Windscale Pile 1: A New Approach

One of the most technically challenging reactor decommissioning projects in the UK, if not the world, is being tackled in a new way managed by a team lead by the United Kingdom Atomic Energy Authority. Windscale Pile 1, a graphite moderated, air cooled, horizontal, natural uranium fuelled reactor was damaged by fire in October 1957. De-fuelling, initial clean-up and isolation operations were carried out in the 1960’s. During the 1980’s and 90’s a successful Phase1 decommissioning campaign resulted in the plant being cleared of all accessible fuel and graphite debris and it being sealed and isolated from associated facilities and put on a monitoring and surveillance regime while plans for dismantling were being developed. For years intrusive inspection of the fire damaged region has been precluded on safety grounds. Consequently early plans for dismantling were constructed using pessimistic assumptions and worst case predictions. This in turn lead to technical, financial and regulatory hurdles which were found to be too high to overcome. The new approach utilises the best from several areas: • The design process incorporates principles of the US DoE safety analysis process to address safety, and adds further key stages of design concept and detail to generate concurrent development of a technical solution and a safety case. • A staged and gated Project Management Process provides for stakeholder involvement and consensus at key stages. • Targeted knowledge acquisition is used to minimise uncertainty. • A stepwise approach to intrusive surveys is employed to systematically increase confidence. The result is a process which yields the optimum solution in terms of safety, environmental impact, technical feasibility, political acceptability and affordability. The change from previous approaches is that the project starts from the hazards and associated hazard management strategies, through engineering concept, to design manufacture and testing of the resulting solution rather than starting with the engineer’s “good idea” and then trying to make it work, safely and at an affordable price. Progress has been made in making the intrusive survey work a reality. This is a significant step in building a realistic picture of the physical and radiological state of the core and in building confidence in the process.

This study investigates human enteric virus contamination of a shellfish harvesting area. Samples were analysed over a 14-month period for Small Round Structured Viruses (SRSVs) using a previously developed nested RT-PCR. A clear seasonal difference was observed with the largest numbers of positive samples obtained during the winter period (October to March). This data concurs with the known winter association of gastroenteric illness due to oyster consumption in the UK and also with the majority of the outbreaks associated with shellfish harvested from this area during the study period. RT-PCR positive amplicons were further characterised by cloning and sequencing. Sequence analysis of the positive samples identified eleven SRSV strains, of both Genogroup I and Genogroup II, occurring throughout the study period. Many shellfish samples contained a mixture of strains with a few samples containing up to three different strains with both Genogroups represented. The observed common occurrence of strain mixtures may have implications for the role of shellfish as a vector for dissemination of SRSV strains. These results show that nested RT-PCR can identify SRSV contamination in shellfish harvesting areas. Virus monitoring of shellfish harvesting areas by specialist laboratories using RT-PCR is a possible approach to combating the transmission of SRSVs by molluscan shellfish and could potentially offer significantly enhanced levels of public health protection.

Finding locally optimal solutions for MAX-CUT and MAX- k -CUT are well-known PLS-complete problems. An instinctive approach to finding such a locally optimum solution is the FLIP method. Even though FLIP requires exponential time in worst-case instances, it tends to terminate quickly in practical instances. To explain this discrepancy, the run-time of FLIP has been studied in the smoothed complexity framework. Etscheid and Röglin (ACM Transactions on Algorithms, 2017) showed that the smoothed complexity of FLIP for max-cut in arbitrary graphs is quasi-polynomial. Angel, Bubeck, Peres, and Wei (STOC, 2017) showed that the smoothed complexity of FLIP for max-cut in complete graphs is ( O Φ 5 n 15.1 ), where Φ is an upper bound on the random edge-weight density and Φ is the number of vertices in the input graph. While Angel, Bubeck, Peres, and Wei’s result showed the first polynomial smoothed complexity, they also conjectured that their run-time bound is far from optimal. In this work, we make substantial progress toward improving the run-time bound. We prove that the smoothed complexity of FLIP for max-cut in complete graphs is O (Φ n 7.83 ). Our results are based on a carefully chosen matrix whose rank captures the run-time of the method along with improved rank bounds for this matrix and an improved union bound based on this matrix. In addition, our techniques provide a general framework for analyzing FLIP in the smoothed framework. We illustrate this general framework by showing that the smoothed complexity of FLIP for MAX-3-CUT in complete graphs is polynomial and for MAX - k - CUT in arbitrary graphs is quasi-polynomial. We believe that our techniques should also be of interest toward showing smoothed polynomial complexity of FLIP for MAX - k - CUT in complete graphs for larger constants k .

Soil health connotes the balance of biological, physicochemical, nutritional, structural, and water-holding components necessary to sustain plant productivity. Despite a substantial knowledge base, achieving sustainable soil health remains a goal because it is difficult to simultaneously: (i) improve soil structure, physicochemistry, water-holding capacity, and nutrient cycling; (ii) suppress pests and diseases while increasing beneficial organisms; and (iii) improve biological functioning leading to improved biomass/crop yield. The objectives of this review are (a) to identify agricultural practices (APs) driving soil health degradations and barriers to developing sustainable soil health, and (b) to describe how the nematode community analyses-based soil food web (SFW) and fertilizer use efficiency (FUE) data visualization models can be used towards developing sustainable soil health. The SFW model considers changes in beneficial nematode population dynamics relative to food and reproduction (enrichment index, EI; y-axis) and resistance to disturbance (structure index, SI; x-axis) in order to identify best-to-worst case scenarios for nutrient cycling and agroecosystem suitability of AP-driven outcomes. The FUE model visualizes associations between beneficial and plant-parasitic nematodes (x-axis) and ecosystem services (e.g., yield or nutrients, y-axis). The x-y relationship identifies best-to-worst case scenarios of the outcomes for sustainability. Both models can serve as platforms towards developing integrated and sustainable soil health management strategies on a location-specific or a one-size-fits-all basis. Future improvements for increased implementation of these models are discussed.

BackgroundBronchiolitis is a major cause of admission to hospital in children. Non-invasive ventilation (NIV) support with continuous positive airway pressure (CPAP) or high-flow nasal cannula (HFNC) oxygen is routinely used for infants in the UK with bronchiolitis.ObjectiveTo establish UK paediatric practice regarding management of bronchiolitis, and to explore issues pertinent to the design of a potential future randomised controlled trial of NIV.DesignScreening logs were completed in hospitals in England capturing information on paediatric bronchiolitis admissions. An online national survey of clinical practice was disseminated to healthcare professionals (HCPs) across the UK to ascertain current management strategies.ResultsScreening logs captured data on 393 infants from 8 hospitals. Reasons for admission were most commonly respiratory distress and/or poor fluid intake. Oxygen was administered for 54% of admissions. Respiratory (CPAP and HFNC) and non-respiratory support administered varied considerably. The national survey was completed by 111 HCPs from 76 hospitals. Data were obtained on criteria used to commence and wean NIV, responsibilities for altering NIV settings, minimum training requirements for staff managing a child on NIV, and numbers of trained staff. Most centres were interested in and capable of running a trial of NIV, even out of normal office hours.ConclusionsRespiratory and non-respiratory management of bronchiolitis in UK centres varies widely. A trial of HFNC oxygen therapy in this group of patients is feasible and HCPs would be willing to randomise patients into such a trial. Future work should focus on defining trial eligibility criteria.

IntroductionThe UK Department of Health have targeted a reduction in stillbirth by 50% by 2025; to achieve this, the first version of the Saving Babies’ Lives Care Bundle (SBLCB) was developed by NHS England in 2016 to improve four key areas of antenatal and intrapartum care. Clinical practice guidelines are a key means by which quality improvement initiatives are disseminated to front-line staff.MethodsSeventy-five clinical practice guidelines covering the four areas of antenatal and intrapartum care in the first version of SBLCB were obtained from 19 maternity providers. The content and quality of guidelines were evaluated using the Appraisal of Guidelines for Research and Evaluation (AGREE II) tool. Maternity health professionals in participating organisations were invited to participate in an anonymous survey to determine perceptions toward and experiences of the use of clinical practice guidelines using a series of Likert scales.ResultsUnit guidelines showed considerable variation in quality with median scores of 50%–58%. Only 4 (5.6%) guidelines were recommended for use in clinical practice without modifications, 54 (75.0%) were recommended for use subject to modifications and 12 (16.7%) were not recommended for use. The lowest scoring domains were ‘rigour of development’, ‘stakeholder involvement’ and ‘applicability’. A significant minority of unit guidelines omitted recommendations from national guidelines. The majority of staff believed that clinical practice guidelines standardised and improved the quality of care but over 30% had insufficient time to use them and 24% stated they were unable to implement recommendations.ConclusionTo successfully implement initiatives such as the SBLCB change is needed to local clinical practice guidelines to reduce variation in quality and to ensure they are consistent with national recommendations . In addition, to improve clinical practice, adequate time and resources need to be in place to deliver and evaluate care recommended in the SBLCB.

<p>Peatlands are globally valued for the ecosystem services they deliver, including water quality regulation and carbon sequestration. In the UK, blanket bogs are the main peatland habitat and previous work has linked blanket bog management, especially rotational burning of heather vegetation on grousemoors, to impacts on these ecosystem services. However, we still lack a mechanistic, process-level understanding of how peatland management and habitat status is linked to ecosystem service provision, which is mostly driven by soil microbial processes.</p><p>Here we examine bacterial and fungal communities across a spectrum of “intact” to degraded UK blanket bogs and under different vegetation management strategies. Sites included grousemoors under burnt and alternative mown or uncut management along with further locations including 'near intact', degraded and restored sites across a UK climatic gradient ranging from Exmoor (South UK), the Peak District (Mid) to the Flow Country (North). Moreover, an experiment was setup at the University of York with peat mesocosms taken from all sites and management/habitat conditions to allow a comparison between field and controlled conditions and assessing root-mediated processes. Using a structural equation model, we linked grousemoor management to specific fungal/bacterial functional groups, and have started to relate this to changes in water quality provision and carbon cycle aspects. This represents a significant step in the effort to use microbial communities as indicators of peatland habitat condition in UK upland blanket bogs. </p><p> </p>

Industry-led culling of badgers has occurred in England to reduce the incidence of bovine tuberculosis in cattle for a number of years. Badger vaccination is also possible, and a move away from culling was “highly desirable” in a recent report to the UK government. Here we used an established simulation model to examine badger control option in a post-cull environment in England. These options included no control, various intermittent culling, badger vaccination and use of a vaccine combined with fertility control. The initial simulated cull led to a dramatic reduction in the number of infected badgers present, which increased slowly if there was no further badger management. All three approaches led to a further reduction in the number of infected badgers, with little to choose between the strategies. We do note that of the management strategies only vaccination on its own leads to a recovery of the badger population, but also an increase in the number of badgers that need to be vaccinated. We conclude that vaccination post-cull, appears to be particularly effective, compared to vaccination when the host population is at carrying capacity.

The authors present an excellent retrograde analysis of a rare condition of a phenomenal number of cases and their surgical outcomes. A majority of the studies in published literature are anecdotal case reports which are a rare and dreadful entity. A comprehensive countrywide view of the UK National Adult Cardiac Surgery Audit database is presented in this study. This study represents the changing trends in the risk factors, management strategies, and outcomes of ventricular septal rupture for over 23 years in a nutshell.

Abstract Production studies have become popular over the past decade. Recent studies have analysed, amongst other things, innovation, management strategies and the effects of convergence on editorial processes. There have only been a few studies that have analysed what happened inside media organisations in the earlier transformative stages (outside the UK and the USA). This paper analyses how the Norwegian public service broadcaster (NRK) adapted to the loss of its monopoly and the beginning of competition during the mid-1980s. It provides a window into how the flagship of public service, the Enlightenment Department, dealt with the new situation. If one follows the production process of the main programme of the department (with the revealing working title ‘Flagship’) from its conception to its realisation as a weekly programme broadcast in prime time, this reveals how innovation at the time was restricted by organisational arrangements, internal values and external pressures. The programme makers included many elements that are still today considered to be advantageous in factual programming (humour, dramatization, popularisation, serialization, recognition, and even interactivity). Along the way several of these were changed: what had started as a proposal for a documentary series turned into something that was predominantly a discussion programme.

Abstract. The STAR-FLOOD (Strengthening and Redesigning European Flood Risk Practices Towards Appropriate and Resilient Flood Risk Governance Arrangements) project, funded by the European Commission, investigates strategies for dealing with flood risk in six European countries: Belgium, the UK, France, the Netherlands, Poland and Sweden and in 18 vulnerable urban regions in these countries. The project aims to describe, analyse, explain, and evaluate the main similarities and differences between the selected EU Member States in terms of development and performance of flood risk governance arrangements. It also discusses the scientific and societal importance of these similarities and differences. Attention is paid to identification and characterization of shifts in flood risk governance arrangements and in flood risk management strategies and to determination of triggering factors and restraining factors. An assessment of a change of resilience and appropriateness (legitimacy, effectiveness, efficiency) of flood risk governance arrangements in Poland is presented and comparison with other European countries is offered.
