service by children aged 5 to 9 years in the city of Sobral, Ceará, northeastern Brazil. RESULTS: Only 558 (17.0%) children used health care services in the 30 days preceding this survey. Children with any health condition (OR = 3.90) who were frequent attenders of primary care strategy of organization (the Family Health Strategy, FHS) (OR = 1.81) and living in the city's urban area (OR = 1.51) were more likely to use health services. Almost 80% of children used FHS as their referral care service. Children from poorer families and with easier access to services were more likely to be FHS users. CONCLUSION: The study showed that access to health services has been relatively equitable through the FHS, a point of entry to the local health system.

Abstract The objective of this study is to describe the profile of use of primary health care services, estimated by the PNS, of the population living in households registered and not registered with the Famly Health Strategy - FHS, in the years 2013 and 2019. Cross-sectional study carried out using microdata from national health surveys 2013 and 2019. The sample originated from a master sample, consisting of a set of units from selected areas in a register..The variables sex, age, skin color, income, education, self-perceived health, home registered with the FHS, medical care in the last year, type of service you seek when you are ill were selected. The dependent variables were use of health services and use of public health services. The dependent and independent variables were described with the respective confidence interval and adjusted logistic regression was performed for each outcome analyzed. In public health services, lower income, have chronic diseases (arterial hypertension or high cholesterol), be pregnant, and having a bad self-perception of health were associated with used more health services in both periods. Living in registered households was associated with more used health services (public or private). The family health strategy is an important strategy for expanding access equally.

ABSTRACTObjective: to check the knowledge of professionals in the Family Health Strategy on activities undertaken by the sector of control, evaluation, regulation and audit. Methodology: this is about a descriptive study from qualitative approach. It was applied a semi-structured interviews with three doctors and 16 nurses who are part of the team from Family Health Strategy in the municipality of Morada Nova/CE, in October 2007. This study was approved by the ethics committee of the Federal University of Ceara with the protocol number COMEP No. 208/07. Results: according to the statements, the study subjects understand the actions performed in the industry as a set of control actions, evaluating and monitoring the work of health professionals, but also to regulate users' access to health services. Most professionals are unaware of the current team that makes up the CARA of the municipality, however agree that the team should be composed of a multidisciplinary team, preferably with specialization in the area. Conclusion: let us consider how the actions performed by CARA may contribute to the improvement of the SUS, aiming at improving the care provided by health services to its users. Descriptors: clinical audit, family health, single health system. RESUMOObjetivo: verificar o conhecimento dos profissionais da Estratégia Saúde da Família sobre as atividades realizadas pelo setor de controle, avaliação, regulação e auditoria. Metodologia: estudo descritivo com abordagem qualitativa. Foi aplicado uma entrevista semi-estruturada com três médicos e 16 enfermeiros que fazem parte da equipe da Estratégia Saúde da Família no município de Morada Nova/CE, em outubro de 2007. Estudo aprovado pelo Comitê de Ética da Universidade Federal do Ceará com o número de protocolo Comepe nº 208/07. Resultados: de acordo com as falas, os sujeitos do estudo entendem as ações realizadas no setor como sendo um conjunto de ações de controle, avaliação e fiscalização do trabalho dos profissionais de saúde, como também de regulação do acesso dos usuários aos serviços de saúde. A maioria dos profissionais não tem conhecimento da atual equipe que compõe a CARA do município, entretanto concordam que a equipe deveria ser composta de uma equipe multidisciplinar, preferencialmente com especialização na área. Conclusão: considere-se o quanto as ações desempenhadas pela CARA podem contribuir para o aperfeiçoamento do SUS, visando à melhoria da assistência prestada pelos serviços de saúde aos seus usuários. Conclusión: vamos a considerar cómo las acciones realizadas por CARA puede contribuir a la mejora del SUS, con el objetivo de mejorar la atención recibida por los servicios de salud a sus usuarios. Objetivo: Conhecer a percepção dos profissionais da Estratégia Saúde da Família a cerca do acesso do homem aos serviços básicos de saúde. Método: Pesquisa exploratória e qualitativa desenvolvida com 16 profissionais da ESF, com aprovação no Comitê de Ética (CEP/UERN - n°052/11). Os dados foram discutidos através da análise temática de Minayo. Resultados: O trabalho é considerado uma barreira para o acesso dessa população aos serviços de saúde ou a continuidade de tratamentos já estabelecidos. Os profissionais responsabilizam o homem pela sua ausência no serviço. A não participação do homem está relacionada à ausência de planejamento e organização na ESF, falta de intersetorialidade e assistência pautada exclusivamente em ações ambulatoriais. Conclusão: identifica-se a ausência de estratégias que atraiam o homem a UBS, os desafios tornam-se evidentes diante da invisibilidade de práticas que favoreçam a procura pelos serviços de saúde. The study aimed to analyze the practices of the team in the Family Health Strategy, and how these guide the access to the health services. This was undertaken through qualitative research with systematic observation of the practice in three Family Health Centers as the data collection technique. The analyzing flowchart guided the analysis of the data. The results revealed that the access occurs, preferentially, through the family registration and the program actions directed towards specific groups. The procedure-centered practice guides the care in the Family Health Strategy, prioritizing the Emergency Room, depicted in emergency care. Some activities undertaken by the nurse were shown to be more susceptible to new therapeutic positions, shown by the service users' satisfaction. The study points to the need to advance in the re-signification of the practices, so as to strengthen improvement of access in the system's "gateway".

Introduction: Brazil is in the 19th position of priority countries for the control of TB/HIV coinfection, so we aimed to analyze the social and health services contexts that are associated with TB/HIV coinfection in São Paulo state. Methodology: Ecological study conducted in 645 cities of the state. The study population consisted of 10,389 new cases of TB/HIV coinfection in state residents between 2010 and 2015. The variables and indicators used in the study were collected from secondary sources. To identify the factors associated with the occurrence of TB/HIV coinfection cases, generalized additive models for location, scale and shape were used. The best distribution model was defined from the lowest Akaike information criterion value. Results: There was an association between the occurrence of coinfection and the diagnosis of TB after death and greater treatment default. There was also an association with greater coverage of nurses and Family Health Strategy, which comprises Primary Care settings focused on families. Regarding the social context, the Gini Coefficient of inequality was identified as a determinant of coinfection. Conclusions: The study presents the complexity of TB/HIV coinfection, proposing critical points in the health services and social context. Despite the high coverage of nurses and Family Health Strategy in some cities, this did not affect the reduction of the incidence of coinfection. These findings may be attributed to a fragmented care and focused on acute conditions. Furthermore, this model of care holds few prospects for care integration or prioritization of prevention and health promotion actions.

The objective of the study was to compare the use of medical and dental services by seniors residing at a seniors-only living facility and in the general community. It was a quantitative study, among 50 residents of the living facility and 173 in the general community. The data were collected between November 2011 and February 2012 through a questionnaire, and subjected to statistical analysis. Performance of clinical exams and satisfaction with health services was greater among seniors living in the general community; however, physical therapy treatment was more common among those living in the facility. The use of medical and dental services showed a statistically significant difference. The seniors in both groups need oral health monitoring and those living in the facility also require coverage by the Family Health Strategy. The presence of professionals with the right profile to adequately serve residents and the network of available services are determining factors for the success of this new housing policy.

A avaliação é uma importante ferramenta da atenção básica, devendo subsidiar a organização e orientação dos serviços de saúde, a partir de lógicas mais centradas no usuário. Esteartigo tem por objetivo apresentar a ótica dos usuários sobre os serviços de saúde inseridos no território de um núcleo de apoio à saúde da família. Os dados provêm de um estudo quantitativo-descritivo desenvolvido com 1023 famílias, cadastradas pela Estratégia Saúde da Família. Os resultados revelaram que 89,3% das famílias utilizam serviços do centro de saúde, tendo como principal motivo a busca por assistência em situação de doença ou dor (76,3%). Já no que tange a resolutividade, somente 9,1% relataram que quase nunca têm seu problema resolvido e 51,1% consideram bom o atendimento no centro de saúde. O artigo destaca, ainda, a satisfação do usuário como dimensão relevante na avaliação da atenção à saúde para o aprimoramento dos serviçose das políticas públicas em saúde.Palavras-chave: Avaliação de Serviços de Saúde; Atenção Primária à Saúde; Satisfação do PacienteEVALUATION OF HEALTH SERVICES IN THE USERS’ VIEWAbstract: The evaluation is an important tool of the primary care that should subsidize the organization and orientation of health services, from logics more centered in the user. The data comes from a quantitative-descriptive study developed with 1023 families registered by the Family Health Strategy.
