Police Community Support Officers: Cultures and Identities within Pluralized Policing presents the first in-depth ethnographic study of Police Community Support Officers (PCSOs) since the creation of the role in 2002. Situated within the tradition of police ethnographies, this text examines the working worlds of uniformed patrol support staff in two English police forces. Based on over 350 hours of direct observation and thirty-three interviews with PCSOs and police constables in both urban and rural contexts, the book offers a detailed analysis of the operational and cultural realities of pluralized policing from within. Using a dramaturgic framework, the author finds that PCSOs have been undermined by their own organizations from the beginning, which has left a lasting legacy in terms of their relationships and interactions with police officer colleagues. The implications of this for police cultures, community policing approaches, and the success of pluralization are examined. The author argues that while PCSOs can have similar occupational experiences to those of constables, their particular circumstances have led to a unique occupational culture, one which has implications for existing police culture theories. The book considers these findings in light of budget reductions and police reforms occurring across the sector, processes in which PCSOs are particularly vulnerable.

Introduction: Cigarette smoking is a leading global cause of morbidity and mortality. Multiple studies internationally have established that cigarette smoking prevalence is higher in emergency department (ED) patients than their respective communities. Previously, we demonstrated the smoking prevalence among Saskatoon ED patients (19.6%) is significantly higher than the provincial average (15.1%), and over 50% of smoking patients would be receptive to ED-specific cessation support. The purpose of this project was to identify nurses’ beliefs regarding smoking cessation in the ED, and barriers to implementing it in the department. Methods: A questionnaire was administered to all nurses employed at St. Paul's Hospital ED in Saskatoon assessing attitudes towards ED cessations, as well as the benefit and feasibility of three potential interventions: brief cessation counselling, referral to community support programs, and distributing educational resources. The questionnaire included Likert scale numerical ratings, and written responses for thematic analysis. Thematic analysis was performed by creating definitions of identified themes, followed by independent review of the data by researchers. Results: 83% of eligible nurses completed the survey (n = 63). Based on Likert scores, ED nurses rarely attempt to provide cessation support, and would be minimally comfortable with personally providing this service. Barriers identified through thematic analysis included time constraints (68.3%), lack of patient readiness (19%), and lack of resources/follow-up (15.9%). Referral to community support programs was deemed most feasible and likely to be beneficial, while counselling within the ED was believed to be least feasible and beneficial. Overall, 93.3% of nurses indicated time and workload as barriers to providing ED cessation support during the survey. Conclusion: Although the ED is a critical location for providing cessation support, the proposed interventions were viewed as a low priority task outside the scope of the ED. Previous literature has demonstrated that multifaceted ED interventions using counselling, handouts, and referrals are more efficacious than a singular approach. While introduction of a referral program has some merit, having professionals dedicated to ED cessation support would be most effective. At minimum, staff education regarding importance of providing smoking cessation therapy, and simple ways to incorporate smoking cessation counselling into routine nursing care could be beneficial.

Purpose – Attachment Theory can be regarded as central to the concept of relational security. There is a paucity of research examining the coherence of this construct for ward-based staff. The paper aims to discuss these issues. Design/methodology/approach – Five female nurses from the acute admission and assessment ward of a UK medium secure unit acted as participants. Semi-structured interviews were conducted, and inductive thematic analysis was applied. Findings – Six themes; “staff-service user relationships”, “staff diversities”, “service user backgrounds”, “variability in service users’ presentations”, “service users with personality disorder are problematic” and “nurses do not use attachment” emerged from the data. The nurses used heuristic models of attachment-related behaviour and they lacked knowledge of constructs associated with Attachment Theory. Research limitations/implications – Acute admissions may not be representative of all treatment contexts. Traditional models of attachment style may have only limited relevance in forensic services. Practical implications – Limited knowledge and confidence in the nurses regarding how Attachment Theory might apply to service users is interesting because it may limit the extent to which care, treatment and risk management might be informed by an understanding of service user representations of therapeutic relationships. Training and educational interventions for nurses that enhance understanding of personality development and attachment styles are warranted. Originality/value – The importance of nurses for achieving relational security is emphasised and the adequacy of their training is questioned.

Purpose Evidence-based psychological therapies are available for severe and enduring mental health problems, but resources and access to these are limited within England. Practitioners in community mental health teams (CMHTs) can act as gatekeepers for access to psychological therapies for those in secondary care, but little is known about how they make referral decisions. This paper aims to understand how CMHT practitioners make decisions about who to refer or not, to secondary care psychological therapy services (PTS). Design/methodology/approach A total of 11 CMHT practitioners were interviewed to understand the decision making processes underpinning their referrals or otherwise, to a PTS within NHS England. The data were analysed qualitatively using thematic analysis. Findings Thematic analysis resulted in 11 sub-themes under three main themes of the self, the organisation and wider structure and the service user. Results indicated that some participants were referred automatically for psychological therapy if a service user asked or if there was external pressure to refer, while others’ decisions were informed by contextual information such as the service user’s ability to engage or change, risk status and limited organisational resources. Originality/value This study explores the decision making of multi-disciplinary professionals referring to PTS. The findings have important implications for understanding some of the factors that can influence patient access to psychological treatment in secondary care.

Background: The Who’s Challenging Who? (WCW) training is coproduced and delivered by people with intellectual disabilities (IDs), and it aims to improve staff empathy for people with challenging behaviour (CB). This study qualitatively describes trainees’ and trainers’ experiences of the WCW. Methods: Semi-structured interviews were undertaken with managers ( n = 7), support staff ( n = 6) and the WCW trainers ( n = 4; three had IDs). Interviews were transcribed verbatim and analysed using thematic analysis. Results: Two cross-cutting themes were drawn from the data: (1) valued roles of the trainers, whereby trainers and trainees benefited from the training being coproduced and delivered by people with IDs, and (2) beyond the training, within which trainees reported that they were engaging in increased reflection about their past and current practice. Conclusions: Being trained by people with IDs and CB appears to be a useful method, which can lead to perspective taking and reflection about supporting people with IDs and CB.

This paper examines business philosophies, sustainable practices, and attitudes towards eco-certification of pio-neering tourism operators engaged in ecotourism in the mature ‘sun and beach’ destination of Jamaica. We conducted a thematic analysis of the data collected from semi-structured interviews with managers and owners, and observations from on-site visits with establishments identified as ecotourism operators in Jamaica. We identified five distinct busi-ness philosophies that guided our participants' operations: presenting Jamaica in a holistic way, preserving the natural and cultural environment, community support, economic sustainability and incentivized preservation, and operational independence. The tourism operations in our study primarily engage in sustainable practices as a means of attaining operational independence. Meanwhile, participants held mixed attitudes towards eco-certification. There was signifi-cant overlap between our findings and the existing conceptual UNWTO framework for ecotourism. We found strong evidence for the role of education as the defining characteristic of ecotourism. Varying attitudes towards and awareness of sustainability and conservation of the country’s cultural and natural assets limit the amount of community consulta-tion in planning and development of these enterprises.

This study investigates the experiences of victims of domestic violence (DV) involved in a bilateral electronic monitoring (EM) program. Semi-structured interviews were conducted with six victims whose associated person of interest participated in an EM program post-release, as well as 13 victim support staff. Thematic analysis revealed seven themes: (1) Safety and validation, (2) Initial anxiety, (3) Minimal intrusion on daily life, (4) Psychological relief and feelings of safety, (5) Freedom to engage in daily activities, (6) Post-EM concerns for safety, and (7) An effective deterrent for some, but not for all. Overall, the experiences reported by victims and support staff were positive and evident of victim-centricity. The main defining experience of the DVEM program for victims was improved feelings of safety during the program and increased autonomy and confidence in going about their daily activities. However, there is an urgent need to consider post-EM safety of victims.

Purpose The purpose of this paper is to present an evaluation of a programme of training and support provided to staff, which aimed to encourage supported communication environments for people with learning disabilities. Design/methodology/approach Training, monitoring and support for communication, specifically augmentative and alternative communication (AAC) strategies, was provided by speech and language therapy staff to two residential services over 46 weeks. Staff and service user communications were observed pre- and post-intervention. Findings In one provision there was an increase in service user initiations and the use of some AAC strategies by support staff. In the other provision there was no change in service user initiations and a decrease in the range of AAC strategies used. It appears that some forms for AAC remain challenging for staff to implement. Originality/value This evaluation explores ways of using specialist support services to improve communication environments for people with learning difficulties. Possible reasons for differences in the outcome of the intervention are discussed. Future research into the types of communication interactions experienced by people with learning disabilities across the range of communication styles may be useful so that support staff can be better helped to provide sustained and enriched communication environments.

Abstract Background Research on hospital resilience is scarce and favours top-down approaches, and evidence from the field and operational levels is lacking. The aim of our study was to understand the mechanisms of hospital and individual resilience experienced by staff from a tertiary hospital in Kathmandu, Nepal, after the 2015 Earthquake. Methods We conducted semi-structured interviews with different professionals in May 2018; undertook a deductive thematic analysis of hospital resilience using the framework proposed by Bruneau et al; and used an inductive thematic analysis for individual resilience. Results The earthquake caused different types of burden to the hospital and individuals. Redundancy was mostly influenced by linkages with lower levels of care. Resourcefulness consisted mostly of spontaneous adaptations, with task shifting the most important for human resources. External provision of resources enabled the transfer of part of the burden but precipitated accountability-related challenges. The hospital's robustness depended not only on its physical resistance, but also on the functional capacity to provide life saving care, although routine services were interrupted and quality decreased. The hospital ensured rapidity in providing life saving care to the victims and re-start of routine activities, but recovery to a sense of normality was more conflicting and dependent on the individual. Individual resilience was dictated by safety, a sense of meaningfulness, self and external appreciation, and community support. Conclusions Our study shows the importance of staff experiences to improve hospital resilience. Disaster plans should acknowledge the role of task shifting, and basic care should be taught to all, not just those with clinical functions. Health workers are extremely overwhelmed during disaster response, and disaster plans should engage staff at an early stage to ensure they feel safe and sufficiently supported. Key messages Our work is one of the first to study hospital resilience with field data from qualitative interviews with hospital staff. Hospital resilience strategies should also address individual resilience of hospital staff.

Strength and conditioning (S&C) is implemented across various sports and levels, but there is limited understanding of the personnel responsible for this, including their S&C practices and perspectives. Whereas recent evidence has shown that coaches and players are often tasked with the responsibility. Therefore, this study aimed to investigate: 1) the personnel responsible for delivering S&C across different levels of soccer, 2) the practices and perspectives of soccer coaches and players, and 3) to ascertain whether the practices employed align with contemporary evidence and guidelines. Forty-two soccer coaches and 30 soccer players completed an online survey with six sections: (a) informed consent, (b) background information, (c) education, qualifications, and prescription, (d) views on S&C, (e) exercise selection and preferences, and (f) issues and improvements. Frequency analysis was used to report responses to fixed-response questions and thematic analysis for open-ended questions. Most respondents reported S&C to be ‘important’ to ‘very important’ for all soccer, physical fitness, and injury parameters, with perspectives being predominantly aligned with S&C guidelines and research in soccer. Although S&C coaches were mostly responsible for delivering S&C sessions, over 60% of respondents disclosed S&C sessions were delivered independently or by support staff. This is problematic given only four coaches held S&C qualifications, and issues and improvements were mostly regarding a lack of S&C expertise and education. This study provides valuable information for S&C and soccer organizations. Particularly regarding the additional support required to ensure those responsible for delivering S&C in soccer have the appropriate knowledge and qualifications.
