Background: A significant proportion of patients presenting with ST segment elevation myocardial infarction (STEMI) have newly diagnosed diabetes mellitus (DM). Hypothesis: Our aim was to identify patients with previously undiagnosed DM and compare their outcomes to those with known DM and without DM after STEMI. Methods: Consecutive patients undergoing primary PCI for STEMI at our center between Jan 2005 - Dec 2012 were included. Routinely performed admission Glycated hemoglobin (HbA1c) was utilized to identify patients with previously undiagnosed DM (HbA1c ≥ 6.5 and no history of DM or diabetes therapy). Patients were compared for in-hospital and long-term mortality based on follow up data from our institutional PCI registry. Results: 1,734 consecutive patients underwent primary PCI for STEMI and follow up data was available for 1,566 (90%) patients. Mean age was 60 years and 67.3% were males. A quarter of the patients (24.3%, n = 382) had prior history of DM and 8% (n=95) of the remainder had undiagnosed DM. Median follow up was 35 months. Mortality was comparable in patients with known DM and newly diagnosed DM both in hospital (11.2% vs. 12.5%, p=0.87) and at long term follow up (Figure 1, 2). Mortality was significantly worse with both groups when compared with patients with no DM (In-hospital mortality 5.6%; p<0.001 for both groups). Conclusions: One in twelve patients presenting with STEMI have previously undiagnosed DM. Cardiologists have a unique opportunity for identification and initiation of diabetic therapy in this vulnerable population. Patients with newly diagnosed DM have similar short and long-term outcomes when compared with patients with a prior history of DM.

Introduction. Vibration disease continues to occupy one of the leading places in the structure of professional pathology. In workers after the termination of contact with vibration generalization and progression of violations in an organism is noted. The pathogenetic mechanisms of the progredient course of disturbances in the nervous system in the post-contact period of vibration exposure remain insufficiently studied.The aim of the study was to test an experimental model of vibration exposure to assess the neurophysiological and morphological effects of vibration in rats in the dynamics of the post-contact period.Materials and methods. The work was performed on 168 white male outbred rats aged 3 months weighing 180–260 g. The vibration effect was carried out on a 40 Hz vibrating table for 60 days 5 times a week for 4 hours a day. Examination of animals was performed after the end of the physical factor, on the 30th, 60th and 120th day of the post-contact period. To assess the long-term neurophysiological and morphofunctional effects of vibration in rats, we used indicators of behavioral reactions, bioelectric activity of the somatosensory zone of the cerebral cortex, somatosensory and visual evoked potentials, parameters of muscle response, morphological parameters of nervous tissue.Results. In the dynamics of the post-contact period observed the preservation of violations of tentatively research, motor and emotional components of behavior. In the Central nervous system instability of activity of rhythms of an electroencephalogram, decrease in amplitude of visual evoked potentials, lengthening of latency of somatosensory evoked potentials, decrease in total number of normal neurons and astroglia is established. In the peripheral nervous system remained changes in indicators: increasing duration and latency, reducing the amplitude of the neuromuscular response.Conclusions: The experimental model allows us to study the long-term neurophysiological and morphological effects of vibration on the body. The formation and preservation of changes in behavioral activity, neurophysiological and morphological effects of vibration from the 30th to the 120th day of the post-contact period were confirmed.

AbstractThis study aimed to assess the role of multifocal visual-evoked potentials (mfVEPs) as a guiding factor for clinical conversion of radiologically isolated syndrome (RIS). We longitudinally followed a cohort of 15 patients diagnosed with RIS. All subjects underwent thorough ophthalmological, neurological and imaging examinations. The mfVEP signals were analysed to obtain features in the time domain (SNRmin: amplitude, Latmax: monocular latency) and in the continuous wavelet transform (CWT) domain (bmax: instant in which the CWT function maximum appears, Nmax: number of CWT function maximums). The best features were used as inputs to a RUSBoost boosting-based sampling algorithm to improve the mfVEP diagnostic performance. Five of the 15 patients developed an objective clinical symptom consistent with an inflammatory demyelinating central nervous system syndrome during follow-up (mean time: 13.40 months). The (SNRmin) variable decreased significantly in the group that converted (2.74 ± 0.92 vs. 4.07 ± 0.95, p = 0.01). Similarly, the (bmax) feature increased significantly in RIS patients who converted (169.44 ± 24.81 vs. 139.03 ± 11.95 (ms), p = 0.02). The area under the curve analysis produced SNRmin and bmax values of 0.92 and 0.88, respectively. These results provide a set of new mfVEP features that can be potentially useful for predicting prognosis in RIS patients.
